1.0 PROJECT INFORMATION

1.1 Purpose and Scope

This Traffic Management Plan (TMP) provides the traffic management procedures to be followed
by the contractors and subcontractors associated with the project. The traffic control devices will
be established on-site by Traffic Services Pty Ltd prior to commencing work on the road. The
devices shall be inspected periodically throughout the day and will be taken down at the end of the
working day.
The project involves the construction of a 550mm sewer main on the eastern road verge of
Wanneroo Road from the manhole at the intersection of Gnangara Road to the manhole opposite
Temporary Gnangara Pump Station No. 902-32, a length of 80 metres.
The works include the reconstruction of the 2.0 metre wide dual-use path from Gnangara Road to
the bus shelter. t also includes the construction of a new concrete driveway at the northern end of
the bus embayment.

1.2 Project Location
The Wanneroo Road/Gnangara Road intersection is located in Madeley, within the City of
Wanneroo. The work area is indicated in Figure 1.0, below.

1.3 Site Constraints/Impacts

Wanneroo Road is under care, control and management of MRWA and is a primary distributor
carrying a large portion (approx 15%) of commercial and industrial traffic from the northern suburbs
to the City. The current daily weekday traffic flow on Wanneroo Road is approximately 34,846 vpd.
Details of traffic volume counts are contained in Appendix A. Gnangara Road is a local
distributor road, carrying approximately 3,000 vehicles per day.
MRWA's Traffic Management for Works on Roads Code of Practice requires the retention of the
number of traffic lanes on all roads adjacent to the works to comply with the requirements of
Australian Standard AS 1742.3-2002.

1.4 Traffic Management Objectives and Strategies

The objectives of the TMP are to:
Provide for a safe environment for all road users;
Provide protection to workers, visitors, agents of the Contractor and the general public from
traffic hazards that may arise as a result of the construction activity;
Minimise the disruption, congestion and delays to all road users;
To ensure network performance is maintained at an acceptable level throughout the term of the
work;
Ensure access to adjacent commercial premises is maintained at all times.

To achieve the above objectives, the Traffic Management Plan will:
Ensure whenever possible, that a sufficient number of traffic lanes to accommodate vehicle
traffic volumes are provided.
Ensure that delays and traffic congestion are kept to a minimum and within acceptable levels
Ensure that appropriate/sufficient warning and information signs are installed and that
adequate guidance is provided to delineate the travel paths through the work site.
Ensure that the work area is free of hazards and that all road users are adequately protected
from excavations and obstructions.
Ensure that all needs of road users, motorists, pedestrians, cyclists, public transport
passengers and people with disabilities are accommodated at and through the work site.
Provide for work activities to be undertaken sequentially to reduce the adverse impacts of the
work.
Provide for safety procedures to enable work personnel to enter and leave the work area in a
safe manner.

1.5 ResponsibiIities

Bob Plumbers Pty Ltd will take the utmost care to prevent the risk of injury and/or property damage
to employees, subcontractors, other contractors, road users and members of the public.

Work will not commence until all appropriate signs, devices and barricades are in place and in
accordance with the requirements of the Traffic Management Plan. All necessary signs and traffic
control devices will be installed at the work site to direct and regulate traffic movements around the
work activity and ensure that adverse impacts associated with the works are kept to a minimum.

2.0 WORKS ON ROAD

2.1 Project Scope

Scope of Project The project involves the construction of a 550mm sewer main on the eastern road
verge of Wanneroo Road, south of Gnangara Road for a distance of 80 metres.

