It is a matter of great pleasure that the Department of Academics, Sir Ganga Ram Hospital under the aegis of The Ganga Ram Institute for Postgraduate Medical Education & Research (GRIPMER) is bringing out the Academic Programme booklet for January 2013 to June 2013. It gives me immense satisfaction to see the phenomenal growth of teaching activities in this hospital. This booklet provides comprehensive information about the day-to-day academic activities of all the departments in the hospital. It has been found to be extremely useful and widely acclaimed by all concerned with the academic activities at this institution of repute. I hope that all postgraduates will be real beneficiaries and will be able to use their learning for better patient care. PREFACE

It is a matter of great honour for me to write a preface for the Academic Programme booklet of January 2013 to May 2013. This booklet contains various teaching activities of the departments and reflects the commitment of every department to teaching and training. The Academic Committee takes meticulous care while planning this programme. Their efforts are focused not only on the basic training but also towards training the DNB students on how to become good clinicians and apply their knowledge to research. The Academic Committee and faculty of our hospital deserve everybodys admiration.
Every student is a potential teacher and every good teacher ensures furtherance of knowledge.

The DNB programme was started at Sir Ganga Ram Hospital in 1984 in five disciplines. In 1996 an Academic Committee was constituted under the chairmanship of Dr K.C. Mahajan, ably assisted by Dr P.S. Gupta and Dr S. Nundy. The Academic Committee was given the status of a Department of Academics in the year 2002. Today we cater DNB courses in 30 disciplines including 13 superspecialities and post-doctoral fellowships in 8 disciplines and is the largest in the country. The following are the activities of the department: Structuring and publishing a single comprehensive booklet every six months in January and July, which has the academic schedule of all the departments. The department ensures that the DNB curriculum is strictly adhered to along with periodic meetings with the DNB students of each department so that an adequate feedback is available. Arranging faculty lectures by outstanding people from India and abroad in their areas of expertise. These have a dual advantage as the hospital is exposed to the visiting faculty and the consultants and residents upgrade their knowledge. Encouraging CME activities which are recognized for accreditation by the Delhi Medical Council. The department also encourages the holding of seminars, workshops, panel discussions and journal clubs. The Department of Academics has a Medical Illustration Unit, headed by Mr Sansar Thakur, which offers the following services: Providing secretarial assistance to students and faculty for the preparation of slides and processing data for publication. Accessing the internet to make available material required to write papers. Providing the latest LCD/ DLP projection facilities. Making available a 24-hour internet service. Recording of clinical combined rounds, faculty lectures, first of the month clinical meetings, seminars, symposia and public awareness lectures. Taking digital photographs of patients in the OPD and wards. Scanning of X-rays/ CT/ MRI and 35 mm slides. Running a basic computer course for DNB students. Giving live demonstrations of various procedures. There is a permanent fibre-optic cable from the main Operation Theatre, Cardiac Operation

Theatre, Endoscopic Room, Academic Room and Department of Minimal Access Surgery to the main Auditorium and Seminar Room. This is manned by in-house staff. Conducting a clinical meeting on the first of every month in which individual departments present their achievements and latest trends in their respective fields. Four of these meetings in a year are in the form of clinicopathological conferences, for which the speaker is an invited faculty member from among the best known institutions of India. It is a matter of great pride that the hospital has such a large wealth of good and interesting clinical data, because of which the department has been able to regularly conduct clinicopathological conferences of high quality. Publishing a quarterly Hospital Newsletter, which highlights the hospitals academic and social activities, and also invites articles from consultants on subjects other than medicine. Ensuring an environment conducive to effective learning. The department has a library with a paper as well as an electronic section. Managing the hospital website and regularly updating information regarding the hospital.

