1.0: INTRODUCTION

A quality Management Plan (QMP) will be developed and implemented to meet


the Quality requirements of Macksams Limited and in accordance with the contract
requirements. This plan also take cognizance of international quality standards and
Ghana statutory requirements such factory, offices and shops Act,1979,(Act 328),
National building regulations,1996-L.I.1630.
MACKSAMS QUALITY POLICY STATEMENT
Macksams Ltd is committed to the delivery of quality work or services to its
clients. In fulfilling this commitment, we provide a quality management system
that comprises quality assurance, control and continuous improvement.
We build Quality into all our construction process hence our belief that Quality is
everyones responsibility.
The principles applied by Macksams Ltd in its Quality Policy are:
All activities shall be assembled into a quality assurance plan.
Constant reference to relevant standards and specifications shall be made.
Activity schedule shall be defined that will ensure collection of data and
comparison of results to the plan..
Resource requirements shall be established and provisions made as and
when needed.
Responsibility levels shall be assigned to specific persons. However, all
team members shall be in-charged.
Compliance with relevant laws and regulations of host (project) country.
Respect of provisions in conditions of contract and adherence to technical
specifications and drawings.
Application of committed quality management system.
Providing close supervision of, and cooperation and coordination with,
subcontractors and suppliers.

Improving and enhancing the level of technical knowledge and skills of our
employees by promoting knowledge acquisition, efficiency, creativity and
adaptation through team-work and cooperation.

Ensuring the efficiency of our operations, through;


o increasing work productivity,
o reducing costs,
Maintaining relationships with teams of professionals as Associated
Consultants for expert advice.
Establishing long-term relationships with subcontractors/specialized teams
that adopt Rules and Principles of Macksams and work within a win &
win principle.
Use of the principles of Total Quality Management(TQM) to develop and
maintain the processes required to produce a level of construction consistent
with international quality standards acceptable to our clients.
This philosophy imbibed in Macksams Limited quality statement will give
assurance to our clients of higher expectation on completed projects in accordance
with specification and contracts.
This quality philosophy will be achieved through top management commitment to
the quality policy statement.

Quality Policy as a way of life.

2.0: Quality Planning


This section describes specification identification and understanding of the
specification by the project team. It also describes how the project team will be
sensitize on Macksams commitment to quality.
The project quality will be planned by the project manager with the support of the
other team members and the actions or activities to be performed includes:
Client-project manager dialogue: Our project manager will engage in a
discussion with the clients technical personnel to fully understand the
requirements and specification of the project. The resolution of all
differences in designs, requirements and specifications will enhance the
implementation and smooth the course of work.
Project manager-project team dialogue: The project manager will organized
Quality planning promotes prevention rather than inspection. It ensures that quality
is planned into the project to achieved project quality.
3.0: Quality Assurance and Control
This section describes the processes or activities that will be followed as well as
monitoring performance and evaluating the outcomes of the project deliverables to ensure
that the project will meet the requirements and specifications of the contract.
To ensure that Quality is built into the production or construction process, the
following steps or activities will be taken:
3.1:Assurance
Specification Compliance

Macksams will ensure that all materials employed for the permanent works
comply with the relevant specification and meet the appropriate standards.
Materials Approvals
Samples of such materials will be submitted to the Clients Representative
for approval together with any relevant specification details, prior to
deployment onto the works. Where necessary we will submit test certificates
of sample materials together with the sample to the project manager of
architect for approval.
Material Testing(Before incorporation)
Tests conducted are as per the Specifications. Upon appointment Macksams
will liaise with the Clients Representative to agree on appropriate testing
regime and authority.
Suppliers and sub-contractors
As described in the quality planning, suppliers and subcontractors to the
project will be sensitized on the quality policy of Macksams Ltd. They will
be given the specifications of the materials they will be supplying or
incorporating into their work. Also Macksams will demand test certificate
from suppliers such as mild steel bars, chippings etc.
Method Statement
A method statement will be written for each activity to ensure that the
require deliverable is achieved. The statement will consist of but not limited
to the method of construction, gang size, health and safety procedures,
duration etc. This will also become the bases of our works program.
3.2 Quality Control
Monitoring specific project results or outcome will be carried out to
determine whether they comply with the relevant quality standards and
identifying ways to eliminate causes of deficiencies. We will therefore
embark on field test.
The purpose of conducting the Quality control to the project outcomes
includes:
1. Results may confirm that all is well. The team will then know that
performance is going according to plan. If there is negative variance then the
team will then take corrective action in good time.

2. Results or outcomes will provide feedback to the quality assurance


process.

