Contractor Safety Web Application.

Access CSW at:

4 Main Functions:
Enter Contractor Headcount/Hours
Enter Contractor Injury/Illness Information
Find Contractor Injury/Illness Case
Delete Injury/Illness Case
Internet Access
(Intel network connection is not required)
Intel Network Account
Suppliers needing an account must request one
through their Intel Sponsor
CSW Account
Apply for an account at:
http://ehs.intel.com/construction/cswa_account.htm
Select Login
https://supplier.intel.com/fctweb/index.aspx
Enter Logon ID (Include Domain Name)
Enter NT Password
Then Click on the SUBMIT Button
Required fields are
marked with a Red
Asterisk (*). Select
the Appropriate
Fields from the
Drop-Down () box
provided.
Sub Project: Select Sub Project.
Contractor: Select Contractor.
Date of Injury: Enter Date or Select Date from Calendar.
Injury Category: Select Appropriate Category. Consult your EHS Professional if
you are unsure.
Injury Classification: Select Injury Classification.
Primary Body Part: Select only ONE Body Part, even though multiple body
parts are involved. Consult your EHS Professional if you are unsure.
Root Cause: Select Root Cause.
Functional Area Group: Select Functional Area Group.
Functional Area: Select Functional Area.
Description of Injury/Investigation Results: Enter a Detailed Description of
the Injury/Investigation Results. Enter as much text as you need to.
Once completed, click SAVE Button.

When complete click
the SAVE Button.
Note: The WWID of
the person who
last edited this
report and the last
Date Updated is
visible near the
bottom of the
screen. These
fields are system
generated, and
cant be changed.
To find an Injury Case, click on
the Find Injury Case link.
Required fields are marked with a Red Asterisk (*).
Select the Appropriate Fields from the Drop-Down ()
box provided. Select the appropriate Injury Start and
Injury End Dates. When complete select the Go Button.

When complete select the Go Button.
Sub Project: Select the Appropriate Sub Project.
Contractor: Select Contractor.
Date of Injury Start: Enter Date or Select Date from Calendar.
Date of Injury End: Enter Date or Select Date from Calendar.
Once completed, click GO Button.
If you do not find your Sub Project or Contractor on the Website:

Please send completed forms to the EHS Account Requests mailbox

All records
matching the
selected
criteria will be
displayed.
Select the Sub
Project, Contractor
and Date Range of
Injury you are
searching for.
To Edit a Particular Record, Click on
the Applicable Date of Injury link.
Edit the
necessary
Fields, click the
Save Button
when you are
finished.
Note: Deleting a record
is Permanent; once
confirmed the record
cant be recovered.
To completely
delete an Injury
Case, click the
Delete Button and
Select OK in the
Message Box.
After Deleting or Editing a Record, youll be
returned to the Find Injury Case screen.

TO REQUEST ACCESS FOR THE FCTWEB:
Send completed forms to the EHS Account Requests mailbox at ehs.account.request@intel.com
INSTRUCTIONS FOR LOGGING INTO FCTWEB:
1) Go to this URL: https://supplier.intel.com/fctweb/default.asp
2) Click on the Login Link
3) Enter your IDSID referred to as NT Logon Name (e.g. AMR\jdoe) click SUBMIT
4) Enter your NT password
5) That's it! You should be in.
RIALTO PASSWORD INSTRUCTIONS:
1) NT Passwords need to be updated every 90 days.
2) Go to this site on the Intel Network: https://rialtoi.intel.com/default.asp
3) Select the 2nd option Synchronize Rialto password with Windows Domain Password to
populate your current NT/Windows Domain Password out to all the Intel Networks. After you do that you
should be able to access the FCT Website.
