This is to explain my absence last Feb.25,2016 at the OB ward,
Benguet General Hospital ,11-7 shift under the supervision of Mr. Joshua Faustino.
I was absent because I was not feeling well and having a
headache. I felt that I really cant attend my duty and had to rest and take in
