IMPORTANT INFORMATION
ON THE 1998 BOILER AND
PRESSURE VESSEL CODE ADDENDA
DISTRIBUTION SCHEDULE
There is a change in the way the Addenda to the 1998 Edition of the ASME Boiler and
Pressure Vessel Code are distributed. The 1998 Edition, published July 1, 1998, incorporates the 1998 Addenda revisions, additions, or deletions. There will be no separate 1998
Addenda issued in a replacement page format. Two additional Addenda to the 1998
Edition, in the form of replacement pages, will be issued on July 1, 1999 and July 1, 2000.
The Summary of Changes published with the 1998 Edition lists and describes the revisions
that are part of the 1998 Addenda. These changes are identified with a margin note, 98,
denoting the affected area.
The Addenda for 1999 and 2000 will also have a Summary of Changes and identifiers
printed on the replacement pages. The revisions, additions, or deletions will be incorporated
directly into the affected pages. It is advisable to retain the title sheets and all replaced
pages for reference.
The effective dates for Code Editions and Addenda are described in the Foreword.

additions, or deletions are incorporated directly into the affected pages. It is advisable, however,
that all replaced pages be retained for reference.

This Article provides the requirements for inspection


by the Authorized Inspection Agency.

PERFORMANCE OF INSPECTION

Authorized Inspection Agency

(a) An Authorized Inspection Agency is one designated by, or is one acceptable to, the appropriate
Enforcement Authority of a state of the United States
or a province of Canada. The Authorized Inspection
Agency shall be accredited by the Society in accordance
with the provisions set forth in ASME QAI-1.
(b) The Authorized Inspection Agency shall notify
the Society when it enters into an agreement with a
Design Owner or Class TP Certificate Holder, or whenever an existing agreement is terminated. The Authorized Inspection Agency shall also notify the Enforcement Authority whenever an agreement with a Design
Owner, Packaging Owner, or Class TP Certificate
Holder is written or a Design Owners, Packaging
Owner, or Class TP Certificate Holder agreement is
terminated.

(a) Supervisors, in conjunction with Inspectors employed by the same Authorized Inspection Agency
(WA-5121), shall participate in the Societys review
of the applicants Quality Assurance Program (WA8160). In those cases where the Supervisor performs
the functions of the Inspector, he may represent both
during the review of the Program. A Supervisor designated by the Authorized Inspection Agency shall review
and accept any proposed modifications to Quality Assurance Manuals before they are put into effect. The
Authorized Nuclear Inspector Supervisor shall audit the
Inspectors performance at least twice per year at
locations where the Class TP Certificate Holder is
actively engaged in Section III work. The Supervisor
shall be available as needed for consultation and support
of the local inspection staff. The Supervisor shall
maintain supervisory control over one or more Authorized Nuclear Inspectors and shall perform all of the
functions and maintain the records required of him in
ASME QAI-1.
(b) The portion of a Class TP Certificate Holders
Quality Assurance Program that involves supply or
manufacture and supply of materials [WA-3820(c)] shall
be audited by the Supervisor at least once each year.

The Authorized Inspection Agency shall employ Authorized Nuclear Inspection Supervisors qualified in
accordance with ASME QAI-1, to supervise the Inspectors.

The duties of the Inspector shall include but not


necessarily be limited to those given in (a) through (i)
below:
(a) verifying the scope of work to be performed
[WA-5230(a)];
(b) monitoring of the Design Owners, Packaging
Owners, and Class TP Certificate Holders Quality
Assurance Program including subcontracted activities
(WA-5240);
(c) reviewing of Design Owners, Packaging Owners, and Class TP Certificate Holders qualification
records (WA-5250);
(d) verifying materials (WA-5260);
(e) witnessing or verifying in-process fabrication,
nondestructive examination, and tests (WA-5270);
(f) witnessing final pressure tests (WA-5280);
(g) certifying Data Reports and Construction Reports
(WA-5290);
(h) reviewing drawings and inspecting in accordance
with them;
(i) performing all other duties specifically required
in ASME QAI-1.

(a) The Certificate Holder shall arrange for the Inspection Agency Personnel to have free access at all
times to those locations where Code activities, including
those concerned with supply or manufacture of materials, are being performed on an item, when so requested.
The Class TP Certificate Holder shall keep the Inspector
informed of the progress of the work and shall notify
him reasonably in advance when the item will be ready
for any required tests or inspections.
(b) The Class TP Certificate Holder shall provide
personnel to accompany the Authorized Nuclear Inspector Supervisor during his required audits.

