Original and copy of Birth Certificate with appropriate annotation by the Local Civil
Registrar printed on security paper by the National Statistics Office (NSO) and
authenticated by the Department of Foreign Affairs (DFA);
Original and copy of Certificate of Finality issued by the Court authenticated by the
DFA.
Authenticate Birth Certificate with annotation of the amended surname of the child
and Marriage Conctract of parents
For women who got married in the Philippines and chose to adopt the surname of their
husband:
Original and photocopy of the dispositive portion of the court judgment granting the
divorce;
Copy of the applicant’s birth certificate issued by the NSO and authenticated by the
DFA;
Photocopy of the data page of Philippine passport.
Copy of the Court decision declaring the annulment of marriage duly authenticated
by the DFA;
Original and copy of Certificate of Finality issued by the Court authenticated by the
DFA.
Copy of the marriage contract with the previous spouse with annotation of the
annulment printed on security paper by the NSO and duly authenticated by the DFA;
Copy of the applicant’s birth certificate issued by the NSO and authenticated by the
DFA;
Photocopy of the data page of Philippine passport.
For widows:
Death certificate of deceased spouse. If husband died in the Philippines, the Death
Certificate should be authenticated by the NSO in Manila and the DFA;
Copy of the applicant’s birth certificate issued by the NSO and authenticated by the
DFA;
Photocopy of the data page of Philippine passport of widow.
For those whose births were registered, follow requirements for new applicants.
For those whose births were not registered.
o Certified True Copy of Late Registered Birth Certificate from the National Statistics
Office.
o Original and photocopy of Voter's Affidavit or NBI Clearance or other supporting
documents indicating date and place of birth and citizenship.
o Certificate from the Office on Muslim Affairs for Muslim convert
o For Muslim converts, submit annotated Birth Certificate in Security Paper (SECPA)
o Court Order to change name.
o Shari'ah Court Order.
