Services, Inc. for inclusion in the United States Conference of Catholic Bishops group tax
exemption ("Group Ruling’) and listing in the 2009 edition of the Official Catholic Directory
((OCD"). Based upon the application and supporting documentation, Servants of Saint
Camillus Disaster Relief Services, Inc. has been approved for inclusion in the Group Ruling
and listing in the OCD under the Archdiocese of Milwaukee.
This letter establishes Servants of Saint Camillus Disaster Relief Services, Inc.'s exemption
from federal income tax under section 501(c)(3) of the Internal Revenue Code ("Code") in the
interim until publication of the 2009 OCD. Servants of Saint Camillus Disaster Relief
Services, Inc. has qualified as a public charity under the provisions of the Code checked below:

qualifies for a mandatory or discretionary exception under section 6033 of the Code. Servants
of Saint Camillus Disaster Relief Services, Inc. should contact its own tax advisor to
determine whether it is required to file Form 990.
Please inform this office promptly of any changes in Servants of Saint Camillus Disaster
Relief Services, Inc.’s name, address, or corporate form, and of any material changes in its
purposes, activities, or sources of support.
