Credit Worthiness Report from the Banker of the unit (Annexure-F of

Application Form).

5.

General Power of Attorney on NJSP of Rs.50/- duly notarised Or Board

Resolution on units letter-head in favour of the person signing the application as per
Annexure-A(1)/A(2) of the Application Form.

6.

Notarised copy of Ownership document/Lease Deed/Rent Deed for Factory

and office premises.

7.

Proof of Ownership of Plant & Machinery and Raw Material along with original
value of & date of purchase of machinery installed (By affidavit as per Annexure B of
Application Form) on NJSP of Rs.20/- duly notarised.

8.

Notarised Undertaking as per following Format:

I, Mr. ______________ Prop./Partner/Director of M/s. __________________, at
_____________________, do hereby undertake that the applicant unit (SSI) does not link
with Large Industry registered with DGS&D/ISI and/or any other manufacturing unit On
units letter head duly notarised.

Statement showing the Result of Operation and Financial Position of last 3

(THREE) years (Annexure C of Application Form), duly certified by Chartered Accountant
and the authorized signatory.

10.

Declaration by the Applicant as per Annexure-D of the Application Form on

NJSP of Rs.20/- duly notarised.

Declaration by the Applicant as per Annexure-E of the Application Form on

units letter-head duly notarised.