The Sir Ganga Ram Hospital website went online in November 1998. Patients and other people interested in finding information about the hospital can visit the website from the comfort of their homes, offices or cyber cafs at any time of the day or night. The latest news about events in the hospital are featured on the HOME page. The DNB and nursing courses, as well as job vacancies, are advertised through the website with the option of downloading the application form. The results of academic courses are declared on the website, which also has the hospitals achievements and new facilities, meetings and conferences. The section About the hospital gives the background information, achievements, hospital statistics and future plans of the hospital. The contact details of the Board of Trustees, Board of Management and hospital administration are also available along with important telephone numbers.

First of the Month Clinical Meeting

The department conducts a clinical meeting on the first of each month where all departments in turn present their achievements and latest trends in their respective fields. The actual presentation should be of 40 minutes, well-rehearsed and coordinated so that it is within the time schedule and there is no redundancy of presentation. Clinical Combined Rounds

Clinical combined rounds are held every Tuesday between 11:30 a.m. and 12:30 p.m. in the Seminar Room. During each round, two departments present cases adhering, as far as possible, to the following pattern: The resident presents the history, examination findings, results of investigations and clinical diagnosis using computerized projection (12 minutes).

The Departments of Pathology and Imaging or other departments describe the contribution they have made to the management (5 minutes). The consultant in-charge of the case highlights its interesting features (2 minutes). The resident discusses the case in the light of the published literature and attempts to derive a message that is more widely applicable (6 minutes). There is then a short general discussion (5 minutes). Combined Medicosurgical Rounds

Combined medicosurgical rounds are held on every Friday between 11:30 a.m. and 12:30 p.m. in the Seminar Room. To make these meetings more beneficial for the participants, the following guidelines are suggested: Two well-prepared clinical cases should be presented and these cases should be of common interest and convey a message to the participants. The case history, physical findings and investigations should be presented by the postgraduate or resident doctor preferably with an LCD projector. Following the presentation, the diagnosis and differential diagnosis of the case should be discussed by the consultant-in-charge. Adequate time should be provided for general discussion, and questions and answers. A review of the literature, if necessary, may be given at the end. When help of the consultants from investigative and other departments is required during the presentation, they should be informed at least one day earlier so that they may come prepared.

In these meetings, three to four interesting cases of both clinical and academic interest are presented in detail. The clinicians present the clinical data and the concerned laboratory presents its findings followed by discussion and theoretical aspects of the case including the differential diagnosis. CARDIAC SURGERY

The Department of Cardiac Surgery has been recognized for the DNB training programme. Beginning from August 2001, two candidates per year are inducted for training, which is for three years.

A Brief Outline of the DNB Training Programme

The main thrust of training is to make candidates independent consultants. The training imparted is in the form of preoperative, operative and postoperative management of various types of cardiac surgical procedures. The department has one senior consultant, two junior consultants and four attending cardiac surgeons.

Requirements
Candidates will be expected to attend all ward rounds and operating sessions. They will be actively involved in preoperative assessment as well as post-operative management of cardiac surgical patients. Each student is required to maintain a log book in which all cases presented, surgeries assisted, surgeries performed, lectures, seminars and Journal Clubs attended are recorded.

Attendance
It is compulsory for students to attend all the lectures, seminars, tutorials and Journal Clubs.

Academic Programme
The department regularly holds lectures, Journal Clubs and seminars. With the induction of DNB candidates, the department will expand its academic programme. It will include faculty lectures, seminars, case presentations, Journal Clubs and discussion on operations. Faculty lectures will be held by consultants of the Departments of Cardiac Surgery, Cardiac Anaesthesia, Cardiology, General Medicine, Nephrology, Pathology, Microbiology, Radiology, Biochemistry and other supportive services. Seminars and Journal Clubs will be held regularly, in which DNB students, assisted by a consultant, will present a surgical topic and review current journals.

Brief Outline
The Department has a Genetics PhD and Fellowship programme and has an academic programme of seminars, journal club and introductory lectures. The Department also participates in the Journal Clubs and Grand Rounds of the Departments of Paediatrics, Medicine, and Obstetrics and Gynaecology. The following activities are planned for this session. We also participate in the Clinical Grand Round, which is attended by all Residents.

