Checklist to convert a Student Registration or CLI Registration to pursue admission to The Florida Bar (Registration previously filed) This

form is designed to assist you in gathering information, completing the Supplement to Registrant Bar Application, and submitting the appropriate forms and information required to convert your student registration or CLI registration in pursuit of admission to The Florida Bar. Click the various links to access information or printer-friendly forms. A Supplement to Registrant Bar Application submitted with accurate and complete item responses and all required supplemental documents can be processed more expeditiously. Processing will not be initiated on defective applications and an additional defective fee may be charged. You must also submit the Re-Examination/Conversion Application to take the General Bar Examination in order to indicate which examination you intend to take. Timely filing of this application is necessary to avoid late filing fees. STEP 1 - PRINT AND USE THIS CHECKLIST ____1. Print this Checklist. ____2. Check each item when completed to ensure that your submission includes all required supplemental documents and information. STEP 2 - GATHER INFORMATIONAL MATERIALS It is suggested that you gather this information and have it available when you complete the Supplement to Florida Bar Application. ____1. A copy of the Registrant Bar Application and all amendments previously filed by you. If you did not retain a copy, you may request a copy by writing to the address below and submitting the $50.00 copying fee. However you do not submit a copy of your application with your supplement. ____2. Read and review each item of the Registrant Application and amendments you previously filed as a student to assure that each response is correct and current. Update any item that has changed since your initial filing that has not been reported in an amendment. ____3. Five personal references who have known you well within the past 5 years, including names and current mailing addresses. ____4. Information to update your residence addresses. ____5. Information to update your employment, including employers' names, addresses, dates employed, position, type of business, supervisor, and reason for leaving. Include self-employment or association with any occupation, business, enterprise, or profession, either part-time or full-time; employment as a law clerk; positions in clinics, internships, externships, or other similar non-paid positions. ____6. Information regarding any delinquent credit, defaulted credit, unfiled tax returns, and all student loans. ____7. Information to update all arrests, charges or accusations (including traffic violations) reporting dates, law enforcement agency, explanation of event and final disposition. If your arrest records are sealed, you must petition the appropriate court to unseal those records. ____8. Information about all bar admission or registration applications filed in any other jurisdiction. ____9. Fingerprints (unless previously submitted as a Florida Bar registrant or CLI registrant) All applicants for admission to The Florida Bar must have their fingerprints scanned for electronic submission to the Florida Department of Law Enforcement and to the FBI by MorphoTrust USA. Instructions for this step are in the answer to How do I submit the required fingerprints? in our FAQ. Confirmation of your compliance with this step must be received by the board before processing of your application can begin. If your fingerprints have already been taken by MorphoTrust USA for the purpose of applying to The Florida Bar, you are not required to be re-fingerprinted. ____10. Retrieve or obtain your NCBE number. You will be required to report your NCBE number on the Examination Application under Step 4 below. Go to www.ncbex.org/ncbe-number to either retrieve your NCBE number or request an NCBE Number. Your unique NCBE Number will be used for identification purposes when you take the Multistate Bar Examination and the Multistate Professional Responsibility Examination and may be used as an identifier for other bar-admissionrelated purposes. It is recommended that you copy and paste your number into the "NCBE Number" field on your Examination Application. Therefore, you should have that window open when you are completing the application. STEP 3 - COMPLETE THE SUPPLEMENT TO FLORIDA BAR APPLICATION

When you have gathered the information above: ____1. Complete and print the Supplement to Florida Bar Application (PDF - 90k). Record each changed item on the form. This form is required to convert the student or CLI registration previously filed in pursuit of admission to The Florida Bar. ____2. Make a copy of the Supplement to Florida Bar Application for your records. STEP 4 - COMPLETE THE RE-EXAMINATION/CONVERSION APPLICATION ONLINE When you have gathered the information above, you are ready to complete the Re-Examination/Conversion Application. A high speed Internet connection is recommended. ____1. Examination Application Click the link above to access the on-line application in a new browser window. Answer the questions in order as some questions are based upon your responses to earlier questions. ____2. Click the Browse button to upload your passport-style photo (see Photo Requirements And Upload Instructions). ____3. When you have responded to all items and have selected your photo for upload, click the button, "Finalize My Application". The program will gather your responses into a document and display it on the screen. ____4. Print application Print 2 copies of your Re-Examination/Conversion Application. You will submit 1 copy to the board and keep 1 copy for your records. Once you close your browser, the Re-Examination/Conversion Application will no longer be available to print. If you fail to print the application, you will have to re-enter all data from the beginning. ____5. Complete the handwriting sample on the last page of the application. STEP 5 - CHECKLIST TO SUBMIT A COMPLETE SUPPLEMENT TO BAR APPLICATION All the following documents must be provided and documents 1-4 must be properly notarized. The notary should require identification and place you under oath prior to requiring you to sign each document. Confirm that the notarization is properly executed (including that the notary properly indicates how you were identified, signs, dates and affixes his or her stamp). Incorrect notarization will cause your application to be returned and delay initiation of the background investigation and may result in your being charged an examination late filing fee or for your late filing fee to be higher. ____1. Supplement to Florida Bar Application Completed and properly notarized. ____2. Re-Examination Application/Conversion Application Completed and properly notarized. ____3. Authorization and Release (PDF - 63k) Three (3) originals, completed and properly notarized. ____4. Affidavit of Compliance (PDF - 62k) Completed and properly notarized. ____5. Student Registration Converter Fee Computed using the Application Fee Worksheet (PDF - 54k). Clip the check to the front page of the Supplement to Florida Bar Application. All of the following as applicable, if not previously filed in conjunction with your Florida Bar Application filed as a student registration or CLI registration.

____6. Fingerprints (unless previously submitted to MorphoTrust USA as a Florida Bar registrant or CLI registrant) All applicants for admission to The Florida Bar must have their fingerprints scanned for electronic submission to the Florida Department of Law Enforcement and to the FBI by MorphoTrust USA. Proof of citizenship If you are a citizen of the United States, submit with your Bar Application a certified copy of your birth certificate, or provide a photocopy of your certificate of naturalization, or certificate of citizenship for submission to the United States Citizenship and Immigration Services for verification of authenticity. If you are not a citizen of the United States, provide a photocopy of the immigration document that documents your status for submission to the United States Citizenship and Immigration Services for verification of authenticity. Copies of letters of request for law school and post law school transcripts ____9. Certificate of Dean (PDF - 171k) Complete the top section and submit the Certificate of Dean to your law school dean after your graduation. If attending a Florida law school, do not provide the form to your school. ____10. Documentation of military service (DD-214, Report of Separation, or equivalent) ____11. Copies of litigation documents ____12. A copy of the judge's signed order to unseal arrest records ____13. Copies of letters to doctors (as required by items 25 and 26) ____14.
