General Objectives for the Incident:


1. Ensure the safety of all deployed personnel
2. Ensure the proper assessment of the working area
3. Ensure the availability of appropriate PPE and equipment
4. Ensure sufficient and adequate instruction and information according to the proper procedures and
regulations.
5. Ensure a proper supervision and coordination to the worksite
6. Ensure a proper incident initial reporting within 6 hours to QP Focal point and followed by full
report within 24 hours.

Lesson Learned:
1. Review the work method statement, risk assessment and communicate to all supervision with
acknowledge signatures.
2. Planned task observation to be frequently carried out to identify incorrect sequence of work to
ensure safety at all times.
3. Initiate day to day safety hints and tips and communicate it to all supervisors and workers with
instructions.
4. Toolbox talks to be schedule with emphasis on the responsibility for an individual to secure
arrangement of equipment shifting
5. Conduct training regarding all aspects of the safety in shifting, proper manual handling including
behavior and perception.
Preventing the Risk of Working at Heights
Planning is very important part of equipment shifting. There are three tools that enable, we do that:
1. Work plan review
Work plan review must be undertaken before activity will start to examine what is required at a
design stage to reduce and eliminate risk as far as reasonable and practicable.
All Management staff (project manager, project engineer, HSE manager, HSE coordinator,
supervisor should involve in the review.
2. Manual Handling plan
Written manual handling in the HSE plan must be prepared for all work to determine the hazards
and controls involved in equipment shifting.
It should be included in all workers induction and training
Audit should be done regularly to ensure compliance
3. Risk assessment
Risk assessment must be completed prior to apply a work permit to identify the hazards, the risk
and the control measure required for a project or task.
Ensure that this risk assessment discuss to all workers before do manual handling works.
Ensure that these risk assessments are approved by the project manager, HSE representative and
the focal point.
Responsibility and Accountability
1. Project Manager
It is natural that the administration of Safety, Health and Environment programs are part of the
responsibility of the Project Manager who is over all in charge of construction activities at site as
delegated by Country Manager Projects Group.

It is recognized that a strong support and active participation by the Project Manager is essential.
The Project Manager:
1.1.

Assures that the subordinates have thorough understanding of their HSE responsibility.

1.2.

Establishes controls to assure uniform performance in compliance with safety and health
program elements.

1.3.

Provides the HSE Section with necessary information required to evaluate the effectiveness
of Safety, Health & Environment system.

1.4.

Motivates the organization to result in the maintenance of high quality of HSE on all
construction / work sites.

2. HSE Manager
Act as technical consultant/advisor on all related safety and health standards of Client.
The HSE Manager:
2.1.
Prepare IMCOs HSE Plan, immediately before the start of a construction project. He will
monitor implementation throughout the duration of project construction.
2.2.
Improve the IMCOs safety performance through Hazard Identification Plan,
Comprehensive Safety Analysis Technique, Safety Audits, Major Accident Investigation and
Analysis, Safety Review of the Construction Project, Safety Award Program.
2.3.
Effectively lead Senior HSE Coordinator, /Safety Officers /Assistants, and manage their
HSE activities to attain the companys Goal of No one gets hurt and No property gets
damaged in all projects under his jurisdiction.
2.4.
Prepare relevant safety instructions, code of safe practices, safety-training manuals and
programs and initiate implementation.
2.5.
Initiate Job Hazard Analysis and Process Hazard Analysis and prepare information
materials for IMCO employee awareness through Safety messages, Safety Meetings and
Trainings.
2.6.
Prepare and conduct safety training on specific topics related to the projects safe
performance. He will be available for continuous safety training on site and at the main
office training room.
2.7.
Review and evaluate all site safety reports and prepare studies to improve safety activities
to attain IMCOS Goal NO ONE GETS HURT AND NO PROPERTY GETS DAMAGED
2.8.
Conduct monthly Quality/HSE Performance Audit and prepare report/studies to improve or
maintain Quality/HSE performance for clients satisfaction.
2.9.
Review monthly reports and maintain filing system of all relevant safety records. The file
will be made available to the Clients HSE Department, if required.
3. HSE Coordinator
Responsible for the implementation and coordination of the projects Safety Plan.

Conduct weekly Safety Meetings and prepare/maintain records of the meeting.


