1. Purpose. This directive prescribes the policy and procedures for New York Guard members
to obtain New York “State Guard” license plates.

2. Application. This regulation applies to all New York Guard members.

3. Scope. This publication describes procedures to be followed in application for New York
Guard Special Plates.

4. Basis. The basis for this directive is the New York Vehicle and Traffic Law §404-d.

CHAPTER I

POLICY

It is the policy of the New York Guard that members of the New York Guard who actively
participate in the New York Guard are authorized to apply for New York “State Guard” special
license plates for their motor vehicles.

Former members of the New York Guard who have retired with an honorable discharge may
continue to maintain their State Guard plates. Members who do not regularly perform duty or
leave without approval may have their privileges revoked.

CHAPTER II

RESPONSIBILITIES

1. New York Guard, Assistant Adjutant General (NYSG-AG/A) will;

a. Review applications and provide certification (NYG Form 4515) to qualified members
IAW this Directive.

b. Receive and periodically review list of issued plates from NYS Department of Motor
Vehicles.

c. Issue revocation of authorization for plates held by deceased or non-performing members.

2. Each member will, adhere to this directive and keep the New York Guard informed as to
status.

CHAPTER III

PROCEDURES

1. New York Guard members submit a written request for NYG special plates to the
Headquarters, New York Guard, 330 Old Niskayuna Road, Latham, New York 12110-2224,
ATTN: NYSG-AG/A.

2. New York Guard (NYSG-AG/A) will verify the applicant’s membership status and drill
participation.

3. New York Guard (NYSG-AG/A) will provide the applicant with certificate of authorization,
NYG Form 4515 (Appendix A).

4. The applicant/member shall take the certificate of authorization to the New York State
Department of Motor Vehicles (DMV) and purchase plates, utilizing New York State
Department of Motor Vehicles Application for Military and Veterans Custom Plates, form MV
412. The member shall inform the NYG (NYSG-AG/A) of receipt of plates.

5. The member may renew as required by DMV, as long as they remain in good standing.
Members who depart under other than honorable conditions may have their renewal privileges
revoked.

FOR NEW YORK GUARD LICENSE PLATE

[ ] INITIAL ISSUE: This will certify that ____________________________________

is a current member in good standing of the New York Guard, and is therefore entitled to
the DMV initial assignment of a New York Guard numbered plate. Request that New
York Guard plate number _______ be assigned to him/her.

[ ] RENEWAL OF SAME PLATE: This will certify that ______________________

continues to be a current member in good standing of the New York Guard, and is
therefore entitled to continuance by the DMV of his/her current New York plate number
_______ upon attainment of the current registration expiration date.

[ ] WITHDRAWL OF PLATE PRIVILEGE: This will certify that the New York
Guard plate number ________ previously issued to _______________________________
be withdrawn inasmuch as he/she is no longer a bone fide member in the New York Guard
owing to the following change in his/her status:
[ ] Death of the service member confirmed on ________________;
[ ] Discharge from the New York Guard effective on ________________.

[ ] REASSIGNMENT OF PLATE NUMBER: This will certify that the New York
Guard plate number ________ previously issued to _______________________________
