Community Care Plan 1

Situation: During the first two days of our duty in Purok Alfonso, Brgy. Bong Bong, Valencia, our group conducted a survey. The results of our survey showed that there were a number of people in the community who were affected with hypertension, Diabetes, fever, cough and colds and asthma as a perceived actual health problem. Community diagnosis/statement of the problem: Inability to make decisions with respect to taking appropriate health action due to lack of knowledge regarding the nature and magnitude/scope of the problem. Goal: A the end of our 4-week care, the families in Purok Alfonso will be able to have the knowledge of the illnesses and take appropriate measures and actions to prevent further transmission and worsening of illnesses. Specific objectives
Within our 4 weeks of duty, the family will be able to: a. verbalize understandin g of the nature, magnitude/ scope of the problem b. aware of the possible complications of hypertension, diabetes mellitus, fever, cough and colds, and asthma c. enumerate the possible

1. Health teaching to family concerning the: a. Nature, magnitude/ scope of the problem. b. Possible complication s of hypertension, diabetes mellitus, fever, cough and colds, and asthma specifically its effects to the respiratory, circulatory and urinary system.

Situation: During our duty in Purok Alfonso, Brgy. Bong Bong, Valencia, we have observed that the purok has several sites for breeding of mosquitoes. We have observed that there was a stagnant canal near the households and the purok doesnt have a proper drainage and piping system for the used water. Community diagnosis/statement of the problem: Inability to provide a home environment conducive to health maintenance due to inability to recognize presence of problem due to inadequate knowledge of proper sanitation and personal hygiene and inability to make decisions with respect to taking appropriate health action due to low salience of the problem/ condition. Goal: A the end of our 4-week care, the families in Purok Alfonso will be aware and understand fully the importance of a healthy and sanitary environment and maintenance of good personal hygiene conducive to health maintenance. Situation: During our interview, survey and visitations in Purok Alfonso, we have observed that there are a number of people who smoke and drink alcohol and we have considered this as one of the health threats in the community. Health Problem: Presence of health threat due to unhealthful lifestyle and personal habits: cigarette/tobacco smoking and excessive alcohol consumption Goal: A the end of our 4-week duty in the community, the residents of Purok Alfonso will take the necessary measures to address their unhealthful lifestyle and minimize smoking and alcohol consumption Specific objectives