The Department of Clinical Haematology has the following academic activities scheduled from July 2012 to December 2012.

The teaching activities are held on Mondays and Thursdays from 11:00 a.m. to 12:00 noon. The case studies are usually bedside teaching for half hour, followed by discussion in departmental office 400B over the remaining half hour. In case of clinical emergency arising the session will be held at a later hour, preferably on the same day afternoon. The sessions are meant for DNB (Haematology) students, usually 2, Junior Resident (JR) and Intern from the department and also, at time residents from Department of Medicine join in. Besides, the Grand Round/Journal Club from Department of Medicine and allied specialities and the Tuesday/Friday meetings are also to be attended by the department.

The Department has a DNB course in clinical microbiology with one candidate every year. The training includes daily benchwork according to the posting schedule, participation in Journal Clubs, seminars, tutorials, training of the technicians and attending other academic activities of the hospital. A Brief Outline of the DNB Training Programme
The purpose of DNB training in Cardiology is to make the candidate an independent consultant in clinical cardiology and therapeutic decisionmaking. Basic training is also imparted in investigative techniques such as Echocardiography, Treadmill testing, Holter monitoring, exposure to Cardiac Catheterization and Interventional Cardiology, Intensive Coronary Care and Postoperative Intensive Care. During the period of their stay of 3 years, candidates are rotated through various disciplines such as the Coronary Care Unit, Intensive Care Unit, wards, Non-invasive laboratory and Cardiac Catheterization laboratory. Clinical and academic training includes bedside clinical case presentation and discussion, interpretation of ECGs and X-rays, regular Journal and Review Clubs. There is a Journal Club once a week, in which important articles from all leading journals of cardiology and cardiac surgery are presented by members of the department by rotation. The DNB candidate is asked to present brief reviews (2030 minutes) every 3 weeks on selected topics, e.g. cardiac anatomy and physiology, embryology, pathology, pharmacology, arrhythmias, therapeutics, interventional cardiology, paediatric cardiology, non-invasive cardiology and hypertension. A long review is presented (1-hour duration) every week.

Thursday: 10:00 a.m.12:00 noon The ward rounds in other wards are taken between 11:00 a.m. and 1:00 p.m. by different units. During their CCU and ward postings, DNB students attend ward rounds of the units to which they are attached.

Postings
DNB students in the Department of Medicine rotate through the Department of Cardiology for three months during their training. They spend 5 weeks in the wards, 5 weeks in the CCU and 2 weeks in the non-invasive cardiac laboratory. During their postings in the CCU and wards, they are expected to workup the cardiac patients and present them during ward rounds. They are encouraged to read about specific clinical problems and discuss them with consultants. During their non-invasive cardiac laboratory posting, they are encouraged to closely observe Echocardiography, Stress testing, Stress echo and Holter monitoring and interact with consultants working in the laboratory. In general, the students are not given an OPD posting unless the department is short of staff.

The academic programme for postgraduate students includes formal lectures, case discussions, seminars, Journal Clubs, experience in the wards and outpatient department, and in the operation theatre. Seminars/ Journal Clubs are held on a weekly basis in which a postgraduate student, assisted by a consultant, presents a surgical topic.

Requirements
Log books: Postgraduate students record all operations (assisted or performed) in their log books which are countersigned by the consultants. Case records: Attendance: A minimum attendance of 75% is mandatory.

Examinations
Board examinations: The DNB examinations in General Surgery are normally held twice a year. Our postgraduate students take the examination in August. Only one batch of students is admitted per year. Internal examinations: Internal examinations are held twice a year. The examination is based on the format of the DNB examinations.

Duties
Postgraduate students of all years perform on-call duties which average 6 per month under the guidance of a senior resident. The duties involve attending to emergencies in the ward as well as in the casualty.

Outpatient Teaching Schedule

Postgraduate students attend outpatient clinics thrice a week along with the rest of the unit in which they are posted.

