Appendix I
Model Form of Daywork/Daily Record Sheets
Sub-Clause 13.6 Daywork requires the Contractor to submit each day to
the Engineer details of resources utilised.
Additionally, there are many instances in the Contract where the keeping
(and agreement) of daily records is crucial if the Contractor is to be properly
reimbursed for his efforts. This applies particularly to those events where the
Contractor is entitled to payment of Cost. Further, there will be other
occasions, particularly relating to claims and their quantification, where
accurate records of actual events would be beneficial.
Following is a proposed standard format which can be varied to suit the
requirements of an individual site. These daywork/daily record sheets should
be colour-printed in groups of five and bound in sets of 100 (20 5).
Each set of five would have a unique sequential number. This would help
identify records that are not signed, not presented or simply lost in the
system.
The top copy and one other would be taken by the Engineer and the next
two copies by the Contractors site office. These four copies would be
perforated for separation. A fifth copy, unperforated, would be kept in the
Contractors field office responsible for producing the record sheets.
It may happen that the Contractors supervisory staff are not fluent in the
language of the Contract. Provided that Badge No./Fleet No./Material
Codes are correctly recorded, together with the relevant quantities, the form
can be conveniently completed by a junior staff member with the correct
language skills.
The valuation of the individual sheets takes place in the Contractors site
office. The quantum, whether for daywork or other purposes, can be sent to
the Engineer as part of the monthly application for an Interim Payment
Certificate or as part of a claim presentation, accompanied by a copy of the
original, signed daywork/daily record sheet.
