VIII. BILL OF MATERIALS AND COST ESTIMATE DULY

IX. XEROX COPY OF UPDATED PROFESSIONAL TAX
RECEIPT AND PROFESSIONAL
REGULATIONS COMMISSION (PRC) IDENTIFICATION
OF ALL SIGNATORY
ENGINEERS/ARCHITECTS TO THE APPLICATION
FORMS AND PLANS.

X. DULY ACCOMPLISHED APPLICATION FORMS FOR
BUILDING PERMIT,
SANITARY/PLUMBING PERMIT, ELECTRICAL PERMIT,
MECHANICAL PERMIT.

XI. ZONING COMPLIANCE CERTIFICATE.

XII. ENVIRONMENTAL COMPLIANCE CERTIFICATE FOR
BUILDING AS FOLLOWS:
