ENVIRONMENTAL Since 69/10 | Reportable Releases None Exceedences ‘06/21/70 — Area 6 Fuel Drums HaS>160 ppm, S-hour avg due to lean amine pump malfunction. Sames as 5/12/40 and 5/25/10. (06/28/10 - #2 FCCU CO >500 pom, T-hr avg due to low sleam header pressure during heavy rain. ‘07/27I10 — #2 FCCU CO >500 ppm, T-hr avg (10 hours) due to faulty servo on Flue Gas Slide Valve (08/01/10 = #2 FCCU CO >500 ppm. 1-hr avg (6 hours) due to faully servo on Flue Gas Slide Valve. (08/04/10 — Area 6 Fuel Drums H.S>160 ppm, 3-hour avg (6 hours) due to shut down of DAO Hydroteater for catalyst change out [ Spits 08/02/10 — Sheen to Mississippi River at Dock due to hole in containment pan for crude transfer hose. Less than 1 gt of oil spilled. (first RQ spill in 218 days) Water Permit Execedances None (last exceedance — July, 2007) Citizen Calls 6 calls for odor = 3 were possibly WWT odor, 7 possibly from vac truck on line Geanout, 1 possibly from Hydrocracker Cooling Tower chemistry, 1 unable to verify 6 calls for noise — 1 for PSV lifting on boiler 8-5 during rain (6/28), 3 for PSV Iiling on boiler B-S during ‘maintenance (7/6), 2 for steam at North Flare during unit shut downs (8/5), 1 call for elevated flame in North Flare ~ shut down of units (8/4) ‘Other MOUSA continues to meet with EPA and DOJ regarding the Consent Decree ‘Odor abatement - WWT running well ‘Water permit renewal — no action from LDEQ.
