INFORMATION
The undersigned Senior Assistant City Prosecutor, upon sworn complaint originally filed
by the offended party, accuses ________________________ of the crime of ESTAFA,
committed as follows:
That on or about and during the period comprised between May 2014 and June 2014 in
the city of Naga, province of Camarines Sur, Philippines, within the jurisdiction of this court, the
said accused, did then and there willfully, unlawfully and feloniously defraud
_________________ as follows: The said accused under false and fraudulent representation
which he made to said __________________ to the effect that he would secure for her, a US
Visa if she would deliver to him the amount of P69,800.00 to cover the cost of processing fee
and the plane ticket supposedly in a cheaper price and by means of other similar deceit, which
representations he well knew were false and fraudulent and were only made to induce the
aforementioned _________________ to give and deliver as in fact the said _________________
gave and delivered the said total amount of P69,800.00, and once in possession of said amount,
willfully, unlawfully and feloniously misappropriated, misapplied and converted the said amount
of P69,800.00 to his own personal use and benefit, to the damage and prejudice of the said
___________.
