ACTION / CONTROL MEASURES

Required action

1. PTW/TBT to be discussed with all

persons involved in the Task.
2. Person in charge to conduct briefing
with all personnel involved and ensure
all persons are fully aware of their
responsibilities and duties.
All persons to sign tool box Talk in
acknowledgement of understanding of
discussed topics.
3. Person in charge to ensure
understanding via spot questions.
4. Bosun and Chief officer to ensure all
control measures are in place before
commencing the job.
1. The power supply on the machine
must not be opened to reset the
breaker whist the machine is
connected to power supply. (call
electrician)
2. The instruction manual is to read
completely before initial start-up.
3. This machine should be run by
trained persons who are fully
familiar with the possible dangers.
4. The Instruction for use should be
explained to all personal prior to
operate the machine. The duty
engineer must be called to start the
FW booster pump when the
machine is running.
5. Inspection of the machines must be
carried out to check for defectives
such as :
a. NO damaged must be
observed on the hoses, and
defective hoses must be
replaced by original hoses.
b. All damaged hoses must
never be repaired and
reused.
c. Check for defectiveness of
power cable, plug
switches, etc
d. Only designated extension
cables to be used.
e. Extension cables on reels
to be completely reeled off
during operation in order
to avoid overheating of the
cable.

must not be tied back. This
will prevent the water jet
from stopping if the lance
is dropped.
i.
The machine must not be
run in areas jeopardized by
fire or explosions.
j.
Periodic inspection of
machine and hoses to be
carried out during the job.
6. Work area to be cleared of loose
objects and trip/slip hazards. Sensitive
fittings that could be damaged by the
water jet to be identified before starting
work.
7. The least amount of joined hoses
should be used to access the work site.
8. Single barrel lance only to be used
with HPCE 520 machine. Swapping
accessories between HPCE 520 and 330
are not interchangeable due to nozzle
size. Using incorrect nozzle will cause
damage to machine.

Operators to ensure a stable
foothold.
Be aware of torque and recoil. The
use of spray wands shorter than
750 mm is prohibited.
Make sure that the machine
cannot move and ensure the unit is
parked with a brake and the
machine is in balance or secured
position.
Personnel to use hardhat with face
shield and safety glasses with
rubber gloves.
The work areas must be clearly
identified and controlled with
effective signage and barrier tapes
to restrict access to unauthorised
workers.
All hoses and coupling connection
to be equipped with whip checks.
Always check the nozzle orifices
before use to ensure that they are
clear of debris that could block the
flow of water and cause over
pressurization of the system or an
unbalanced nozzle.
When hand lancing, the operator
should always maintain a body
position that enables the greatest
control of the lance.
Lighting and ventilation of
confined spaces must be adequate
to ensure that workers have good
visibility and clean atmospheric
air.
Never modify the jetting gun with
shorter barrel lengths than
originally equipped or with trigger
devices.
Never leave a high pressure
system unattended. Always
disengage the high pressure pump
when workers are absent from the
immediate work area.
Ensure that the access and egress
is always identified during any

14. Any hoses that have obvious signs
of wear and tear must never be
used. .
15. Hoses that are suspended must be
tied off in a manner that prevent
excessive sway, avoids undue
strain on fittings and does not
create short-radius bends in the
hose.
16. The object to be cleaned must not
be hand-held or secured with the
foot or another body part. Small
object must be secured fastened to
a vice or similar tools.
17. Reaction forces should be
considered and regulated based on
the ability of the worker to
maintain control of the jetting
gun.
18. Stand-by man should be used to
ensure safety areas are
maintained. If inside the marked
safety area the standby man must
wear full PPE. Proper
communications with lance
operator should be maintained.
19. Regular breaks must be taken in
rotation with standby man to
avoid fatigue whilst using the
lance.
20. Inlet water pressure must be
maintained above 2 bar when
lance trigger is operated,
otherwise machine will trip. If
machine trips clean/change inlet
filter and check ER water booster
pump is running.

At the end of the meeting, agreement has been reached that the scope of work, as identified, has been adequately assessed. All anticipated hazards have been identified, all control measures have been agreed. All risks have been reduced AFARP. The personnel, required to take action are correctly identified and in agreement
with actions.
