1. Planning is: the process of designing nursing activities required
to prevent, reduce, or eliminate a clients health problems.
Planning is the 3rd step in the nursing process.
The end product in planning is to prevent, reduce, or eliminate a clients health
problems
Planning involves the nurse, the client, support persons, and
other caregivers.
2. 3 types of planning:
1. Initial Planning
Planning should be initiated as soon as possible after the initial assessment.
2. Ongoing Planning
Is done by all nurses who work with the client. As nurses obtain new information and
evaluate the client's responses to care, they can individualize the initial care plan
further.
3. Discharge Planning
The process of anticipating and planning for needs after discharge. Discharge
planning starts at the moment the client is admitted into the unit.
3. Differentiate between protocols, policies, procedures, and standing orders:
Protocols
are developed to indicate the action commonly required for a
particular group of clients
Policies and Procedures
are developed to govern the handling of frequently ocurring
situations.
Standing order
is a written document about policies, rules, regulations, or orders
regarding client care. Standing orders give nurses authority to
carry out specific actions under certain circumstances, often
when a priary care provider is not immediately available.
4. What are the 10 guidelines for writing nursing care plans?
1. Date and sign the plan. 2. Use category headings.
3. Use standardized/approved medical or English symbols
and key words rather than complete sentences to communicate
your ideas unless the agency policy dictates otherwise
4. Be specific
5. Refer to procedure books or other sources of information
rather than including all the steps on a written plan.
6. Tailor the plan to the unique characteristics of the
client by ensuring that the clients choices, such as preferences

about the times of care and the methods used,


are included
7. Ensure that the nursing plan incorporates preventive and
health maintenance aspects as well as restorative ones.

client
9. Include collaborative and coordination activities in the
plan
10. Include plans for the clients discharge and home care
needs.
Each step is important because in order to get the end result you must do each step.
Omitting a step would change or alter the outcome.
5. What is meant by the activity of priority setting in the planning process? What factors
need to be considered when assigning priorities? Nursing diagnoses are assigned high,
medium, and low priorities in consultation with the client, if health permits. Prioities are
set according to the current state of health of the client. The current situation is
addressed by prioritizing the most critical issue down to the other iuuses currently going
on
6. What is the purpose of desired goals and or outcomes? Is to observe the patient and
see if they have been able to achieve the desired outcomes or goals. The goal is to to
prevent, reduce, or eliminate a clients health problems.