Access to the Design Owners and


Packaging Owners Facilities

The Design Owner and Packaging Owner shall arrange for the Authorized Inspection Agency Personnel
to have free access to the Design Owners and Packaging
Owners facilities as required to perform duties under
the Design Owners and Packaging Owners Agreement
with the Authorized Inspection Agency (WA-5121).

WA-5200 DUTIES OF INSPECTOR

GENERAL INSPECTION DUTIES

(a) The Inspector who performs the detailed inspections in compliance with this Division shall witness or
otherwise verify all examinations and make all inspections required by this Division. He shall also make
any other inspections and witness or verify (including
making measurements) any other examinations and
additional investigations which, in his judgment, are
necessary to ascertain whether the item being inspected
has been constructed (WA-1110) in compliance with
the rules of this Division. Parts and piping subassemblies
shall be in accordance with the accepted documents
(WA-3251).
(b) The duties of the Inspector shall not be interpreted
by virtue of these rules to extend to any construction
requirements beyond those of this Division which may
be set forth in the Construction Specifications (WA3451). However, such requirements shall not result in
construction which fails to conform with the requirements of this Division (WA-3570).

properly certified in accordance with WA-3255 and
WA-3455.
(c) The Inspector shall not be held responsible for
the scope or adequacy of the Design Specifications,
for the completeness or accuracy of the Design Report
or calculations or for the qualification of registered
Professional Engineers certifying documents in compliance with the requirements of this Division.
(d) The Inspector shall verify that Design Calculations have been prepared for those components not
requiring Design Reports. The Inspector shall not be
responsible for the accuracy of the calculations.

Society. The Inspector shall verify that all changes to
the Quality Assurance Manual have been accepted by
the Authorized Inspection Agency before they are put
into effect.
(b) The Inspector shall monitor the Packaging Owners compilation supporting data needed to complete
the ASME Data Report Form N-3.

Examination Procedures

The Inspector shall assure himself that the examination and testing procedures required by this Division
have been qualified. When there is a specific reason
to question whether the examination or testing procedure
requirements are being met, the Inspector may require
requalification of the procedure.

The Inspector shall indicate on the Class TP Certificate Holders process sheets or checklist his concurrence
that compliance has been attained at each point stipulated by him (WA-5241).

Review of Qualification Records

Nondestructive Examination Personnel

The Inspector has the duty to verify the qualification


and certification of nondestructive examination personnel employed by the Class TP Certificate Holder and
has the duty to monitor the nondestructive examination
activities and require requalification of any personnel
when there is reason to question the performance of
that person. In addition, the Inspector shall monitor
the Class TP Certificate Holders Quality Assurance
Program as it relates to the nondestructive examination
activities of Material Organizations, and NDE subcontractors that the Certificate Holder qualified.

The Inspector shall review the qualification records


of the Class TP Certificate Holder.
WA-5253

Welders and Welding Operators

The Inspector shall assure himself that all welding


is performed by welders or welding operators qualified
under the provisions of this Division. The Class TP
Certificate Holder shall make available to the Inspector
a certified copy of the record of performance qualification tests of each welder and welding operator as
evidence that these requirements have been met. When
there is a specific reason to question the ability of the
welder or the welding operator to make welds that
meet the requirements of the specification, the Inspector
may require requalification before the welder or welding
operator is permitted to continue welding on work
subject to his inspection. The Inspector shall also assure
himself that each welder and welding operator has been
assigned an identifying symbol and that such symbols
are regularly and consistently applied when required
by this Division.

Prior to the issuance of process sheets or controls


required by WA-4134.9, the Class TP Certificate Holder
shall review them and the applicable drawings with
the Inspector, who shall then stipulate the inspections
he intends to make in order to fulfill the requirements
of WA-5210.

WA-5261

Welding Procedures

The Inspector shall assure himself that the welding


procedures employed in construction have been qualified
under the provisions of this Division. The Class TP
Certificate Holder shall submit evidence to the Inspector
that these requirements have been met. When there is
a specific reason to question the welding procedure,
the Inspector may require requalification as a requirement for the procedure to be used on work subject to
his inspection.

Compliance

The Inspector shall assure himself that all materials


used comply with all applicable requirements of this
Division. The Class TP Certificate Holder shall make
available to the Inspector certified reports of the results

of all tests performed in accordance with (a) and (b)


below:
(a) the material specifications;
(b) the requirements in the applicable materials
Articles of this Division, including certified reports
of the results of all required tests and examinations
performed.