The works to be undertaken involve the excavation of a trench from the south-eastern corner of Wanneroo
Road and Gnangara Road on the eastern road verge for the construction of a 550mm sewer main.
t will be necessary to demolish and remove the existing concrete dual-use path on the eastern road verge
between Gnangara Road and the bus shelter, which is on the alignment of the sewer. The trench will need
to be excavated with the excavator positioned adjacent to the kerb because of the existing retaining wall on
the street alignment boundary. The 550mm sewer main is to be laid to the design levels and gradients.
Following the backfilling and compacting of the trench, the 2.0metre wide dual use path and concrete cross-
over is to be constructed.
Staging of Work
The work will be progressed in the following stages:
Stage 1 - Demolition and removal of the concrete footpath on the eastern side of Wanneroo Road
between Gnanagra Road and the Bus Stop. Removal of the Bus Shelter. The truck removing
the spoil and delivering material to the site will be contained in the bus embayment.
Stage 2 - Excavation of trench to lay the 550 mm sewer main from the manhole located on the south-
eastern corner of the intersection. The excavation will be backfilled and the excavation made
safe by 2.30pm. Work (excavation and pipe laying) will commence at the manhole opposite the
pump station until the end of the work day at 5.50pm.
Stage 3 - The excavation and pipe laying for the sewer main linking the two sections will be completed.
The backfilled trenches will be compacted in layers.
Stage 4 - The formwork for the concrete path is to be set-up. Concrete will be placed for the dual-use
path and driveway to the corner property.
Stage 5 - Re-instatement of grassed verge and bus shelter.

The specification for Contract 234/06 limits work to between 07:00 and 17:30.
Other ssues The project is scheduled to be completed over a one week period, with the works being
undertaken between 7.00am and 5.30pm, Monday to Friday. Traffic lane closures will only
be implemented between 9.30am and 2.30pm when traffic volumes will be lower than those
experienced during peak periods. Off-road works, and works not requiring traffic lane
closures will be undertaken during the peak hours.
Traffic management will be designed and implemented by Traffic Services Pty Ltd,
Reviewed by an RTM in accordance with the MRWA Code of Practice, with Suitability and
Compliance Audits being conducted independently by J B Consulting (refer front cover).

2.2 Existing Traffic and Speed Environment

The current daily weekday traffic flow on Wanneroo Road is approximately 38,222 vpd. Details of
traffic volume counts are contained in Appendix A. The existing speed limit for that section of
Wanneroo Road, the subject of the proposed roadworks, is 70km/h. Gnangara Road is a local
distributor road carrying approximately 3,000 vehicles per day where the sign posted speed limit
is 70 km/h.
Part of the works will necessitate the closure of the southbound kerbside traffic lane in Stage 2 of
the works to provide a safe workplace for the construction plant associated with the sewer
construction.
The remaining part of the works will necessitate only a partial closure of the kerbside traffic lane
(4.8 metres wide) to maintain a 1.2 metre clearance to the traffic lane. Traffic cones will be
established to delineate the edge of the traffic lane.

2.3 RoIes and ResponsibiIities

The Project Manager has the ultimate responsibility and authority to ensure the TMP is
implemented for the prevention of property damage and injury to employees, contractors, sub-
contractors, road users and all members of the public. He will ensure all site personnel are fully
aware of their responsibilities, and traffic-controllers are appropriately trained and accredited. He
will ensure that sufficient controllers are available to ensure appropriate breaks are taken.

All personnel engaged in the field activities will follow the correct work practices as required by
AS1742.3. The Superintendent's Representative shall be notified should a situation arise that is
not covered by this TMP or AS1742.3.

The following outlines the management hierarchy that will apply to the project.

3.0 STATUTORY REQUIREMENTS

3.1 OccupationaI Safety and HeaIth

Principals, employers and persons in control of workplaces have a statutory duty of care to provide
a safe workplace for all personnel working at the site, accessing the site or impacted by the
construction activity including employees, contractors, subcontractors, visitors to the site and the
general public.

This TMP forms part of the overall project Safety Management Plan, and provides details on how
all road users considered likely to travel through, past, or around the worksite and those impacted
by the works will be safely and efficiently managed for the full duration of the site occupancy and
works.

