BS PSYCHOLOGY PROGRAM
TRAINING CONTRACT
Know All Men by These Presents:
This Training Contract is made and entered into in Davao City,
Philippines, on the 23rd day of February, 2015 by and between the
UNIVERSITY OF MINDANAO (with SEC registration No. 1849 dated July 27,
1946), College of Arts and Sciences (with Government Recognition No. 632 S.
1953/PSYCHOLOGY PROGRAM), a duly recognized institution by the
Commission on Higher Education (with Government Recognition No. S1993
and No. S. 1993 for 1 st-2nd yr. and 3rd-4th yrs. respectively of the B.S.
Psychology course), represented by KHRISTINE MARIE D. CONCEPCION,
Ph.D. (DEAN, of the College/University) and NOEMI U. BATAGA, MAEd-GC
(BS PSYCHOLOGY Program Coordinator) hereinafter referred to as the PARTY
of the FIRST PART, DAVAO CITY TREATMENT AND REHABILITATION
CENTER FOR DRUG DEPENDENTS, with office address at Bago Oshiro,
Tugbok District, Davao City, represented by OLIVER CAPUNAN(Supervisor),
hereinafter referred to as the PARTY of the SECOND PART, and
JOHN
RUSSELL V. MORALES, hereinafter referred to as the PARTY of the THIRD
PART, presently enrolled in Bachelor of Science in Psychology of the
UNIVERSITY OF MINDANAO, second term, second semester, 2015.
WITNESSETH
1. The PSYCHOLOGY PROGRAM of the College of Arts and Sciences Education
of the UNIVERSITY OF MINDANAO, as the party of the FIRST PART agrees
and allow the affiliation and training of Psychology students at the Party of
the SECOND PART subject to the policies, procedures and guidelines
governing the Internship Program of the BS Psychology Program and the
said HTE of deployment, further than the FIRST, SECOND and THIRD
PARTIES shall be responsible and liable to the fullest extent of their
respective roles, tasks, and responsibilities, to wit as follows:
A. The Internship Adviser. The Internship Adviser serves the liaison
function between the school and the agency. She/he focuses on what the
individual student is gaining and learning in the internship program in
relation to her /his total academic experience. Further, the Internship
Adviser
1. Consults with the HTE Supervisor about the placement of student/s
assigned. Among other things consultation shall aim to arrive at a
feasible program to activities for the Internship Program, which
considers each students need, as well as the minimum requirements
spelled out for each.
4. The status of the student while in the Internship Program shall be that
of the student trainee/learner and not an employee/employee
relationship.
5. The student intern must devote a minimum of 8 hours per day, and be
constantly under the supervision of HTE and the internship instructor.
6. The student while in training shall progress from task to task in order
to gain experiences in all phases of operations and duties.
7. The student intern shall observe the rules and regulations of the HTE,
to abide by all implied and stated terms in the memorandum.
8. The student intern agrees further to exert every effort to comply with
all time requirements (i.e. internship uniform, journal, DTR, outputs,
among others)and to notify the agency and the program
head/internship instructor concerned of any unavoidable absences.
9. The Psychology agency agrees further to make a report to the program
head through its Internship Adviser, on the progress of student trainees
as to their work attitudes and regularity of attendance.
Any violation of the above responsibilities, obligations, terms and
conditions may be a legitimate ground for removal in the Internship Program
and cancellation of this Training Contract.
This Training Contract shall be effective upon the commencement of
the internship program of the BS Psychology in Clinical Setting.
IN WITNESS whereof the parties here to have signed this contract in
Davao City, Philippines, this 23rd day of February, 2015.
