UTP 3 - CASE STUDY

Case study 1 (Due date: 24/10/2014)


Choose a location in a polytechnic Ungku Omar and identify the
hazard, risk assessment and risk control using a HIRARC method.
Location:
As Mechanical Engineering (ME) department students you are
encouraged to ME department facilities. (e.g. automotive workshop,
air-conditioning, machining, welding, sheet metal, laboratories, etc.)
Submission:
The report content of this task should be inclusive of
relevant/necessary diagrams/illustrations/figures and appropriate
laminated A4 size signage (minimum 5)[loose sheet]

Case study 2 (Due date: 14/11/2014)


Diana is a receptionist in a large trading company. Much of her work
involves using a telephone to take messages and redirect calls to
appropriate departments. Diana regularly uses a computer (display
screen equipment or DSE) to make appointments, record messages
and respond to e-mails. After working at the front desk for eight
months, Diana found she was leaving work with an aching shoulder
and neck, and with sore eyes and a headache. Diana asked her
manager for a risk assessment to identify possible problems with her
working arrangements and to determine where improvements could
be made.
Assume you as a health and safety representative and respond to the
problems by suggesting modifications step to take.
Case Study 3 (Due date: 5/12/2014)
Farhan works on an engine assembly line. In his job he has to use a
handheld impact wrench to fit a temporary adapter to an engine. The
assembly line makes up to 2400 engines a day and it takes
approximately 3 seconds to tighten each adapter.
As well as the risk from using a vibrating tool, Farhan often had to
adopt poor postures to reach some parts of the engine. He had to

repeatedly stretch out his arm and constrain his posture while
tightening the adapter. After some time on the job, Farhan found he
was leaving work with shoulder and neck pain. One tea break,
Farhans line manager saw him rubbing his neck and shoulder and
recognized the pain could be due to the type of work Farhan was
doing. The line manager told the company health and safety
representative about what she had seen.
Assume you as a health and safety representative and respond to the
problems by suggesting modifications step to take.

Instructions:
The focus of this task is to assess the performance on student in
demonstrating the theory and understanding of the self-regulatory
concepts and provisions under the Occupational Safety & Health Act
(OSHA). The content of this task should be inclusive of
relevant/necessary diagrams/illustrations/figures and laminated A4
size signage (minimum 5)

Groups: Group consists of 5 members each, with the exception of the


female students as one group. Each group should appoint a leader.

Marks: assessment will be based on team works,


graphical/media, and format of the case study reports.
