1.0 SCOPE OF WORK Describe briefly the nature of the project Include a brief description of each unit (or major facility) indicating purpose, capacity, areas and other significant features Highlight any new or out of the ordinary type of works carried out. Geographic location Soil conditions and water table ATTACHMENT # 1 Includes Key Engineering Drawings such as: Plot plan Building equipment arrangements Selected building elevation drawings (and/ or cross section) Process flow diagram and/or heat balance Electrical single-line diagrams 2.0 PROJECT PHOTOS Provide a least one photograph for each month that best shows the overall progress of the works (not less than 25 progress photos). Photos should show project status during early, mid-point and final stages of construction. 3.0 CONTRACT TERMS The basic type of contract: i.e. cost reimbursable plus percent fee, cost reimbursable plus fixed fee, lump sum amount, fixed unit price, etc. Description of any co-contractor or joint venture agreements involved in the project Description of third party contracts between Employer and other firms for other services (project management, quantity surveying, ). 4.0 END USER RELATIONS Provide a brief synopsis of the relationship with the end user on the project. Identify any lessons learned or noteworthy developments, positive or negative, during the course of the project to be considered in future dealings. 5.0 PROJECT PROGRAM / PROGRESS Provide an overview on progress throughout the course of the project. Provide a brief bulleted narrative on any Key points of the Construction that attributed to the success of the project or caused major problems during the course of the Project. What were the technical issues raised during the progress of the project? What was the performance of the Mechanical, Electrical and Plumping MEP Consultant, how would you rate the Consultant? Where there any particularly changeling technical aspects of the project that had to be tackled and how were they overcome?. What level of re-engineering did we carry out? Did we have to produce any new method statements for any particular aspect of this project?. If so were can a copy of this be obtained, it may be useful on another project. 7.0 ESTIMATING / BOQ Highlight any major quantity discrepancies if applicable. Identify any significant positive or negative costs in comparison to that included the estimate/BOQ. A copy of the payment log should be attached to the report in Attachment # 3 It is very important to address a rough estimate of the average cost / unit. ( i.e. XXX $/m2) 8.0 CHANGE ORDERS This summary tabulates the development of the final budget. It is maintained over the course of the project and one is referred for guidance . Attachment # 4 CLAIMS This summary tabulates the all claims that were raised during the course of the project. The form Claims log should be included in the Attachment # 5. Another form of outmost importance should be carefully completed: to address the following: o Brief Description. o Claim Information: Amount and / or value o Cause (Brief). o Contract basis of claim (Clause #) o Employer Settlement : Justification. (elaborate) Date Amount (Dhs) EOT Days o Lessons learnt. o How to avoid in future o If there are any suggestions to modify the conditions of contract or Specs The form is attached in Claims History Log. Attachment # 6. SUBCONTRACTORS & SUPPLIERS Summarize all project subcontracts.

Highlight any Particular Sub-contractor that performed well and should be considered for future partnering or preferred sub-contractor arrangements. Identify poor performance were we should seriously consider not using the Sub-Contractor on future projects. Identify poor performance were we should seriously consider not using the supplier on future projects. 11.0 STUDIES Most of the information contained in this Report can be completed only at the end of construction. In many cases, however, special studies can be prepared and distributed soon after the particular events studied are completed. Examples of such special studies might include the following: Unusual site preparation operations Demolition Dredging Piling Well-point system Unusual or innovative construction operation/Lessons Learned Complex or first-of-a-kind concrete structures Heavy rigging operations (particularly when performed by subcontractors) Erection of major mechanical equipment items Project Problems/Lessons Learned Rigging or equipment handling problems Storm damage Unusual weather-related costs (heating, temporary enclosures, etc.) Major design changes involving removal or re-construction work Studies of the above typical operations should be prepared and distributed soon after the event or operation is completed. 12.0 CONCLUSION Provide a brief overview on the project. Highlight again any positive aspects of the project, mention names of those that performed well both inside and outside the Employer for this project. Do you recommend the contractor for future work?