In addition to daily ward rounds, teaching ward rounds are conducted by the consultants as per the following schedule.

The curriculum of DNB Hospital Administration aims to make the trainee knowledgeable about the basic principles of management, improve their skills in the art and science of management. It also aims to give them a comprehensive working of the elements of hospital functioning, functioning of various departments and facilities with a special understanding of the matrix of their interdependence. Trainees are required to learn: 1. To be able to understand the various processes of hospital functioning. 2. To acquire the art and science of management. 3. To accquire skills to identify bottlenecks and problems in hospital processes and work out solutions for them. 4. To create quality, efficiency and effectiveness of tasks at various levels of functioning. 5. To identify challenges in the hospital system and propose growth and development in the wake of changing hospital scenario. 6. To be proficient in all modern management techniques and their application in day-to-day affairs of the hospital. 7. To review the literature and modern developments in healthcare with emphasis on hospital care. 8. To acquire teaching skills and data presentation and to be the source of knowledge transfer and technical skills to PG students, interns, residents and nursing students. 9. The DNB Neonatology programme has been running in the department for the past 7 years. Department has also started NNF fellowship (1 year) from this year (2012) in which two students are enrolled every year. DNB students from the Department of Paediatrics come for rotation for a period of 6 months out of their total posting of 3 years in Paediatrics. There is a morning round in which all patients are discussed in detail. Case presentation is done on Thursdays. DNB students from the Department of Gynaecology and Obstetrics are posted with us for a period of 2 weeks and selected topics are taught to them by the consultants of the department. The department is also involved in the teaching programme of the nurses posted in the department. Every Thursday between 1:00 p.m. and 2:00 p.m., one of the consultants takes their class on relevant topics. These classes are also attended by the residents of the department. Department has regular observers which are trained in the department for three months.

Outline The Institute of Minimal Access, Metabolic and Bariatric Surgery is a leading centre of excellence in its field, in South-East Asia. The Institute is actively involved in innovating surgical care in its domain and disseminating its expertise through formal and informal academic activities. The Institute conducts the following training programmes: a. Post-Doctoral Fellowship The post-doctoral fellowship programme (F.N.B.) in Minimal Access Surgery is conducted under the National Board of Examinations (NBE) for a period of two years. Two fellows are admitted each year through a national selection process based on academic results. The purpose of the fellowship is to train fellows in the science of advanced minimal access surgical care with a view to make them independent clinicians/ surgeons. There will be stress on the acquisition of academic knowledge and practical skills during the fellowship. 1. The fellows are expected to be proficient in relevant endoscopic surgical anatomy of the areas studied and minimal access surgical physiology (within 3 months). 2. The fellows are expected to choose a research topic of interest with the assistance of their guide, which will be their dissertation towards qualifying for their post-doctoral fellowship (within 3 months). 3. The fellows are expected to be proficient in the knowledge of basic and advanced minimal access surgical equipment (within 6 months). 4. The fellows are expected to be proficient in the theoretical and practical knowledge of basic minimal access surgical procedures (within one year). 5. The fellows are expected to be proficient in the theoretical and practical knowledge of advanced minimal access surgical procedures (within 24 months). The fellows will witness, assist and perform minimal access surgery in a graded format. The fellows are to maintain a properly indexed log book indicating work done, academic presentations made and internal assessments. There will be a periodic assessment of clinical aptitude and academic performance. Progress during the fellowship is contingent on the fellows global performance. The fellows are required to attend all ward rounds, document clinical activity (electronic and paper), operating sessions, academic sessions, undertake active research, present and publish.