All traffic management works and control devices shall be in accordance with
OS&H Act (1984)

3.2 ResponsibiIities

3.2.1 Project Manager
The project manager shall:
Ensure all traffic control measures for this TMP are placed and maintained in accordance with
this plan and the relevant Acts, Codes, Standards and Guidelines
Ensure suitable communication and consultation with the affected stakeholders is maintained
at all times
Ensure inspections of the Traffic Controls are undertaken in accordance with the TMP, and
results recorded. Any variations shall be detailed together with reasons
Arrange and/or undertake any necessary audits and incident investigations

3.2.2 Supervisor
The supervisor is responsible for overseeing the day-to-day activities, and is therefore responsible
for the practical application of the TMP, and shall:
nstruct workers on the relevant safety standards, including the correct wearing of high visibility
safety vests, safety boots and other equipment as required (See 3.2.4).
Ensure traffic control measures are implemented and maintained in accordance with the TMP
Undertake and submit the required inspection and evaluation reports to management
Render assistance to road users and stakeholders when incidents arising out of the works
affect the network performance or the safety of road users and workers
Take appropriate action to correct unsafe conditions, including any necessary modifications to
the TMP.

3.2.3 Traffic Management PersonneI
Traffic Services Pty Ltd will ensure that at least one person on site shall be accredited in Basic
Worksite Traffic Management, and shall have the responsibility of ensuring the traffic management
devices are set out in accordance with the TMP. A person accredited in Advanced Worksite Traffic
Management shall be available to attend the site at short notice at all times to manage variations,
contingencies and emergencies, and to take overall responsibility for traffic management.

3.2.4 Traffic ControIIers
Traffic Controllers shall be used to control road users to avoid conflict with plant, workers, traffic
and pedestrians, and to stop and direct traffic in emergency situations. Traffic Controllers shall:
Operate in accordance with Section 4.6 and Appendix B of AS1742.3
Hold a current Traffic Controller's accreditation in Western Australia.
Take appropriate breaks as required by AS1742.3 and/or OS&H Regulations.

3.2.5 Workers and Subcontractors
Workers and Subcontractors shall:
Correctly wear high visibility vests, in addition to other protective equipment required (e.g.
footwear, eye protection, helmet, sun protection etc), at all times whilst on the worksite
Comply with the requirements of the TMP and ensure no activity is undertaken that will
endanger the safety of other workers or the general public
Enter and leave the site by approved routes and in accordance with safe work practices.

3.3 PersonaI Protective Equipment

All personnel entering the work site shall correctly wear high visibility vests to AS/NZS 4602, in
addition to other protective equipment required on a site-by-site basis (e.g. protective footwear, eye
protection, helmet, sun protection, respiratory devices etc) at all times whilst on the worksite.

3.4 PIant and Equipment

All plant and equipment at the workplace shall meet statutory requirements and have the required
registration, licences or certification where required. All mobile equipment shall be fitted with
suitable reversing alarms. All mobile plant and vehicles shall be fitted with a pair of rotating
flashing yellow lamps in accordance with AS1742.3 clause 3.12.1.

3.5 Incident/Accident Procedures

n the event of an incident or accident, whether or not involving traffic or road users, all work shall
cease and traffic shall be stopped as necessary to avoid further deterioration of the situation. First
Aid shall be administered as necessary, and medical assistance shall be called for if required. For
life threatening injuries an ambulance shall be called on telephone number 000. The Police shall
also be called on 000 for traffic accidents where life threatening injuries are apparent. Any traffic
crash resulting in non-life threatening injury shall immediately be reported to the WA Police Service
on 131444.

Broken down vehicles and vehicles involved in minor non-injury crashes shall be temporarily
moved to the verge as soon as possible after details of the crash locations have been gathered
and noted. Where necessary to maintain traffic flow, vehicles shall be temporarily moved into the
closed section of the work area behind the cones, providing there is no risk to vehicles and their
occupants or workers. Assistance shall be rendered to ensure the impact of the incident on the
network is minimised. Details of all incidents and accidents shall be reported to the site supervisor
and project manager using the incident report form at Appendix D (or similar).

