5. Briefly describe the childs Present Levels of Academic Achievement and Functional Performance. Include what the child can
and cannot do.
Patti is described as a happy child who aims to please her parents. She is very social around family and seeks to help out.
Patti is able to assist in dressing and undressing herself. She can use to restroom and feed herself unassisted. Patti is
able to rote count to ten, identify some colors, shapes, numbers, and letters of the alphabet. She can also identify basic
body parts and is aware of some spatial concepts. Patti however writes with a modified grasp. She tends to hold the
scissors thumb down and struggles to manipulate the paper when cutting. Patti also has difficulties with articulation and
receptive language. She tends to omit the sounds |k|, |g|, and |f| in all positions of words. Socially Patti has difficulty
interacting with peers. She doesnt like to be around them and will remove herself from the situation. Patti may at times
become aggressive or angry but knows how to self-regulate.

Goal 1: Socialization
Standard: ELS-CON2-02- The child demonstrates the ability to give and take during social interactions
To meet the goal, when a child initiates an interaction with Patti she will remain with the peer for a minimum of five
minutes with minimal adult assistance three out of four opportunities per week consistently over six consecutive weeks.
Goal 2: Fine Motor
Standard: PHSS-CON3-01- The child uses fingers and hands to manipulate tools and materials.

To meet the goal Patti will independently use scissors to cut across a six inch piece of sturdy paper, such as card stock,
using spring loaded scissors independently a minimum of one time per week.
7. What accommodations does the child need when in the general education classroom, Accommodations Page, Form E?
Patti will be provided with visual and verbal cues when answering wh questions. Patti will be allowed to use spring
loaded scissors when cutting heavy stock paper. Patti will be allowed to interact with small groups of children, no more
than two or three, during centers and outdoor play.