b. Basic Minimal Access Surgical Course A formal basic training course in Minimal Access Surgery is held every two months. The duration of the course is five days. c. Postgraduate Course A postgraduate introductory course is specifically designed keeping in mind the basic needs of students appearing for postgraduate examinations. The duration of the course is three days, to be held on the last Monday of every month. d. Endohernia Course Endoscopic hernia repair is being increasingly performed by surgeons practicing minimal access surgery. A large volume of these procedures are conducted in our department. In an effort to provide high quality practical training, the department has established a structured training course in various aspects of laparoscopic hernia repair. The duration of the course will vary from 4 to 8 weeks (depending on the experience of the surgeons). The above fellowship is stipendiary. Endoscopic hernia repair is being increasingly performed by surgeons practising minimal access surgery. In an effort to provide high quality practical training, our Institute established a three-day intensive course focusing on latest technology and development in endohernia surgery. Advanced minimally invasive techniques including laparoscopic totally extraperitoneal hernioplasty (TEP), laparoscopic repair of ventral/incisional hernias and controversies in hernia surgery will be addressed. It will also consist of lectures on various approaches to hernia surgery and the latest advances in its development. All participants will receive a certificate of participation. e. Bariatric Surgery Course The course is organized, keeping in mind the new treatment options available in the bariatric field and to develop treatment modalities of morbid obesity in India. It is designed to meet the educational needs for laparoscopic and Bariatric surgeons. The duration of the course is two days. f. Observership Observership courses where a batch of six observes undergo two weeks training.

g. OT Surgical Staff Training Course Minimal access surgery is the future. It is a highly specialized, technology driven surgery that requires special training and exposure to sophisticated endovision equipment and hand instruments. The department is committed to provide educational opportunities to staff nurses and OT technicians to enhance their abilities, interests and knowledge in minimal access surgery. The duration of the course is two weeks. h. Research These positions are offered to motivated doctors and medical students for research. Training Courses Overview 1. Series of discussion about basic instrumentation, their maintenance and use, surgical procedures, complications and prevention of complications. 2. Acquisition of skills for laparoscopic surgery by endotrainer and simulators so that trainees get familiar with operating under 2D videoscopic guidance and learn how to manipulate instruments without injuring structures and to appreciate operating without depth perception. This helps in developing handeye coordination in an unstressed environment. 3. Active involvement in the operation theatre, initially observing operations performed by proficient surgeons and then assisting them during surgery in a graduated manner. 4. Training seminars and case discussions, which include various intricacies of surgery. Appropriate clinical assessment of every case is of vital importance in any laparoscopic surgery. 5. Video discussions, consisting of group discussions by the department on previously operated patients, unedited video recordings, critical evaluation of procedures, technique of handling difficult and unexpected situations. The department has an exhaustive video/CD library consisting of a large collection of videos/CDs (edited/unedited) of basic and advanced laparoscopic surgical procedures recorded digitally for teaching candidates and future references. 6. Monthly audit of all the cases performed in the previous month, to check on case assessment, instrument assessment, procedural difficulties, complications as well as discuss methods to improve patient care. 7. Basic reading of general and some specific procedures from textbooks as well as selected papers from latest journals. 8. Exposure to the latest developments in the field of minimal access surgery. 9. The Department of Medical Oncology was started in 1997. The department is involved in the management of haematological malignancies and solid tumours. It provides cancer chemotherapy and immunotherapy. The department is also involved in conducting autologous as well as autogenic bone marrow transplants. It is also involved in teaching DNB students of various disciplines. The department participates in the Grand Medical Round and Journal Club with the Department of Medicine.

DNB training in Family Medicine
The goal of Family Medicine training is that after qualifying the final examination, the candidate should be able to function as a junior consultant (specialist) in Family Medicine. He should be able to render health services to the community by providing primary healthcare to all members irrespective of age, sex, specialty and socioeconomic background. He should be able to decide for appropriate referral in order to provide secondary/tertiary health services when necessary. He should be clinically competent and should be able to take personal responsibility for rendering comprehensive and continuing care of his patients in their own family settings. The training in the Medicine department will be as per schedule of the training programme of DNB (Medicine). Brief Outline

