ADDITIONAL INFORMATON There is no conflict between the families as stated by the mother. The head of the family decides and the rest of the family member follows. The family has a good interaction with each other.

There are no observable conflicts in the family. The family has a good verbal communication. The head of the family is basically the first one to be confronted if there are arising problems.

The house is concrete. During assessment, their living space is adequate. There are 3 bedrooms, a comfort room, sala and a kitchen.

Their garbage is collected every other day by the city trucks and is segregated by putting it on garbage bags. But sometimes they burn dried leafs and or papers.

The drainage system is open and may have a risk on their health because it can be a breeding site for mosquitoes and other insects.

Source of water supply is water pump. They use it for many purposes such as, as drinking water, washing of kitchen utensils and bathroom and laundry needs. Their drinking water storages are covered and put into plastic pitchers and bottles.

Food storage/cooking facilities are covered and placed into pots/pans.

Common household pests found at home is rats.

There are a lot of pets and animals found in their home like: cats, dog, pigs

There is present accident hazard in their house, like the stagnant water in front, which is very slippery and may cause injury for those who will step on that area. Another is the stairs which is prone from injuries also for toddlers.

V. HEALTH ANMD HEALTH PRACTICES

A. Common illness encountered for the last 6 months and the treatment applied.
None.

B. Whom do you consult for health related problems? Basically, the family consults first in the health centers. But, for diseases, they consult on doctors. For mild injuries, they also go to the nearest manghihilot.

C. For problems other than health, whom do you consult?


If there is a problem in the family, they consult the other family members.

They sleep 5-6 hours a day, the Mother wake up early in the morning to start their daily lifestyle. They take a regular walk as their exercise and relax in their house through watching television and eating their meals and merienda together. They manage stress through sleeping. VI. ENVIRONMENT The mother states that they have good and kind neighbors which they can rely on to whenever there is a problem in their family. Their health center is near to their house, so whenever there is an emergency situation happened within their community, they can easily access to health care deliveries. There is an abundant transportation facility in the barangay. They use tricycle or jeepneys as a transportation.

A. Are you aware of existing organizations in the community?


The mother is not a member of any organizations and is not aware either.

B. Name 5 formal and non-formal leaders of the community whom you think can lead the people. Fernando Almendares ( Brgy. Captain) and all of the barangay officials and health care workers have the ability to lead our community and is responsible with their jobs.
