Introduction 1. There are three sources of information on offenders who are or have been under the supervision of the Washington State Department of Corrections (DOC) that are available to law enforcement and criminal justice agencies via ACCESS: The WACIC-DOC File, available through WACIC, the DOC locator file and the DOC Felony Offender Reporting System (FORS) available through the DOC system. Each file will be explained in the following sections. 2. The purpose for making these files available via ACCESS is to provide law enforcement agencies with investigative/lead information and enhance officer and citizen safety by providing the current status and other relevant information of all offenders under active supervision by DOC. 3. Users are cautioned that an inquiry into these files is not a substitute for a WACIC/NCIC wanted person inquiry. Escapees from institutions, work release facilities, or community custody supervision will be flagged in DOC records; however, there may well be delays in updating the files. An inquiry should always be made to WACIC/NCIC as well as to DOC. 4. Note: Under no circumstances should an arrest or confinement be based solely on information contained in a DOC response. B. WACIC-DOC File 1. This database is a copy of data contained in the DOC database and is stored in the WACIC system. It is queried with each WACIC wanted person inquiry submitted to WACIC. 2. Inquiry a. The WACIC-DOC file is accessible by making any WACIC wanted person inquiry (MKE QW, DW, W, or QWH). See the WACIC Manual, Wanted Person File, for further information on inquiries. b. The entries in the file are indexed by phonetic name, as are other WACIC files. Selection of records is by matching phonetic names, filtered by the date of birth plus and minus one year. 3. Response Formats a. Up to 20 records can be retrieved on one inquiry. As noted previously, these responses contain only minimal identity information, users should check the DOC locator or DOC FORS files for further information if hits are received through the WACIC-DOC file. 4. 3. Inquiry a. An inquiry to the DOC locator file may be made by SID number or by NAM and DOB. An inquiry by SID number can be made with or without message field codes. The SID number must contain ten characters, the first two must be WA and the last eight must be numeric. An inquiry by NAM and DOB requires the DOB field to be identified. Examples of inquiries are shown below: BY SID P;P..SID/WA99999999(Send) P;P..WA99999999(Send) BY NAM/DOB P;P..NAM/SMITH, JOHN P.DOB/19430229(Send) P;P..SMITH, JOHN P.DOB/19430229(Send) b. ORI optional, if not included ACCESS will insert the default ORI for that terminal. c. If an inquiry is submitted by NAM and DOB and more than one record is found, the response will indicate MULTIPLE RECORDS FOUND and will return all records on file. After the last record in a multiple record response, the phrase END OF MULTIPLE RECORD DISPLAY will be appended. If an inquiry is submitted by NAM and DOB and no record is found, a partial search is conducted using last name, first initial, and DOB. If a match is found, the response will indicate FOLLOWING MATCH ON LAST NAME, FI, DOB and return the record on file. 4. Sample Responses a. The second line of the response may be an optional DOC locator program message that will, generally, be self-explanatory. In some instances, the program message will inform the user of an error in the inquiry. In this case, it will be necessary for the operator to reformat the message and resubmit. DOB - Date of birth subject was committed under. 4. SID - State ID Number. Will be WA followed by eight numerics. 5. DOC - Department of Corrections number. Is provided to further identify the correct DOC client. 6. SEX - Will be either M or F. 7. RAC - Race code will be one of the following: A, B, I, U, or W (see NCIC Code Manual). 8. HGT - Height expressed in three digits (i.e. 509 = 5 feet 9 inches). 9. WGT - Weight expressed in three digits (i.e. 095 = 95 pounds). 10. EYE - Eye color (see NCIC Code Manual for colors). 11. HAI - Hair color (see NCIC Code Manual for colors). 12. M-SERIOUS CRIME UNDER ACTIVE SUPERVISION AS Variable length field will be one of the following: a. INSTITUTION INMATE. b. WORK-TRAINING RELEASE INMATE. c. PAROLE CLIENT.