The curriculum of the department aims to make the trainees independent clinicians/consultants. The objectives of the training are that trainees should: be acquainted with the current literature on relevant aspects of basic, investigative and clinical neurosciences. have acquired performance skills and the ability to interpret relevant clinical investigations. be able to diagnose, plan investigations and treat common conditions in the specialty by relevant current therapeutic methods. be acquainted with allied and general clinical disciplines to ensure appropriate and timely referral. be acquainted with relevant education delivery systems. be able to identify, frame and carry out research proposals in their specialty. be aware of their limitations. Emphasis will be on-the-job training rather than didactic lectures. Training imparted will be in the form of departmental seminars, Journal Clubs, combined neurology and neurosurgical conferences, bedside case discussions, and practical training in clinical neurophysiology techniques and neuroradiological procedures. Neuroanatomy, neurophysiology and relevant clinical aspects will also be taught in the form of departmental seminars. In the first 6 months, trainees are expected to know neuroanatomy, neurophysiology, clinical localization and basics of EEG, EMG, nerve conduction/evoked response studies and Doppler studies. By the end of the first year they are expected to know about various disease processes, their pathophysiology, pharmacology, neurochemistry and EEG/EMG changes. The second year is spent reviewing the important literature published in old and new journals and analysing it critically.

Neurobiochemistry and neuroimmunology

Didactic lectures will be held by trained professionals to familiarize the trainees with the elements and techniques of neurochemistry and neuroimmunology.

Neuropathology
Trainees will get ample opportunity to perform muscle and nerve biopsies,

and also to study them with the pathologist. For the brain cutting session, they will attend neuropathology sessions every Friday in AIIMS under a trained neuropathologist. They will be rotated to the Department of Neuropathology either in PGI, Chandigarh or AIIMS to familiarize them with the techniques of grossing, staining procedures, autopsy methods and tissue processing, including frozen sections.

Neurophysiology
Trainees are required to learn the techniques of application of EEG/EMG and evoked response machines under the guidance of technical assistants and consultants. They are also expected to learn the interpretation of nerve conduction studies, EMG, evoked response and ultrasound during their first year of training, interpretation of EEG records, to report under the guidance of senior colleagues and consultants in the first year, and independently in the second year of training.

Requirements
To be eligible for the final examinations, candidates are required to have: a properly indexed log book indicating the work done, rotations and internal assessments one paper, a review of available clinical material and a laboratory-oriented project written in the form of an article for Neurology (India).

Evaluation
Teachers concerned with the training of candidates both inside and outside the parent department evaluate them independently. A standardized marking system is followed and the result is entered in a log book. The Course Director averages these independent evaluations and enters the final evaluation.

Brief Outline
The DNB programme is a 3-year course after internship. Students get training in clinical work; they learn to present cases, participate in Journal Clubs and prepare seminars. They do research and write a dissertation. They work as junior residents and do emergency duties twice a week. Students who have already passed the DGO are trained for one year and are given the work of third-year junior residents. They work in rotation in different departments for two weeks each. Seniors assist students to perform various minor and major operations.

Requirements
It is mandatory for the students to maintain a log book.

Examinations
Terminal examinations are held every three months. The dates for the external examinations will be set by the National Board of Examinations. On last Saturday of every month an examination is held from the topics discussed.

The teaching schedule of the Department of Ophthalmology will include regular lectures by the consultants of the department, clinical teaching by the senior residents and presentation of seminars and Journal Clubs by residents. A viteoretinal fellowship programme is also running in the department and there will be special classes for the same.

Requirements

The students will maintain books in which all cases presented, lectures/ seminars/ Journal Clubs and emergencies attended during duties will be recorded. The log book will be checked once a week by the Head of the Unit and once a month by the Head of the Department.

Attendance
A minimum attendance of 75% is needed for all the above-mentioned disciplines in the Orthopaedic Training Schedule, failing which DNB students will not be issued the certificate necessary to appear for the examination.