Inspector only after they have been certified by a


responsible representative of the Class TP Certificate
Holder and after he has satisfied himself that all requirements of this Division have been met and that each
Data Report certified is a correct record.
(b) The N-3 Data Report Form (WA-3220) shall be
certified by the Inspector only after:
(1) it has been certified by the Packaging Owner;
(2) the Inspector has reviewed the N-3 Form and
verified that the Data Reports referenced on the N-3
Form are on file and that such Data Reports verify Code
compliance of all components or parts incorporated into
the containment system of the transport packaging or
that portion of the system covered by the N-3 Form.

Dimensional Check

The Inspector shall satisfy himself:


(a) that the item is being constructed within the
tolerance required by the Design Specification, Design
Drawings, and Construction Specifications, and this
Division;
(b) that head and shell sections conform to the
prescribed shape and meet the thickness requirements;
(c) that fittings and attachments to be welded to the
vessel fit properly to the curvature of the vessel surface.
If required by the Inspector, the Class TP Certificate
Holder responsible for the vessel shall make available
accurately formed templates for his use.
WA-5263

WA-5300 RESPONSIBILITIES OF THE


AUTHORIZED INSPECTION
AGENCY
The responsibilities of the Authorized Inspection
Agency shall include but not necessarily be limited to
those given in (a) through (k) below.
(a) Maintain a staff of Authorized Nuclear Inspectors
and Authorized Nuclear Inspector Supervisors (WA5123).
(b) Make agreements with Design Owners, Packaging
Owners, and Class TP Certificate Holders for inspection
service (WA-5121 and WA-8130). Notify the Society
whenever such agreements are terminated (WA-5121).
(c) Provide for participation in the Societys review
of the applicants Quality Assurance Program (WA5125).
(d) Provide for the review and acceptance of any
proposed modifications to Quality Assurance Manuals
before they are put into effect (WA-5125).
(e) Review and accept the Class TP Certificate Holders method of securing the nameplate to components
to which, because of size or other considerations, the
nameplate cannot be directly attached [WA-8220(b)].
(f) Review and accept the Class TP Certificate Holders alternative method of identification, including the
unique method of marking of components to which,
because of size or other considerations, nameplates
cannot be directly attached [WA-8220(b)].
(g) Review and accept the Class TP Certificate Holders alternative method of marking of parts and piping
subassemblies (WA-8230).
(h) Review and accept the Class TP Certificate Holders provisions of positive identification and traceability
of items from which nameplates are removed [WA8240(b)].

Check of Heat Treatment Practice

The Inspector shall satisfy himself that all heat


treatment operations required by this Division are correctly performed and that the temperature readings and
gradients conform to the requirements.

WA-5270

EXAMINATIONS AND TESTS

The Inspector shall witness in-process fabrication


nondestructive examinations and destructive tests, when
feasible; alternatively, he shall check the examination
and test records to determine the acceptability of the
items involved.

FINAL TESTS

The Inspector shall witness final hydrostatic, pneumatic, structural integrity, or leakage tests required by
this Division and examinations performed during such
tests by the Class TP Certificate Holder.

General Requirements

Essential Variables for Manual


Welding

The welding procedure must be qualified as a new


procedure specification and shall be completely requalified when any of the changes listed in (a) and (b)
below are made in the procedure:
(a) a change of more than 10% of any dimension
of the weld joint cross section other than the included
angle of the groove, except that base material thickness
may vary from minus 10% to 2.5 times the thickness
qualified when welding parts of the same material
thickness, or the thin member may vary from minus
10% to 2 times the thickness qualified when welding
a thin seal membrane to a thick member;
(b) when preplaced filler metal is melted to form
all or part of a weld, a change from one alloy type
or classification to any other alloy type or classification
of base material or filler metal, even though previously
qualified base materials and filler metals are of the
same P-Number or A-Number.

(a) Specially designed welded seals are defined as


the walls or membranes, such as an omega shaped seal
membrane, which confine the fluid and where strength
is provided by a separate device.
(b) The welding procedure shall be qualified as a
new procedure specification and shall be completely
requalified when any of the essential variables specified
or listed in the following paragraphs are made in the
procedure. Changes other than those so given may
be made in the procedure without the necessity for
requalification provided the procedure is amended to
show these changes. In addition, the essential variables
specified in Section IX shall apply for both procedure
and performance qualification.