3.6 Trip Hazards

The worksite and its immediate surroundings shall be secured by the installation of plastic barrier
mesh fencing during and after work hours. Unauthorised access to the work site is prohibited.

Pedestrians will be re-directed across Gnangara Road to the temporary bus stop north of the
intersection. The road verge will be stripped and compacted to provide a temporary surface
suitable for pedestrians.
4.0 PLANNING

4.1 Risk Identification and Assessment

Risk analysis of the proposed works has identified a number of risk events/items that will be
managed by effective traffic management planning and the implementation of this TMP. A risk
analysis table is attached at Appendix B. The assessment process has been undertaken in
accordance with Australian Standard AS/NZS 4360-2004, Risk Management.
All identified risks have been treated by development of this TMP. Unforseen risks arising during
the works will be treated in accordance with standard work practices and procedures where
appropriate.

4.2 LegisIative and Other Provisions

Traffic Services Pty Ltd recognises that the traffic management plan has been developed and shall
be implemented with due consideration and in accordance with the following legislative,

Traffic Services Pty Ltd shall ensure that the requirements of these documents and other relevant
information will be monitored and the Traffic Management Plan adjusted to meet changing
requirements where necessary.

4.3 Traffic Assessment (VehicuIar Traffic)

4.3.1 VoIume and Composition
Volumes used in this report are based on average Monday to Friday traffic figures derived from
historical counts. Australian Standard AS1742.3 Traffic Control Devices for Works on Roads
indicates that the mid block capacity of multi-lane roadways is 1,000 vehicles per lane per hour
(vplph) and 500 vehicles per hour within 200m of an intersection for each lane.
The traffic flows (See Appendix A) show that the two traffic lanes on Wanneroo Road are to
remain open during the majority of the daylight hours. The proposed closure of one traffic lane to
enable the works to be undertaken will be limited to between 9.00am and 2.30pm where the
southbound traffic volumes fall below 1300 vehicles per hour. A variation to the requirements of
AS 1742.3-2002 is being sought. A minimum traffic lane width of 3.5 m will be maintained
throughout the term of the work. This will accommodate the truck traffic using this section of
Wanneroo Road.

4.3.2 Existing & Proposed Speed Zones
The posted speed limit is currently 70kph. The traffic lane closure will enable workers and plant to
be contained a minimum of 1.2 metres clear of moving traffic. t will however, necessitate the
introduction of a temporary speed limit of 60kph past the work site.

4.3.3 Intersection Capacity
The work is located immediately past the intersection and as such, the closure of the slow
kerbside through traffic lane is required prior to the intersection. Southbound traffic volumes
recorded are in the order of 1 900 vph during peak hours. t is likely that allocation of additional
green time for this movement will be made for the phasing of the traffic signals. This will
accommodate the additional traffic in the single traffic lane.

t is proposed to retain the single traffic lane southbound through the intersection between the
hours of 9.00am and 2.30pm when the traffic volumes fall below that of the peak hours (See
Appendix A).

4.3.4 Existing Parking FaciIities
There is no parking permitted within the carriageways of Wanneroo Road and Gnangara Road
where the works are being undertaken.

4.3.5 Heavy and Oversized VehicIes and Loads
Vehicle classification data indicate that approximately 15% of traffic is commercial vehicles. This
road is not a designated heavy haulage route or 'high-wide load' route, however all temporary
pavements and changes of alignment will be designed to cater for semi-trailer traffic.

4.3.6 PubIic Transport
Wanneroo Road is a public transport route with a bus stop being located south of the intersection.
The bus stop will be temporarily relocated to the north of the intersection. A hard-stand area will
be provided at the bus stop. Path Transit will post notices to users of the bus services of the
temporary changes in the week prior to the commencement of the work.

