This application document checklist details the information required to assist with the lodgement of a
complete application with the department. You may also need to provide additional information and
documentation after you have made your application if the department requires it.
Important
a. Please note that all foreign language documents submitted with your application must be
accompanied a certified English translation.
b. All original documents submitted with your application must be accompanied by a certified copy.
Original documents will be returned to you after they have been assessed by our office.

Information about the adopted child
Adoption Order issued by the Vietnamese Ministry of Justice.
Certified copy of the childs birth certificate.
Certified copy of the bio-data page of the childs passport (including the photograph).

-2Certified copy of (every page) of the family book / household register where the child currently
lives.
Two recent passport size photographs (45mmx35mm) of the child you have adopted.
If the applicant is aged 16 years or over:
a. A certified copy of their National Identity Card.
b. An original Curriculum Vitae certified by the local Peoples Committee.

Original Police Certificates from any country that the applicant have spent a cumulative time
of 12 months or more in, over the last ten (10) years, including the applicant(s) current
country of residence.

Important information on what Police Certificate(s) are required from other countries is
specified at this link, we will not accept any other type of Police Certificate. For Vietnam, the
Judicial Record Card Number 2 issued by the local Service of Justice is required. For Australia, a
National Police Check (Complete Disclosure) from the Australian Federal Police is required.

Information about the sponsor
Evidence that the sponsor has been living outside of Australia for the last 12 months prior to the
lodgement of this visa application.
Evidence as to the purpose of the sponsors overseas residence for the last 12 months (this may
include letters from your employer, tax records, university enrolment records).
Evidence that the sponsor is an Australian citizen, the holder of an Australian permanent visa, or
an eligible New Zealand citizen.
For New Zealand citizens
Evidence of length of residence in Australia and of continuing links with Australia.
Evidence of the sponsors income during the last two years.
If the sponsor is self-employed or self-funded from other sources, business documents or a
letter from your sponsors accountant.
Where applicable:
A statement from the sponsor regarding any other person for whom the sponsor has child
support payments and/or an Assurance of Support. This statement must detail the sponsors
relationship with this other person or persons; specify the dates of lodgement of any
sponsorship or nominations (including any current sponsorship) or Assurances of Support; and
specify the amount and frequency of child support payments.
Original Australian Federal Police (AFP) National Police Check (Complete Disclosure), if the
sponsor has spent a total of 12 months or more in Australia since turning 16 years of age.

Two recent passport size photographs (45mmx35mm) of the sponsor (and Partner, where
applicable). You should print the name of the person on the back of each photograph.
Original police certificates from each country in which the sponsor has spent a total of 12
months or more in the last 10 years since turning 16 years of age.
If the sponsor has been resident in Vietnam, please provide a Police Clearance Number 2
(Judicial Record Card Number 2) from Service of Justice.

If the sponsor has a spouse of de factor partner, we also require the above Police Clearances
from that person.
