.& After careful consideration- t!e APCPDCL !ereby adopts t!e guidelines issued by AP#RA$SC% vide its *emo $o& 2D *ec!l AS(*8P) P%(*ed) *R *5 +,9 .,''- dt&.+-,'.,'. for t!e benefit of t!e employees pensioners and t!eir dependents "amily Pensioners of APCPDCL as follo<s:

'& #!e claim is to be preferred <it!in (+) mont!s from t!e date of disc!arge of t!e patient from !ospital& .& #!e application format s!ould be in page $os& only as prescribed by AP#RA$SC% along <it! declaration certificate of dependency&

+& #!e application s!ould be signed by t!e controlling officer for<arding officers <it! date and stamp <it!out t!e dated bills by t!e controlling officer- t!e same s!all not be admitted& 5& All t!e columns in t!e c!ec= list of application !as to be filled up by t!e employee pensioner& 9& 2ssentiality certificate >A? for out patient treatment and >@? for inpatient treatment to be invariably furnis!ed and duly filled in and signed by t!e medical attendant Doctor <!o treated t!e patient <it! date and seal of t!e Doctor& 3& #!e cas! receipts are to be counter signed by t!e Doctor toget!er <it! name 8 seal of office <!o treated t!e patient- Lab test- P!armacy s!ould be accompanied <it! prescription slips issued by Doctor& 1& #!e dated cas! receipts are to be enclosed along<it! application& 4& #!e dated cas! receipts and t!e amount claimed s!ould be tallied for total claims& /& #!e bills s!ould be claimed only under @ill $o& on eac! bill- <it!out bill $o& t!e same s!all be reAected& Co$ d.P.#

If employee of AP#RA$SC% spouse is <or=ing in AP#RA$SC% DISC%*S Central 6overnment State 6overnment Corporation Local @odies and Private organiGation s!ould submit a declaration stating t!at t!ey s!all avail medical facility from any one side only and also a no-obAection certificate from t!e department <!ere spouse is <or=ing- s!all be submitted&

.,& Incase any fracture cases- bill s!all be admitted only <it! *LC $o& and on "IR in case of R#A Accidents "racture+&

.'& All t!e employees pensioners family pensioners are directed to affiE t!eir signature attendant on completion of inpatient treatment obtained on credit card and also see= a copy of t!e bills& ..& If t!e application does not confirm to any one of t!e above points- t!e application s!all not be processed for admission of claim& .+& #!e 7ead of t!e Department s!all affiE signature <it! date of receipt of medical claims- any bill beyond + mont!s from date of disc!arge s!all not be entertained eEcept in case of deat!- 3 mont!s period from t!e date s!all only be allo<ed for claim&
