Situation: During the first two days of our duty in Purok Alfonso, Brgy. Bong Bong, Valencia, our group conducted a survey. The results of our survey showed that there were a number of people in the community who were affected with hypertension, Diabetes, fever, cough and colds and asthma as a perceived actual health problem. Community diagnosis/statement of the problem: Inability to make decisions with respect to taking appropriate health action due to lack of knowledge regarding the nature and magnitude/scope of the problem. Goal: A the end of our 4-week care, the families in Purok Alfonso will be able to have the knowledge of the illnesses and take appropriate measures and actions to prevent further transmission and worsening of illnesses. Specific objectives

Situation: During our duty in Purok Alfonso, Brgy. Bong Bong, Valencia, we have observed that the purok has several sites for breeding of mosquitoes. We have observed that there was a stagnant canal near the households and the purok doesnt have a proper drainage and piping system for the used water. Community diagnosis/statement of the problem: Inability to provide a home environment conducive to health maintenance due to inability to recognize presence of problem due to inadequate knowledge of proper sanitation and personal hygiene and inability to make decisions with respect to taking appropriate health action due to low salience of the problem/ condition.