Essential Variables for Automatic,


Machine, and Semiautomatic Welding

The welding procedure shall be qualified as a new


procedure specification and shall be completely requalified when any of the changes listed in this paragraph
or the applicable portions of Section IX are made:
(a) when preplaced filler metal is melted to form
all or part of a weld, a change from one alloy type
or classification to any other alloy type or classification
of base material or filler metal, even though previously
qualified base materials and filler metals are of the
same P-Number or A-Number;
(b) a change of any dimension of the weld joint for
automatic or machine welding or a change of any
dimension of the weld joint by more than 10% for
semiautomatic welding beyond that qualified;
(c) a change in the nominal size or shape of any
filler metal added to the arc;

Test Assembly

The test assembly shall consist of a duplicate of the


production weld except that tolerances as stated in
WB-4362(b) or WB-4363(a) are permitted. Except for
automatic welding, the Certificate Holder may use a
12 in. (305 mm) length to qualify, if the length of the
production weld is considered to be greater than necessary to qualify.
WB-4366.1 Automatic Welding. For automatic
welding, at least six consecutive test assemblies representing the range of dimensions to be qualified shall be
required to establish the reproducibility of the welding
procedure. In addition, the Certificate Holder making
the production welds shall verify this capability of

to adjustments and settings which affect the welding

When used in components other than piping, backing


rings shall conform to the requirements of WB-4240.
Backing rings shall not be used in piping unless removed
after welding and the inside surfaces of the roots are
examined by a magnetic particle or liquid penetrant
method, in accordance with WB-5110, and meeting the
acceptance standards of WB-5340 or WB-5350. The
material for backing rings, when used, shall be compatible with the base metal. Permanent backing rings, when
permitted by WB-3342, shall be continuous, and any
splices shall be made by full penetration welds. Spacer
pins shall not be incorporated into the welds.

Performance Qualification Test

Cleanliness and Protection of Welding


Surfaces

The method used to prepare the base metal shall


leave the weld preparation with reasonably smooth
surfaces. The surfaces for welding shall be free of
scale, rust, oil, grease, and other deleterious material.
The work shall be protected from deleterious contamination and from rain, snow, and wind during welding.
Welding shall not be performed on wet surfaces.

(a) Where 100% weld penetration is required, the


test assembly shall be sectioned, if necessary, to permit
examination of the entire underside weld surface.
(b) A minimum of four cross sections shall be taken
from each test assembly. One cross section shall be
made in a weld start and stop area and the others shall
be taken at random. Each cross section shall be examined
at 10 to 15 magnification in accordance with ASTM
Standard E 2. All surfaces shall be free of cracks,
incomplete penetration, incomplete melting of insert on
consumable type welds, and porosity or inclusions in
excess of one rounded void with the maximum dimension not greater than 10% of the thickness of the weld.
When doubt exists as to the acceptability of the weld,
after examination of four cross sections, the Inspector
may require that additional metallographic cross sections
be prepared for examination of the weld and adjacent
base material.

Miscellaneous Welding Requirements

(a) Before applying weld metal on the second side


to be welded, the root of full penetration double welded
joints shall be prepared by suitable methods, such as
chipping, grinding, or thermal gouging, except for those
processes of welding by which proper fusion and penetration are otherwise obtained and demonstrated to be
satisfactory by welding procedure qualification.
(b) If the welding is stopped for any reason, extra
care shall be taken in restarting to get the required

Weld Metal Cladding

Hard Surfacing

Hard surfacing weld metal shall be examined by the


liquid penetrant method in accordance with WB-2546,
and the acceptance standards applicable to materials
less than 58 in. (16 mm) thick shall apply. Penetrant
examination is not required for hard surfacing on valves
with inlet connections NPS 4 (DN 100) or less.

Special Exceptions

When the joint detail does not permit radiographic


examination to be performed in accordance with procedures of this Subsection for joints attaching penetration
assemblies, which are fabricated as appurtenances or
of the closing seam within an electrical penetration
assembly, not necessarily fabricated as an appurtenance,
ultrasonic examination plus liquid penetrant or magnetic
particle examination of the completed weld may be
substituted for the radiographic examination. The absence of suitable radiographic equipment shall not be
justification for such substitution. The substitution of
ultrasonic examination can be made provided the examination is performed using a detailed written procedure
which has been proven by actual demonstration to the
satisfaction of the Inspector as capable of detecting
and locating defects described in this Division. The
nondestructive examination shall be in accordance with
Section V and meet the acceptance standards of
WB-5300.

Tube-to-Tubesheet Welded Joints

Tube-to-tubesheet welded joints shall be examined


by the liquid penetrant method.

Electroslag Welds

In addition to the requirements for the type of weld


being examined, all complete penetration welds made
by the electroslag welding process in ferritic materials
shall be ultrasonically examined.