4.3.7 SpeciaI Events and Other Works
Contact with the Local Government Authority and service providers have indicated that there are
no other works expected in vicinity to the construction site. As such no impacts are expected.

4.4 Non-motorised Road Users

4.4.1 CycIists and Pedestrians
Site observation indicates that cyclists use the wide kerbside lane as an on-road facility. Cyclists
will be assisted past the work site by traffic controllers. Bus patrons are the only likely pedestrians
in the vicinity of the works (See Public Transport).

4.4.2 PeopIe with DisabiIities and Other VuInerabIe Road Users
There is no specific facility or service nearby that would increase normal use of the road facilities
by people with disabilities and other vulnerable road users.
There are no schools in the vicinity of the worksite, and no significant numbers of children are
expected.

4.4.3 SchooI Crossings
There are no school crossings in the vicinity of the worksite.

4.5 Site Assessment

4.5.1 Access to Adjoining Properties
Vehicular access to the Garden Supplies on the east side of Wanneroo Road, north of Gnangara
Road will be maintained. The layout of the traffic cones will accommodate vehicle entry and exit
from the property.

4.5.2 EnvironmentaI Conditions
Weather:
(Rain, Floods, Heat, Sun Glare, Fog)
Stormwater drainage for the existing road pavements has been installed; the works involve the
extension of the system to cater for the new road pavement, hence no significant flooding or water
sheeting is expected.
Wanneroo Road is oriented north-south; it is unlikely that sun glare would be problem for road
users. Fog is also an unlikely occurrence in this area.

Road Geometry / Terrain:
(Horizontal and Vertical approach geometry, Safe stopping distances, Visibility, Vegetation)
The road is flat with ample sight distances on the approaches to the worksite. There are no grades
to affect deceleration or acceleration of vehicles and current geometry meets safe stopping sight
distances for the existing 70km/h zone.

Existing Signage:
(Obstruction, Visibility of temporary signage)
There are 70km/h speed limit signs erected just outside the work area, these signs will serve to re-
establish the speed limit past the work site.
There are no other traffic or advertising signs in the vicinity which could cause distractions or
confusion, or which restrict sight lines.

Other:
(Structures, Dust, Noise, Fumes)
There are no structures affecting sight lines or access. The surrounding environment is built-up
and/or landscaped. There are no significant dust-producing elements. The works being
undertaken are Off-road works and will not generate excessive dust.
The work is being carried out during normal weekday daytime hours. There are no nearby
facilities, which require specific noise limiting measures. There are no specific noise restrictions by
the Environmental Protection Agency.

4.5.3 Impact on Adjoining Road Network
t is anticipated that traffic congestion will occur when only one traffic lane southbound is in
operation. A Variable Message Sign (VMS) will be erected in advance of the preceding
intersection to alert motorists to the possible delays and to select an alternate route.

4.6 Works Programming

4.6.1 Work Sequence

Stage 1. Demolition and removal of the concrete footpath on the eastern side of
Wanneroo Road between Gnangara Road and the Bus Stop. Removal of
the Bus Shelter. The truck removing the spoil and delivering material to
the site will be contained in the bus embayment.

Stage 2. Excavation of trench to lay the 550 mm sewer main from the manhole
located on the south-eastern corner of the intersection. The excavation
will be backfilled and the excavation made safe by 2.30pm. Work

Stage 3. The excavation and pipe laying for the sewer main linking the two sections

No night works are being undertaken. The existing street lighting provides adequate illumination
for traffic moving through the work site after working hours.

4.7 Emergency PIanning
Following liaison with FESA representatives it has been determined that the works are classified as
low-impact/low risk accordingly no special provisions are required.

nature, location, date and times as well as contact details for the site supervisor.