Attends Construction Progress Meeting where Safety is an item on the agenda.
Conduct weekly safety inspection and motivate work force to work safely and achieve the
Goal of No one gets hurt and No one gets damaged. Prepare report of infractions, and
follow-up of actions taken.
Assists Site Engineer / Supervisors / Foremen in the investigation of accidents /near
misses and maintain accident log recording of all pertinent accident information.
Communicate safety statistics, accident reports, etc. to Client representatives.
Consult with HSE Manager and co-ordinate training activities.
Review and evaluate safety reports.
Maintain and keep record of rigging and lifting equipment on site.
Monitor heavy lifts for proper equipment usage.
Review and evaluate all site safety reports and prepare studies to improve safety activities
to attain GOAL OF NO ONE GETS HURT AND NO PROPERTY GETS DAMAGED.
Prepare required monthly reports and maintain filing system of all relevant safety records.
The file will be made available to the Clients Project Management and HSE Department, if
required.
Communicate safety statistics, accident reports, etc.

4. Safety Officers
Safety Officer is responsible for all administration of HSE program. He will report directly to the
HSE Coordinator.
In carrying out his responsibility, the HSE Officer:

Implements established policy, procedures and work practices to promote the company
project safety and health program and administer assigned functions to aid in these overall
responsibilities.
Monitors compliance with mandatory HSE laws, standards or codes.
Conducts inspection of job sites, documents and reports any unsafe acts & unsafe
conditions to the responsible Engineer / Supervisor for immediate corrective action.
Tests the work environment and documents the results in order to eliminate or control
hazard which could contribute to or result in an occupational illness.
Arranges and conducts regular HSE meeting for field Supervisors. Ensures that a foreman
or representative from each unit is present at these meetings.
Attends all weekly progress and/or schedules meeting to provide input regarding safety for
current and future project activities.
Ensures that unit supervisor conducts weekly tool box meetings with all personnel in
attendance provides appropriate topic or discussion and initiates follow-up response to
HSE concerns addressed during these meetings.
Investigates all incidents and accidents that could involve potential liability, maintains
adequate records of relevant data and complies the required reports on experience.
If assigned, monitors compliance with established pollution control and environmental
protection standards and regulations.
Arranges for securing safety permits based on site conditions.

Performs air monitoring for harmful or explosive substance in the work environment.
Ensure that all required notices, posters and other related literature is properly displayed
for employees information. Organizes necessary HSE propaganda program for promoting
safety and health program for HSE awareness.
Recommends, selects and ensures that Safety, First aid, fire protection equipment are of
relevant standards and also ensures that such material is always at hand.
Assists Project Supervisor in the inspection of tools, equipment and facilities.
Provides training to site personnel with regards to specific safe work practices / procedures,
use and care of personnel protective equipment, and handling hazardous material /
equipments.
Identifies the safe areas near by the work place, locations of emergency facilities such as
fire service, water, first aid clinic, etc.
Ensures that all work forces are aware of such in 4.16.
The Officer has the full authority to stop any work activity for any safety reasons.

5. Project Engineer
The Project Engineer is responsible for all activities on his job including the prime responsibility for
HSE.
In carrying out his duty, he will make certain that:

HSE rules, regulations and standards noted in this manual, and the laws and regulations of
various statutory bodies are complied with and enforced.
In the absence of dedicated HSE officer, it is his responsibility to ensure safe working
conditions to his subordinates.
Employees are given adequate training and instruction.
Safety inspections of job and equipment as required are carried out.
Accidents receive prompt investigation and reporting and that the necessary corrective
action taken.
Weekly safety meetings are instituted.
Work in close cooperation with the HSE officer to eliminate and correct all practices and
conditions that are deemed to be unsafe.
Delegate safe work procedure that will ensure safe operation.
Ensure that assigned tools and equipments are in sound condition.
Inspect construction area to ensure freedom from unsafe conditions.
Observe activities of subordinates to know they are following correct procedure.
Accompany HSE Representative on site audits.
The Project Engineer / Supervisor have the full authority to stop any work activity for any
safety reasons.

6. Worksite Supervisor
Each Worksite Supervisor is responsible for workman, under his supervision with duties that
include the following:

Maintain safe working conditions in all areas under his supervision.


Enforce the use of protective equipment, such as hard hats, safety boots, protective
glasses, goggles, rubber gloves, etc., required by corresponding jobs.
Report all accidents to the HSE supervisor and conduct investigations and interviews with
those involved and witnesses.
Conduct weekly tool box safety meetings with craft person.
Inspect work areas for unsafe conditions and unsafe work practices, which are likely to
cause personal injury or property damage and initiate corrective action immediately.
Enforce all sites and units the job HSE rules.
Take appropriate measures to eliminate all recognized hazards.
The Worksite Supervisor has the full authority to stop any work activity for any safety
reasons.