Weld metal cladding shall be examined by the liquid


penetrant method.

Electron Beam Welds

In addition to the requirements for the type of weld


being examined, all complete penetration welds made by
the electron beam welding process shall be ultrasonically
examined.

Welded joints of this type shall be examined by


either the magnetic particle or liquid penetrant method.

Inertia and Continuous Drive Friction


Welds

(a) When radiographic examination is required by


this Article, inertia and continuous drive friction welds
shall also be examined by the ultrasonic method to
verify bonding over the entire area.
(b) The materials used shall be those assigned a PNumber by Section IX, but shall not include rimmed
or semikilled steel.
(c) One of the two parts to be joined must be held
in a fixed position and the other part rotated. The two
faces to be joined must be symmetrical with respect
to the axis of rotation.
(d) The weld between the two members shall be a
full penetration weld.

Fillet and partial penetration welded joints, except


for nonstructural attachments (WB-1132.1), and socket
welds shall be examined by the magnetic particle or
liquid penetrant method.

Brazed Joints

Flux and flux residue shall be removed from all


surfaces prior to examination. Joints shall be visually
examined on all accessible surfaces to determine
whether there has been adequate flow of brazing metal
through the joint. Optical aids may be employed for
indirect visual examination of joints which cannot be
directly examined.

Indications shown on the radiographs of welds and


characterized as imperfections are unacceptable under
the following conditions:
(a) any indication characterized as a crack or zone
of incomplete fusion or penetration;
(b) any indication characterized as incomplete penetration or incomplete fusion, except that such indications
in welds between P-8 metals that were made using
GTAW or SMAW in which the deposit analysis is A8 or A-9 are acceptable provided they do not exceed
the length in WB-5320(c).
(c) any other elongated indication which has a length
greater than:
(1) 14 in. for t up to 34 in., inclusive
(2) 13t for t from 34 in. to 214 in., inclusive
(3) 34 in. for t over 214 in.

Evaluation of Indications

(a) Mechanical discontinuities at the surface are revealed by the retention of the examination medium.
All indications are not necessarily defects, however,
since certain metallurgical discontinuities and magnetic
permeability variations may produce similar indications
which are not relevant.
(b) Any indication that is believed to be nonrelevant
shall be reexamined by the same or other nondestructive
examination methods to verify whether or not actual
defects are present. Surface conditioning may precede
the reexamination. After an indication has been verified
to be nonrelevant, it is not necessary to reinvestigate
repetitive nonrelevant indications of the same type.
Nonrelevant indications that would mask defects are
unacceptable.
(c) Relevant indications are indications which result
from imperfections. Linear indications are indications
in which the length is more than three times the width.
Rounded indications are indications which are circular
or elliptical with the length equal to or less than three
times the width.

where t is the thickness of the thinner portion of


the weld;
(d) internal root weld conditions are acceptable when
the density change as indicated in the radiograph is
not abrupt; elongated indications on the radiograph at
either edge of such conditions shall be unacceptable,
as provided in (c) above;
(e) any group of aligned indications having an aggregate length greater than t in a length of 12t, unless
the minimum distance between successive indications
exceeds 6L, in which case the aggregate length is
unlimited, L being the length of the largest indication;
(f) rounded indications in excess of that shown as
acceptable in Appendix VI.

Acceptance Standards

(a) Only imperfections producing indications with


major dimensions greater than 116 in. (1.6 mm) shall
be considered relevant imperfections.
(b) Imperfections producing the following indications
are unacceptable:
(1) any cracks and linear indications;
(2) rounded indications with dimensions greater
than 316 in. (4.8 mm);
(3) four or more rounded indications in a line
separated by 116 in. (1.6 mm) or less edge to edge;
(4) ten or more rounded indications in any 6 sq
in. (3,871 mm2) of surface with the major dimension
of this area not to exceed 6 in. (152 mm) with the
area taken in the most unfavorable location relative to
the indications being evaluated.

ULTRASONIC ACCEPTANCE
STANDARDS

All imperfections which produce a response greater


than 20% of the reference level shall be investigated
to the extent that the operator can determine the shape,
identity, and location of all such imperfections and
evaluate them in terms of the acceptance standards
given in (a) and (b) below.
(a) Imperfections are unacceptable if the indications
exceed the reference level amplitude and have lengths
exceeding:
(1) 14 in. for t up to 34 in., inclusive
(2) 13t for t from 34 in. to 214 in., inclusive
(3) 34 in. for t over 214 in.