Damage to Services
n the event that gas services are damaged, all work shall cease immediately, machinery and
vehicles stopped and the area cleared of personnel as soon as possible. Traffic Controllers
(and other personnel if necessary) shall be deployed immediately to ensure no traffic or other
road users approach the area. The Police Service and relevant supply authority shall be called
immediately. Damage to any other services shall be treated in a similar manner except
machinery may remain operational and access may be maintained where it is safe to do so.
All site personnel shall be briefed on evacuation and control procedures.

4.8.2 PubIic Notification
The Garden Supplies Nursery will be advised in writing of the roadworks. Arrangements will be
made to accommodate vehicular deliveries to and from the property during the progress of the
roadworks.
All public transport users will be advised of changes to bus stops at least 7 days prior to the
commencement of the works. The bus stop will be temporarily relocated to the north of Gnangara
Road. Path Transit will post notices to users of the bus services of the temporary changes in the
week prior to the commencement of the work.

4.8.3 Notification of Other Agencies
n accordance with the CoP all relevant agencies shall be notified using the 'Notification of
Roadworks' form attached at Appendix E. A distribution list is provided on the bottom of the form.
Other agencies shall be notified as required.

nformation obtained from services (by calling the 1100 number) indicates that there is a gas main
in the verge close to the kerb line. All other services are recorded as being within the verge, away
from the work area. There are no services under the proposed work area.

5.0 IMPLEMENTATION

5.1 Hazard Identification, Risk Assessment and ControI

n establishing adequate controls for the hazards identified in Section 4.1, Southern Roadworks Pty
Ltd have used a structured approach via the use of the hierarchy of control as outlined below:

Traffic Services Pty Ltd traffic management practices require that the Supervisor evaluate all traffic
arrangements before they are open to traffic and immediately following the opening to traffic.
Adjustments are to be made as required and recorded in the daily diary, including reasons for the
changes. The Supervisor is also required to evaluate the traffic arrangements where site
conditions change. New hazards that arise throughout the work will be subject to risk assessment
and incorporated onto the Risk Register.

5.2 Traffic ControI Diagrams

The Traffic Control Diagrams outlined in Appendix F have been provided for the following stages
to demonstrate the type of controls that will be implemented throughout the term of the contract.

5.3 Traffic ControI Devices

Traffic control devices shall be erected in accordance with the TCD's (refer Appendix F)

Before work commences, signs and devices at the approaches to the work area shall be erected in
accordance with the installation plan in the following sequence:
a. Advance warning signs. (Erect approach and departure signs on approaches to the work
site)
b. All intermediate advance and positional signs and devices required in advance of the taper
or start of the work area.
c. All delineating devices required to form the taper including the illuminated flashing arrow
sign at the end of the taper where required. (nstall delineation devices and lane closures).
d. Delineation past the work area.
e. All other required warning and regulatory signs.

A vehicle displaying a vehicle mounted warning device shall be used in advance of the signs and
traffic control devices to protect workers setting out the signs or traffic cones associated with the
taper. (Note:- Vehicle mounted warning devices are approved under the Vehicle Standards
Regulations. These devices shall not be used outside the limits of the road works).

At the end of work shift, lay flat the Worker (Symbolic) sign, the temporary hazard marker (T5-4 &
T5-5) signs, remove arrow board and the traffic cones not required for the traffic lane closure. At
start of work shift, prior to commencing work, stand-up all Worker (Symbolic) signs and install aII
traffic control devices as shown in the TCDs.

The signs and traffic control devices are to be removed in the reverse order of installation. A
vehicle displaying a vehicle mounted warning device shall be used in advance of the signs and
traffic control devices to protect workers removing the signs or traffic control devices.

A detailed listing depicting the type and quantity of devices required to implement this TMP is
included in the TCD. Should the use of additional (not shown on the TCD or listing of devices) or
reduced number of devices be required due to unforseen needs, they shall be recorded within the
Daily Diary as a variation to the TMP, following prior approval.

