org & faxed to (414)465-3601 March 20, 2011 Anne Ballentine Vice President - Communication & Public Relations Wheaton Franciscan Healthcare 400 W. River Woods Parkway Glendale, WI 53212 Ms. Ballentine: Next week I'm submitting letters of concern to oversight agencies regarding alleged first aid training programs conducted in Wisconsin by the Save-A-Life Foundation (SALF). Prior to doing so, I wanted to provide you with the opportunity to address what appears to be a serious discrepancy regarding Wheaton Franciscan's SALF program. In recent e-mails to writer Lee Cary of Little Elm, TX, you stated that your program provided first aid training to thousands of students in the Milwaukee Public Schools. However, in a March 17, 2011 FOIA response, the schools failed to locate any records for the programs described in your correspondence. Therefore, this is to request that you provide me with the following information so that I may include it in my forthcoming letters of concern: 1) The names of all Milwaukee Public Schools employees with whom your organization had any contact regarding your organization's SALF program. 2) The names and contact information of all Wheaton Franciscan employees who administered and/or participated in your organization's SALF program in the Milwaukee Public Schools. 3) The names and contact information of all trainers who conducted classes in the Milwaukee Public Schools in your organization's SALF program. Unless you inform me that you require more time, I look forward to receiving the requested information by March 25, 2011. In the event that I do not receive the requested information by that date, my understanding will be that Wheaton Franciscan is unable or unwilling to provide it. Sincerely,

April 27, 2011 VIA CERTIFIED MAIL Mr. Peter M. Heimlich 3630 River Hollow Run Duluth, GA 30096 RE: 'Wheaton Franciscan Healthcare Save A Life Program

Dear Mr. Heimlich: I am writing to you in response to YOLlr arch 20, 2011 correspondence to Ms. Anne M Ballentine of Wheaton Franciscan Healthcare ("Wheaton"). Please be advised that I serve as in-house legal counsel to Wheaton. Accordingly, I ask that any future correspondence be directed to my attention. I have reviewed your letter where YOLl allege "a serious discrepancy regarding Wheaton Franciscan's SALF program". We appreciate you bringing your concems to our attention. Wheaton's mission is committed to providing our patients and communities with exceptional and compassionate healthcare services. I have investigated this matter and your concerns. Please accept my assurance that there is no "discrepancy" regarding Wheaton Franciscan's Save a Life Program. Wheaton's Save A Life Program is not affiliated with the former Safe A Life Foundation run by Carol Spizzirri. Wheaton's Save a Life Program brings on-site first aid training to the community at large. The program also provides age appropriate instruction to area schools, clubs, and other children's groups and provided instruction to over 600 students in 2010. The one hour simplified children's training program introduces kids to recognize an emergency, contact emergency medical personnel, control bleeding and other basic skills. Bystander Basics is a two hour program that meets the needs of children ages 13-18 years old. It delivers hands on basic skills and teaches the youth how to recognize an emergency and how to care for someone who is ill or hurt until emergency medical help arrives. The above programs provide valuable skills to school children who often times experience few successes in their lives. Each class that completes the training gets a commemorative plaque that is hung in their classroom and serves as a reminder of their newly learned skills and is often a source of pride. The sixteen instructors include registered nurses fr0111 Wheaton and EMT/firefighters and paramedics. All instructors

I'd appreciate receiving your answers to the following questions. 1) You wrote: Wheaton's Save A Life Program is not affiliated with the former Safe A Life Foundation run by Carol Spizzirri. Has Wheaton Franciscan or any of its entities ever been associated with the Save-A-Life Foundation of Schiller Park, IL (SALF) as SALF claimed on numerous documents, including financial reports submitted to the US Centers for Disease Control and Prevention (CDC) and in fund raising materials? If so, what was the nature of the relationship and on what dates did it begin and end? 2) Did Wheaton Franciscan or any of its entities ever receive funding from SALF? If so, would you please provide the dollar amounts, the dates when the funding was received, and the purpose for which SALF provided the money? 3) Did any employees of Wheaton Franciscan or its entities ever correspond with representatives of SALF? If so, would you please provide me with the names of the SALF employees? 4) Did your program ever provide first aid training classes to students in the Milwaukee Public Schools (MPS)? June 8, 2011 John D. Oliverio President and Chief Executive Officer Wheaton Franciscan Healthcare (WF) 400 W. River Woods Parkway Glendale, WI 53212 Mr. Oliverio: In recent months I've unsuccessfully attempted to clarify a number of discrepancies regarding St. Joseph Medical Center's Save-A-Life Foundation first aid training program and that program's partnership with the Save-A-Life Foundation Inc., a Chicago-area nonprofit reportedly under investigation by the Illinois Attorney General and apparently by at least one federal agency. To summarize, on March 20, 2011 I e-mailed several questions to Anne Ballentine, WF's Vice President of Communications & Public Relations. In response, I received an April 27 reply from WF legal counsel Matthew W. Moran. His letter failed to answer any of the questions I asked Ms. Ballentine, but did invite me to submit further inquiries. Assuming Mr. Moran's offer was sincere, I sent him a May 5 inquiry that included my unanswered questions and other questions based on assertions made in Mr. Moran's letter. I didn't receive a reply so I sent him a courtesy follow-up on May 24. (Please click here to review the correspondence.) I haven't received a reply from Mr. Moran therefore I'm bringing the matter to your attention. Further, since my previous correspondence, I've become aware of other discrepancies that I hope to resolve. Earlier this year, in a series of e-mails to writer Lee Cary, Ms. Ballentine claimed that since 2001 many thousands of Wisconsin students in the Milwaukee Public Schools (MPS) and the School District of Brown Deer participated in first aid training classes conducted by St. Joseph's Save-ALife program. For example, in a January 7, 2011 e-mail to Mr. Cary, Ms. Ballentine wrote: Annually, the largest number of presentations done by SAL Milwaukee are to the Milwaukee Public Schools. Approximately 6,000 students were trained in 2007. In a January 10 follow-up e-mail, Mr. Cary asked Ms. Ballentine to provide him with the names of the Milwaukee Schools where the approximately 6000 students were trained. In a January 18 reply e-mail, Ms. Ballentine included a chart listing approximately a dozen MPS schools where the training supposedly took place. However, in recent responses to FOIA requests I submitted to MPS (one of which was based on the chart Ms. Ballentine provided to Mr. Cary) and to the School District of Brown Deer, neither institution was able to locate any records associated with any such program. (Please click here to review those documents.) It remains unclear what accounts for these and other discrepancies regarding Wheaton Franciscan's Save-A-Life program, therefore this is to request that you review your organization's records and to provide me with detailed answers to the questions I submitted to Ms. Ballentine and to Mr. Moran. For your convenience, please click here to download a pdf file that combines those inquires and the FOIA records described in the previous paragraph. Thank you for your consideration and I'd appreciate a courtesy e-mail no later than this Monday June 13 providing me with some indication of how you intend to proceed.