CRIME - Revised Code of Washington (RCW) description of the crime the subject was placed under active supervision for in the facility or programlisted in DOE TO. 14. DOS - Date of sentence in month-day-year format for crime specified in CRIME. 15. PL-CONV - Description of place convicted of the crime described under CRIME. May have other convictions. May be county, state, or country. Will normally be a county within Washington (e.g., KING). 16. DOE TO - Represents the date of entry to a facility or program (for the offense listed under CRIME) expressed as month-day-year, and the facility or program committed to (i.e. 06-01-95 CLEARWATER CORRECTIONS CENTER). a. Mailing and physical address of facility identified in DOE TO. May consist of street address and mailing address. Example: Box 2500 Star Route 1, P.O. Box 1539 b. Will be a maximum of 20 characters, comma, space, and 20 additional characters. 17. ADDR (second line) - City address of facility committed to, one space, state, zip, two spaces, SCAN phone number, two spaces, public phone number of the facility identified in DOE TO. 18. E. Error Messages 1. The DOC Locator file will respond to inquiries containing format or other errors by returning the inquiry followed by a Locator program message : DOC TIME: 1044 DATE: 070195 TO: OLYPD P.WA0340100.SMITH,JOHN,P.022943 INVALID NAM/ DOB/ FORMAT DOB FORMAT MUST BE MMDDYY a. In the above example, the message field code was omitted prior to the date of birth and the date of birth was interpreted as invalid. The following error messages may appear, either singly or in combination: (1) SID NOT FOR STATE OF WA - First two characters of SID not WA. (2) INVALID SID FORMAT - Characters three through ten not numeric, or missing EOT. (3) INVALID NAM/ DOB/ FORMAT - Field delimiter (.) not found between name and mandatory DOB message field code, or MFC DOB not found. (4) INVALID INQUIRY FORMAT - Invalid MKE or ORI delimiter not found. (5) DOB FORMAT MUST BE MM-DD-YY - Invalid DOB. (6) ORI MISSING OR INVALID. (7) ORI NOT FOR STATE OF WA. (8) ORI CONTAINS BLANKS. (9) THE DATABASE IS NOT AVAILABLE - While this message does not indicate an error on the part of the terminal operator, it will be necessary to resubmit the inquiry later when the database is available. b. Other informational messages will appear on the Info program message line on occasion. These messages indicate the type of search conducted or type of response and do not require resubmission of the inquiry. Overview The Felony Offender Reporting System (FORS) provides felony offender information maintained by Washington State Department of Corrections (WA DOC) to a variety of users outside of WA DOC. In the past, WA DOC received over 600 inquiries for offender information each week. Inquiries came from a variety of sources including Federal Criminal Justice Agencies (FBI, Bureau of Alcohol, Tobacco, & Firearms, etc.), out of state Criminal Justice, local hospitals, local Criminal Justice Agencies and the general public. These inquiries led to the development of the Felony Offender Reporting System. There are eleven screens that comprise FORS. The first screen provides for entry of information that is used to search the database for the offender information. The user may enter a Social Security Number, State Identification Number, Department of Corrections offender identification number, Department of Corrections conviction county and cause number or, if the user does not know one of these numbers, may enter both a full name (first and last names) and a date of birth. The user also enters a Function Code. The Function Code will specify what information the user wishes to view. The second screen provides text indicating whether the search found the person they were inquiring on or not and provides Revised Code of Washington (RCW) and disclaimer information. Screens three, four and five provide publicly disclosable information on offenders. Screen six information is only available for disclosure to Law Enforcement Personnel. Appropriate security is included to ensure proper restrictions to this screen. Information on screens seven and eight is only available for disclosure to County Clerk personnel. Appropriate security is included to ensure proper restrictions to this screen. Information for screens nine, ten, and eleven is also only available for disclosure to Law Enforcement Personnel. Appropriate security is included to ensure proper restrictions to these screens.

Using FORS FORS was designed for a variety of users. Because of this, each agency may enter FORS differently. For example, if you have the technical ability to sign on to State of Washington Department of Corrections OBTS region (contact your technical support staff to determine this) you may automatically be presented with the FORS Menu screen once you have successfully signed on. If you can sign on to the OBTS region and you have authorization for both the OBTS database AND FORS, then you must type the transaction FORS to enter the FORS Menu screen. In simpler terms, if you sign on to the region where FORS is and you see a blank screen, type in FORS in the top left-hand corner. If you sign on to the region where FORS is and you are immediately presented with the FORS Menu you don't need to type FORS and, in fact, FORS will not allow you to. If you are a current OBTS user, you may type FORS from any OBTS screen to gain entry to the FORS Menu. Please keep in mind that the FORS application is separate from OBTS, and at this time, it will not carry over the DOC number from OBTS. You must enter the search information on the Menu. Also, FORS has different functionality from OBTS - so read the function key descriptions at the bottom of the screen! For example F3 will log you off the region, not just the FORS application. Whereas, pressing the CLEAR key may take you to a clear screen. FORS provides user information at the bottom of every screen. The function keys available are listed on each screen, and error messages will display if you press an invalid key, or if the information you enter on the Menu screen is in error. ADMINISTRATOR of the COURTS

(AOC) USERS
The Felony Offender Reporting System (FORS) is now available to AOC users. The following is a short description of the steps needed to successfully access FORS. NOTE - This reference page is specific to the AOC to DOC connection. 1. 2. 3. 4. Sign on to the AOC system as usual. Select the Felony Offender Reporting System (FORS) option from the AOC TPX menu. You have successfully accessed the FORS system. You may now refer to the remainder of the FORS User Guide for more information. Because of the special technical connection established between AOC and DOC, some function keys perform differently than indicated on page 3 of this manual. From the FORS system, the CLEAR key and the F3 key will take you back to the AOC TPX Menu.