Field Test.
Macksams will conduct field-testing to verify that control measures are
adequate to provide a product that conforms to the contract requirements and
specifications.
Field testing will be under the organization of the Quality control officer and
materials engineer and will be conducted as follows:
Arrange for field testing in accordance with applicable testing
parameters(Contract specification, National building regulations,1996L.I.1630 etc)
Verify that testing facilities and testing equipment are available and comply
with testing standards and ensure that testing facilities are approved by the
employer, Ghana Standards Authority and validated
Check test equipment calibration data against certified standards
Verify that all tests are documented and submitted as part of quality control
system reporting
Review all test documentation before submittal

All testing results will be recorded on a daily quality control report. Any
concerns or deviations from the required material specifications and actions
taken to correct the problems will be noted on the report. Information recorded
on the daily quality control report will include:

Types and numbers of tests performed


Results of testing
Nature of defects or cause for rejection
Proposed corrective action(s)
Corrective action taken and date
Deficiencies

3.3: Roles and Responsibilities


To avoid conflicts of interest and also promote the effective implementation of this
Quality Management Plan, there is the need of defining the roles and responsibilities of

the Quality committee. The functional responsibilities of key personnel are summarized
as follows,
Top Management
op Management, consist of the company chair, managing director, director of projects etc will be
responsible for the project success. They will provide resources and leadership to the site
management to ensure the project is executed to the requirements of the contract.
Site

Management

A Quality Management Committee, consisting of the Project Manager, the material


and Quality Control Officer, Site Agent, Construction Engineer, Architect and the
civil Engineer will be responsible for the effective implementation and operation of
the Quality Management System.
All Staff are to follow the rules set by this committee and heed notifications and/or
instructions on all matters of Quality issued by the Quality Management Committee
with the same respect and importance as if being issued by the company itself.
Accordingly, representation of the Quality Management Committee shall be
empowered with the following authorities:
- To report directly to the Company Top Management Committee,
- To issue stop work instructions on the spot to any project staff if so required in
the interest of providing Quality work,
Quality Personnel
Quality Officers will be engaged in the day-to-day activities regarding Quality.
Duties, Responsibilities, Authority
General

Quality is everyones concern. It is a matter that the Company holds in high


esteem. Consequently everyone will be involved in it and held responsible for
certain areas of its implementation.

The following people will play a key role in the implementation and successful
execution of the Project Quality Plan.
The Project Manager (Contractor)
The Project Manager will be the overall responsible person. He delegates certain
authorities and responsibilities to people below him. He reports on all matters
relating to the Quality Policy and Quality Management System. He is responsible
to appoint a Quality Officer and provide him with sufficient authority and
resources to carry out his responsibility to operate and maintain the Quality
Management System.
The Project Manager chairs the Quality Management System Review Meetings.
He appoints members to the Quality Management Committee from a cross-section
of Section Managers and supervisory staff.

Quality Officer
The Quality Officer (QO) will report directly to the project manager. His function
will be to ensure compliance with the contract plans and specifications.
His duties/responsibilities include the following:
On site implementation of the Quality Management System
Monitoring and reporting to the Project Manager/Site Agent on the
performance of the Project Quality Management System
Conducting Internal Quality Audits quarterly
Maintaining as-built drawings

Providing coordination of required quality control testing, reviewing results,


and submitting.
Tracking construction deficiencies and ensuring corrective action in good
time
Line of Communication
Formal Quality discussions will take place at Quality Management Committee
Meetings, together with frequent site Quality awareness talks, which are minute
and kept record.
These meetings provide a forum to address and/or resolve particular issues of
Quality.
Other than in those meeting, any parson working on site can bring to the attention
of a Quality Officer, who will raise the issue with the project manager. If it cannot
be resolved at this level, the issue has to go the Clients site supervisor.
As a means of communication from the Quality Committee to the workers, a Site
Notice Board will be erected which carry as a minimum the appropriate
Regulations, Quality Policy,
In addition, all site personnel will be issued a copy of the Quality Plan and
Procedure Manual.
4.0: Quality Metrics

5.0: Quality Management System


This section describes how Macksams Ltd will achieve quality consistency through
system Audit and Review.
Quality audit and review activities will be done in three phases namely the
preparatory phase, initial phase, and the follow-up phase.

Preparatory Phase
This is the phase prior to beginning work on each work activity and the activities
that will be performed include:
o A review of the applicable requirements and specifications.
o A check to ensure that all materials and /or equipment have been tested,
submitted, and approved.
o A check to assure that provisions have been made to provide required
control inspection and testing.
o A physical examination of required materials, equipment, and sample work
to assure that they are on hand and conform to approved drawings or
submitted data.
o Discussion of procedures for constructing the work including the review of
repetitive deficiencies.
Initial Phase
During this phase, the quality officer and the team will verify that control for
the work developed in the preparatory phase meeting is implemented and the
work is performed to the level of workmanship mutually agreed to. The
following activities will be carried out:
o Check preliminary work
o Verify adequacy of controls to ensure that full contract compliance.
o Establish level of workmanship.
o Resolve all differences.
o Check safety to include compliance with the safety plan and activity
hazard analysis.
This phase will be repeated for each new crew to work onsite, or any time
established level of workmanship.

Follow-up phase
Daily checks shall be performed to assured continuing compliance with
contract requirements, including safety and control testing, until completion
of the particular feature of work. The checks shall be made a matter of
record in the quality control report. Final follow-up checks shall be carried
out and all deficiencies corrected prior to the start of additional features of
work. The Quality control personnel will continually refer back to the
standards set in the preparatory and initial phases.