Work will not commence or continue until all signs, devices and barricades are in place and
operational in accordance with the requirements of the TMP. The number, type and location of
signs, devices and barricades shall be to a standard not less than Appendix F of this plan and
AS1742.3 (except where specifically detailed in this TMP with reasons for the variations). Devices
no longer required shall be promptly and completely removed from road user's lines of sight.

5.3.1 Signs
All signs shall be in accordance with AS1742 (and manufactured in accordance with AS1743),
shall be at least size 'B' and shall be Class 1 retro-reflective. The Symbolic Worker sign shall also
be fluorescent. Prior to the installation all signs shall be checked for damage and cleanliness and
repaired, replaced or cleaned as necessary.

Signs and devices shall be erected in accordance with the locations and spacings shown on the
drawings such that:
They are properly displayed and securely mounted;
They are within the driver's line of sight;
They cannot be obscured from view;
They do not obscure other devices from the driver's line of sight;

They do not become a possible hazard to workers or vehicles; and
They do not deflect traffic into an undesirable path.
All existing 70 km/h speed limit signs on the carriageway within the work site shall be retained as
speed re-establishment signs. There are no other traffic or advertising signs in the vicinity which
could cause distractions or confusion, or which restrict sight lines.

5.3.2 Pavement Marking
The off-road works will not have any impact on the existing pavement markings.

5.3.3 VariabIe Message Signs
A Variable Message Sign (VMS) will be erected in Wanneroo Road, 70 metres in advance of the
Whitfords Avenue intersection onIy when Stage 2 works are being undertaken.
The following messages will be displayed on the VMS.

5.3.4 DeIineation
Traffic cones or bollards will be erected in accordance with the drawings as temporary lane
separators between through traffic lanes as shown in the TCDs in Appendix F. Cones and
bollards shall be fitted with suitable white retro-reflective tape placed in accordance with AS
1742.3.
All cones or bollards will be inspected daily and where displaced or missing made good
immediately.
Traffic Cones shall be at least 450mm high, fluorescent red and fitted with Class 1 retro-reflective
tape. Alternatively fluorescent red bollards with Class 1 retro-reflective tape may be used. The
base of the cones and bollards shall be designed to be stable under reasonably expected wind
conditions and air turbulence from passing traffic. The Supervisor will inspect cones at intervals
necessary to ensure any miss-alignment or displacement is identified and corrected prior to this
causing disruption to traffic.

5.3.5 Temporary Speed Zones
The traffic lane closure and part-closure will necessitate the introduction of temporary speed limits
of 60km/h. Close Point delineation will be put in place where traffic is required to travel within 2.5
metres of the excavation39.

5.4 Emergency Arrangements

A Traffic Controller shall assist emergency vehicles travelling through the worksite. Emergency
services shall be notified via FESA (phone 9323 9300) of the proposed works nature, location, date
and times as well as contact details for the site supervisor.

Vehicle breakdown and/or crashes can cause considerable delay and congestion. Traffic Services
Pty Ltd will also render assistance where possible to ensure the impact of crashes and breakdown
on the network is minimised.

5.5 Site Access

Drivers accessing the work site on the east side of Wanneroo Road are required to approach the
site from the north. They should decelerate slowly and signal their intention by indicator to leave
the traffic stream, the vehicle's rotating yellow lamps should be activated once a speed of 20 km/h
has been reached at 50 metres from the point of entry. A spotter will remove traffic cones to
allow work vehicles to enter the work site.

Drivers leaving the work site shall have the vehicle's rotating yellow lamps activated and only enter
the traffic stream when advised by the spotter when it is safe to do so. The rotating yellow lamps
should be turned off once a speed of 40 km/h is reached.

5.6 Communicating TMP Requirements

The requirements of the TMP will be communicated to all personnel entering the site through the
site induction program.