Radiology Conference
Each postgraduate will be required to attend the Radiology Conference held every week on Thursday. Postgraduates are also required to attend an interdisciplinary clinical meeting of all the departments in the hospital held once a month. They will also attend other disciplines as per schedule in the DNB course rotation.

Examinations
A theory test is held on the last Friday of February, April, June, August, October and December covering topics according to the schedule.

Brief Outline
The DNB course in ENT is spread over 3 years. The aim is to produce a complete Ear, Nose and Throat Specialist, well-versed in the theoretical aspects, as well as a competent and safe surgeon.

An attempt is made during the period of the course to impart an overview of the standard teachings as well as recent advances in the specialty. The student is actively involved in various surgical procedures and total patient care, ranging from OPD work-up, writing of case records, surgery and postoperative care. In the course of these three years, the trainee is expected to inculcate a clear and comprehensive knowledge of the fundamentals of otorhinolaryngology. develop clinical skills in patient care in the OPD, casualty and wards in order to diagnose, investigate, plan and execute their medical and surgical treatment independently. to acquire skills to do basic and clinical research in otorhinolaryngology. to acquire teaching skills to be able to impart knowledge and technical skills to undergraduates, postgraduates and nursing students. to develop a sense of self appraisal, learning and advancement through literature and CME programmes in the field of otorhinolaryngology. Clinical skill enhancement the outpatient department of otorhinolaryngology is equipped with an operating microscope, nasal and fibre-optic laryngoscope which are attached to an endoscopic camera and monitor with a recording system. Diagnostic as well as minor procedures are being carried out by the trainees independently under the supervision of the senior residents/ consultants. in the operation theatre, all surgeries can be viewed on the monitor for learning as well as for its supervision when being performed by the trainees. Navigation image-guided surgery is also being conducted which allows the trainee to correlate CT scan anatomy of the nose, paranasal sinuses and skull base, with an online endoscopic surgical one. temporal bone dissection is being carried out by the trainees in the clinical skills centre. the trainees attend and participate in various CME surgical workshops which are being conducted by the department periodically and annually.

These are ear surgery and temporal bone dissection, cochlear implant surgery and endoscopic sinus surgery. Acquiring skills emergencyto be able to independently/ under supervision handle emergency situations such as foreign body removal, epistaxis, trauma, tracheostomy, intubation and others surgeryto perform independently but under supervision, surgical procedures such as tympanoplasty, mastoidectomy, myringotomy, adenoidectomy, tonsillectomy, septoplasty, FESS, MLS, biopsies and others to have assisted in surgeries such as the cochlear implant, middle ear implant, parotidectomy, thyroidectomy, laryngectomy, maxillectomy, neck dissection, laser surgeries for voice disorders and other indications, tracheal stenosis, choanal atresia, optic nerve decompression, orbital decompression, DCR, pituitary fossa surgery, anterior and lateral skull base surgery, CSF rhinorrhoea and others.

Requirements
The DNB student maintains a log book in which all cases seen and discussed, all operations assisted or performed, are entered and countersigned by the consultant. The student is supposed to attend the hospital 6 days a week from 8 a.m. to 4 p.m. The student does one emergency duty per week on an average, with Sunday duty by rotation.

Examinations
Every 2 months, a multiple choice examination of the topics covered is held and grades are assigned. Every 6 months, a clinical examination consisting of long and short cases, is taken.

The Department of Plastic Surgery has 3 units. The department comprises an Emeritus consultant, 2 senior consultants, 4 consultants, 2 associate consultants. There is a Surgical Skills centre with an operating microscope to learn microvascular surgery and endotrainer to learn endoscopic surgery. The ward round in the units starts at 9 a.m. on Monday, Wednesday and Saturday. The department has five operating days on Tuesday, Wednesday, Thursday, Friday and Saturday. The department runs a charity projectSmile Train Sir Ganga Ram Hospital, Cleft Project for free operations of poor patients with cleft lip and palate. The department conducts its academic meetings on Mondays, Saturdays and on second Mondays. The faculty takes part in delivering lectures and coordinating seminars with DNB students.

