1. To run the SetupSurvey macro, choose
Tools | Macro | Macros
Select the SetupSurvey macro, and click Run.
You will be prompted for the number of questions in the survey.
This creates a survey form with Responses valued 0-4,
and an N/A response.

2. After creating the survey:

-- Weighting in column B can be changed from 1 to any value.
-- You can format the worksheet, and change the heading text
-- To clear a set of option buttons, delete the score in
column C for that row.
-- You can move the Survey sheet to another workbook,