6.0 MONITORING AND MEASUREMENT

6.1 Site Inspections & Record Keeping
The Project Manager will ensure that the Traffic Management Plan is implemented and evaluated
for effectiveness. The Supervisor shall inspect and monitor traffic movements around the site in
conjunction with the personnel who have erected the control measures. The outcomes of the
inspection will be diarised for the information of the Project Manager.
nspections shall be undertaken as required and at a minimum on the following occasions:
Before the start of work activities on site; Closing down at the end of the shift period;
and
During the hours of work; After hours.

A daily record of the inspections should be kept indicating:
When traffic controls where erected;
When changes to controls occurred and why the changes were undertaken;
Any significant incidents or observations associated with the traffic controls and their impacts
on road users or adjacent properties.

Where significant changes to the work or traffic environment or adverse impacts are observed, the
controls should be reviewed as a matter of urgency. Daily nspection Sheets shall be completed
by the person undertaking the inspections and reviewed by the Supervisor. All variations to the
TMP/TCD, non-conformances, incidents and accidents shall be recorded. Copies of the completed
report shall be forwarded to the Project Manager. A suggested Daily nspection Report Form is at
Appendix C. One sheet per inspection should be used, with the relevant section to be filled in.

6.2 TMP Auditing

One compliance audit (using the 'Compliance Audit Checklist for Traffic Management for Works on
Roads' available on the Main Roads website) shall be conducted following setting-up of the traffic
management and prior to commencement of the works. Audit findings, recommendations and
actions taken shall be documented and copies forwarded to the Project Manager.

6.3 PubIic Feedback

Traffic Services Pty Ltd will implement a procedure that ensures comments and complaints
received from the public are registered. The Supervisor shall be responsible for the monitoring of
the Register on a daily basis.

7.0 MANAGEMENT REVIEW

7.1 TMP Review and Improvement

As this project is of a short-term nature, a review of the effectiveness of the TMP will be
undertaken by the Project Manager as part of the close-out procedure.

7.2 Variations to Standards and PIans

The Stage 2 works involving the excavation and laying of the sewer main requires the closure of
the traffic lane adjacent to the kerbline. This in turn requires the closure of the kerbside slow
straight-through traffic lane at the approach to the signals. The traffic volume count exceeds that
prescribed in Table 4.8 of AS 1742.3-2002. A variation to this requirement has been sought and is
contained in Appendix G.

On-site variations, if required, shall generally only be made following approval by the
Superintendent's Representative and recorded in the daily diary. n emergency situations, on-site
variations shall be made and recorded in the daily diary, and the Project Manager notified as soon
as practicable.

Traffic Risk CIassification


1. In order to clearly understand the risks associated with this Contract and
then outline the manner in which identiIied risks will be managed, the Contractor shall
undertake an assessment oI all signiIicant Ioreseeable risks associated with the Contract
and determined the treatment measures that, so Iar as practicable, minimise the risk.

2. The identiIication and assessment process must be undertaken in
accordance with AS/NZS 4360 and the likelihood and consequences rated beIore the
application oI risk treatments (Primary Risk) and aIter (residual risk) the determined
controls utilizing Table 202B.1, Table 202B.2 and Table 202B.3 oI this Annexure 202B.

3. The Contractor shall, so Iar as practicable, control or reduce identiIied
risks in accordance with the hierarchy oI control as deIined by AS/NZS4801. Treatment
measures shall be authorised and managed by the Contractor in accordance with Table
202B.4 Management Approach for Residual Risk Rating.
Risk Control
and Reduction
4. The Superintendent may direct the Contractor as to the Primary Risk
Rating and the Residual Risk Rating to apply to any risk. The Contractor shall reassess,
authorise and manage its risk control measures in accordance with the level oI risk
directed by the Superintendent.

5. A Residual Risk Rating oI Extreme is not permissible under the Contract.
6. The Contractors shall use the OSH risk classiIication in accordance with
SpeciIication 203 OCCUPATIONAL SAFETY AND HEALTH when addressing
saIety hazards oI the general public and road users moving through the Site.