The 3-year DNB course in Radiodiagnosis is aimed at imparting training in both conventional radiology and modern imaging techniques so that the candidate is fully competent to practise, teach and do research in the broad discipline of radiology including ultrasonography, computed tomography, PET CT, nuclear imaging, and magnetic resonance imaging. On completing the course, a resident should be able to acquire a good basic knowledge of various subspecialties of radiology, to independently conduct and interpret all routine and special radiological and imaging investigations. The training is spread over 3 years and includes the following components: 1. Rotational posting in various subspecialties. 2. Seminars, Case discussions, Journal Clubs, Film quizzes. 3. Research methodology and dissertation.

Requirements
A log book will be maintained by each resident in which he/she is required to record investigations performed independently, case discussions, Journal Clubs and seminars presented. The log book will be signed by the respective consultant at the end of each rotation and will be presented to external examiners at the time of the final examination.

Attendance
A minimum attendance of 75% is needed in all postings in various subspecialties.

Academic Programme
Weekly seminars, Journal Clubs and case discussions will be held where the residents are required to prepare pre-selected topics under the guidance of a consultant and present it to the full house. In addition, day-to-day assessment of performance of the procedure and reporting by the resident will be done by the consultants in various subspecialties. At the end of each posting, the resident will be assessed by a film quiz. Residents are also posted in the Department of Pathology to study specimens so as to correlate the gross morphology of pathological conditions with imaging features.

Clinical Meetings/Conferences
These are held regularly every 2nd and 4th Thursday. The consultants present cases of academic interest and discussions are held along with the clinician

RENAL TRANSPLANT SURGERY

The Department of Renal Transplant Surgery has been in existence since 1988 and has conducted over 2500 successful live-donated and cadaveric renal transplants. It has also conducted cadaveric kidney transplants. It has a number of firsts to its credit, e.g. the introduction of CAPD in India. In 2010, 300 patients with CRF underwent CAPD catheter insertion. There are no postgraduate students in the department and therefore there is no formal teaching programme. However, the department participates in and conducts lectures/seminars and has informal teaching sessions for PGs in the Departments of Nephrology, Urology and Surgery. Dr H. Jauhari has been appointed coordinator for conducting an interactive hands-on course on the art of surgery and basic surgical skills for DNB and post-fellowship students of all surgical specialties. The purpose of the programme is to train residents to manage patients with complex surgical disorders of the gastrointestinal tract, liver and pancreas. They will witness different operative procedures and there will be stress on the practical as well as the academic approach to such problems. The trainee will have to maintain good records, both paper and electronic, and should be able to derive messages as well as write papers after analysing these. The residents will also be exposed to liver transplantation procedures.

Urology is one of the superspecialties with a very fast growth in instrumentation, innovation and practically applicable procedures. These procedures have yielded satisfying results for conditions relating to the entire genitourinary tract. Public awareness about this has placed an ever-growing demand on the knowledge and skills of practising as well as trainee urologists. It is equally important for young trainees in surgery to keep abreast of the current concepts and newer developments, besides the basic principles of urology and time-tested conventional procedures. The present programme aims at educating the trainee in upgrading his or her acumen and skills in the art and practice of urology with a rational approach. For the consultant, it aims at updating knowledge of the latest innovations and techniques in order to offer the best to the patient while monitoring and, where needed, rectifying patient care and teaching and training of the residents. In spite of easy access to the literature and self-learning opportunities available today, more than ever before, there is a great need for bedside presentations and problem-based discussions, particularly for DNB trainee residents. With the DNB (Urology) having completed 8 years since inception, the lessons learnt have been utilized in the current Academic Programme. With an increasing number of complicated cases being referred which need special urological care, it is time to start a training programme for operation theatre personnel and nursing staff. Residents can be effectively utilized in this endeavour.
