Alok Hospital and Research Centre (Provider) an institution located in 21-Tagore Town Colony Bhojubeer
Varanasi, having their registered office at 21-Tagore Town Colony Bhojubeer Varanasi (here in after referred
to as Provider, which expression shall, unless repugnant to the context or meaning thereof, be deemed to
mean and include it's successors and permitted assigns) as party of the FIRST PART
AND
Royal Sundaram Alliance Insurance Company Limited, a Company registered under the provisions of the
Companies Act, 1956 and having its registered office at 21, Patulous Road, Chennai 600 014 and Corporate
Office at 45 & 46, Whites Road, Chennai 600 014 (hereinafter referred to as Insurer which expression shall,
unless repugnant to the context or meaning thereof, be deemed to mean and include it's successors, affiliate
and assigns) as party of the SECOND PART.
The (Provider) and Insurer are individually referred to as a "Party or party" and collectively as "Parties or
parties")
WHEREAS
1.

Provider means a hospital or nursing home or day care center (herein after referred as Provider)
duly recognized and authorized by appropriate authorities to impart heath care services to the public
at large.

2.

Insurer is registered with Insurance Regulatory and Development Authority to conduct non life
insurance business including health insurance business.