ACCESS USERS Users going through the ACCESS Switch will be entering FORS just as they would any of the other information available through ACCESS. Please refer to your ACCESS manual from Washington State Patrol, Section 7B - DOC/Info. Inquiries can be made by State Identification (SID) number, Social Security (SOC) number, Federal Bureau of Investigation (FBI) number, Department of Corrections (DOC) number or Name and Date of Birth.

Format Examples: By SID By SOC By DOC By FBI By NAME/DOB * ORI code is optional For the quickest response time use SID, FBI, or DOC numbers whenever possible. When using name and date of birth, a search will be done for an exact match on both name and date of birth. If an exact match is not found, a five-year spread on birth date (both five years previous and five years after) will be used to try to find a match. If multiples are found when searching by name and date of birth, a response will list name, Department of Corrections number, date of birth, social security number and SID number of all matches (including aliases) within a five-year date of birth scope. In addition, a message will appear suggesting the use of one of the numbers in the list in order to obtain an exact match on the desired offender. Once an exact match is made, the requested information can be retrieved and presented. If no 'hit' is made, a response indicating this will be sent. When a hit is made against a singular DOC offender, the information requested by the message key (EG: CA, CB, CC, CD, CE, CF, CG, or CH) will be sent to the user. FORS will only return ONE record per inquiry. SECURITY If you are currently an ACCESS Switch user, you already have security to FORS. If you have AOC TPX Menu access and the FORS selection displays, you have security to the FORS system (be sure to check the second page of your menu). If you do not have FORS security through AOC TPX contact AOC Help Center at 1-800-442-2169. If you are a CICS user, you will need to request a Security Logon ID from State Of Washington's Department of Corrections Customer Assistance Center. Fill out the form on the next page and mail to the address provided. You may make copies of this form if multiple requests are necessary. The first time CICS users sign-on with their new Logon Id and the password provide to you. During the first log-on process, you will then be prompted to change your password. Passwords are required to be change every 60 days and be unique for 5 changes. The system will automatically prompt you for a new password when 60 days have elapsed. (This procedure is not applicable to the ACCESS Switch or AOC TPX users.) Please remember that passwords are confidential and should not be shared. If you type in a wrong password three times in a row, your Logon ID will be revoked. Once you sign-on successfully, the revocation is reset to zero. If your Logon ID should be revoked, the DOC Customer Assistance Center will have to reset it for you. 1) State Identification Number (SID): Eight-digit number assigned by Washington State Patrol positively identifying an individual based on fingerprints. Do not enter any dashes, slashes or special characters. Social Security Number (SSN): Nine-digit number assigned by the Federal Government upon application by the individual. Do not enter any dashes, slashes or special characters. 3) Department of Corrections Number (DOC): Six-digit number assigned by WA DOC to identify an individual offender. Do not enter any dashes, slashes or special characters. 4) 5) 6) 7) County Code and Cause Number Two-digit county code (valid values are 01 through 39) and cause number Last Name: Up to 20 characters of last name may be entered. Function Code 'C' will display Screen C which provides conviction information about the offender. Function Code 'D' is only authorized to Law Enforcement Personnel. This function will display Screen D which provides confidential offender information such as the offender's residence address, the offender's behavior pattern, the offender's FBI number, etc. Function Code 'E' is only authorized for County Clerk Personnel. Screen R displays an offenders LFO payment schedule/billing interrupt history for specific county causes. Function Code 'F' is only authorized for Law Enforcement personnel. This function will display Screen F which provides offender information relating to HB1172. That is, the DOC sex/kidnap offender registration information: release date, registration date, release address, release county, release employer name and release employer city. If DOC sex/kidnap offender registration information is not applicable to the current offender, the words Not Applicable will be displayed in the registration information areas of the screen. Function Code 'G' is only authorized for Law Enforcement personnel. This will display Screen G which provides conviction information about the offender. Function code G will also provide access to screen S, by way of screen G. Screen S displays an offenders court-ordered and Department of Corrections-imposed conditions, treatments, crime-related prohibitions and affirmative conduct for specific county causes. Function Code 'Z' will allow the user to scroll through all of the screens to which they have security. Law Enforcement personnel will scroll through screens A, B, D, F and G (if DOC sex/kidnap offender registration information is not applicable to the current offender, screen F will not be displayed). SID#: Eight-digit State Identification Number assigned by Washington State Patrol positively identifying an individual based on fingerprints. 4) Alias: All known aliases will be listed. If there are more than three aliases, the user may scroll forward using F10 to view additional aliases. 5) DOB (Date of Birth): Date when born in format MM/DD/YYYY. CR - Indicates that the offender has overpaid cost of supervision fees. 11) Legal Financial Obligation (LFO) Information by County: LFO information is displayed alphabetically by County and within the County from oldest to newest with one line per cause number. A blank line separates a change in County. Depending on the number of different counties of conviction, a maximum of four causes can be shown on each screen. If there are more causes than one screen can display, the user may scroll forward using F8 to view additional causes. 12) County: The name of the sentencing County. If it is an interstate case, it will display the sentencing state 2-digit code. An asterisk ('*') will display to the right of the County name if the case is closed. A case is considered closed when the DOC no longer has jurisdiction over that particular case. All prison cases close at the time an offender leaves prison and this would be noted on FORS. However, should the offender have some form of DOC post release supervision following prison, the cause will be re-opened. Please note that it is possible for a case to be closed with DOC but still open with the Court. 13) Cause: The County cause number as described on the judgment and sentence and any subsequent court orders (sometimes referred to as the SCOMIS id). 14) 15) Date of Sent: The date of conviction for the cause, in the format MM/DD/YY. Information displayed will be: Deceased indicates the offender is Deceased. Jail indicates offender is in Jail. No WA DOC Juris indicates the offender is No longer under the jurisdiction of WA DOC. Prison indicates the offender is in a State of Washington Pre-release facility or in either a State of Washington or Out-of-State Prison. Supervised indicates the offender is under Supervision within the jurisdiction of WA DOC. This includes time on Parole or Probation, Community Custody, etc. Unavailable indicates offender is Unavailable for in-person reporting to WA DOC. Work Release indicates offender is in a State of Washington Work-Release facility. 8) CCO: Community Corrections Officer - If the offender is under community supervision, the officer's name who is supervising the offender. 9) EMAIL: Community Corrections Officers E-mail Address - If the offender is under community supervision and the officers e-mail address is on file with the WA DOC. 10) Newest to Oldest Sentence Information: Conviction information is displayed from newest to oldest with one line per cause number. A maximum of eight convictions will be shown on each screen. If there are more than eight convictions, the user may scroll forward using F8 to view all felony convictions. 11) County: The name of the sentencing County. Screen R - Legal Financial Obligation Scheduled Payment History 1) 2) Name: First and Last name used by WA DOC for the offender. County: The name of the sentencing County. If it is an interstate case it will display the state 2-digit code. An asterisk ('*') will display to the right of the County name if the case is closed. A case is considered closed when the DOC no longer has jurisdiction over that particular case. All prison cases close at the time an offender leaves prison and this would be noted on FORS. However, should the offender have some form of DOC post release supervision following prison, the cause will be re-opened. Please note that it is possible for a case to be closed with DOC but still open with the Court. 3) 4) DOC#: Six-digit number assigned by WA DOC to identify an individual offender. Only conditions in effect will display on this screen. 1) 2) Name: First and Last name used by WA DOC for the offender. County: The name of the sentencing County. If it is an interstate case, it will display the sentencing state 2-digit code. 3) 4) DOC#: Six-digit number assigned by WA DOC to identify an individual offender. SID#: Eight-digit State Identification Number assigned by Washington State Patrol positively identifying an individual based on fingerprints. 5) St Mx Dt: The Statutory Maximum Date is the longest time this offender may be under DOC control on a specific date as set by statute. The valid entries are: blank, a date, or LIFE. 6) Cause: The County cause number as described on the judgment and sentence and any subsequent court orders (sometimes referred to as the SCOMIS id). 7) Imposed By: Indicates the jurisdiction imposing the condition(s) (affirmative conduct, crime-related prohibition, other or treatment). Information displayed will be: Board Indicates the Indeterminate Sentence Review Board has placed specific conditions on the offender. Court Indicates a Court order directing the offender to follow prescribed conditions. DOC-Emergency (DOC/E) Indicates the DOC has placed specific conditions on the offender that remain in effect for no more than seven (7) days. Should an Emergency condition be approved by the Indeterminate Sentence Review Board within the 7-day timeframe, an active DOC-Board Approved condition replaces the now inactive Emergency condition. DOC-Imposed (DOC/I) Indicates the DOC has placed specific conditions on the offender. 8) Type: DOC type code for the condition. (Condition types beginning with 'A' or 'O' are Affirmative Conduct conditions; types beginning with 'C' are Crimerelated Prohibition conditions; types beginning with 'T' are Treatment conditions). 9) 10) 11) Description: Description for the DOC type code of the condition. Effective Date: Date the condition began.
