1. General
Slab (plate) is a horizontal structural element which serves to channel dead load and live load to
the vertical supporting frame structure of a system. The horizontal elements can be made to
work in one direction or the work of two mutually perpendicular directions (biaxial).
Ground slabs are those slabs that are poured directly into excavated trenches in the ground.
They rely entirely on the existing ground for support. The ground (more correctly known in the
industry as the foundation) must be strong enough to support the concrete slab.
This document is describing general part of the method for Ground Slab work. This document
includes the scope of work, location, resources, schedule, work method, environmental plan,
and safety plan.

Sequence of Ground Slab Work

a) Formwork Fabrication and Installation


Marking for dimension and position of the formwork based on drawing approved, joint every
formwork with closely and carefully until formed as good as possible. Thoroughly clean the
areas that will be used for concreting. Formwork is finished and can be proceed with the
Rebar installation.
b) Rebar Installation
Prior rebar installation the construction team must be check the following point such as:

The depth or concrete cover

The rebar dimension

Rebar cutting and bending

Make guidance for levelling concreting

Make sure installation for additional equipment and check on the position

Manufacturing, process consist of cutting and bending reinforcement bar. Cutting


reinforcement bar is used with bar cutter, bending used bar bender.
All of these things must be accordance with the design drawing and plan.
c) Concreting
Check the slump of ready mix concrete according to quality/grade of concrete required and
prepare cylinder sample for compressive strength. Then clean the location from all of the
dirt, mud, dust and other things that may cause harm to the concrete. High free fall pouring
the concrete for a maximum of 1.5 m, if it is used tremie tunnel (can be a hose/pipe).
Pouring evenly arranged so that the alignment with manual horizontal direction can be
avoided and compaction using vibrator machine to make sure that the concrete fill the area
and decrease the probability of honeycomb.
When the vibrating needle began slippery surface pull gently until the hole left vibrator
vibrating needle close by itself (1 point 15 seconds) and during the time of casting, or
formwork scaffolding condition is monitored. After reaching the top level of casting carried
out finishing with a trowel.
d) Curing

Curing need to prevent concrete lose moisture content rapidly. Curing method for this
project is covering concrete by wet blanket sheet.
5. Inspection and Test Plan
Inspection and Test Plan is referred to TCI-TWJO-ITP-SLB-XXXX
6. Environmental Plan
TCI will consider for environmental impact during construction. The consents of
environmental plan on this activity are as follows:
a) Dust Control
The possible treatment on dust control is including:
- Watering the working area regularly during the activity.

Always using Personal Protective Equipment (PPE) for worker

b) Noise
The control of noise instrument will be applied around working area to control the noise
always keep on tolerance.

c) Cleaning for Tire of Transportation Vehicle


Transportation vehicle delivery concrete truck for material will be used for concreting
work. Before going out from site, condition of tire shall be checked whether tire is dirty, In
case that the tires of vehicle become dirty, washing by jet washer and manual will be
conducted to prevent the existing road from being dirty by vehicle.

d) Cleaning around the Site


During Ground Slab work, TCI will keep cleaning in surrounding area, Include inside
area that Ground Slab work on process.

7. Safety Plan
Safety is important things that shall be enforced during Ground Slab work activity. The
relevant safety procedure will be applied during this work.
The safety plan that will be applied is including:
a. Access Road

TCI will provide a safety access road for heavy equipment passing and cross, a
boundary line will be carry out around the access road to Area.

b. Job Safety and Analysis (JSA), Permit To Work and Toolbox Talk Agenda,
explain :

c. Personnel Standard Safety Tools,


i.e. safety helmet, reflector vest and safety shoes. These safety tools shall be
appropriate with safety standard. TCI will keep all of personnel to always use Personnel
Standard Safety Tools during working. During Ground Slab install, TCI will provide safety
harness for workers

d. Safety on the equipment before operated


Safety officer and mechanic from TCI must ensure condition heavy equipment in
process pre mobilization before heavy equipment mobilization to site. Safety Officer
must ensure heavy equipment completed with: Document (SIA), Operator License (SIO),
Safety Device, Rotary Lamp, Back Alarm, Fire Extinguisher, First Aid Box. Mechanic
must ensure heavy equipment in good condition and ready to work.
e. Safety on the equipment when operated
Safety Officer and Site supervisor must ensure working area in good condition.

Safety Facilities

TCI will provide safety facilities at working area. The safety facilities including: traffic
sign, barricade, lamp, PPE accessories (gloves, glasses etc.), shelter, toilet etc.
g. Safety Scaffolding
TCI will provide scaffolders by certificated personnel to ensure and check the scaffolding
is safe to be used. Safety chief Scaffolder will be tagging the scaffolding with two color
(red tag and green tag). Red tag mean the scaffolding need more repair for support and
strengthnes , and green tag is mean the scaffolding are ready and safe to be used.

Prior to the commencement of this work, Safety Officer will conduct safety instruction to all
workers and other relevant project staff to keep working on safe condition. Safety Officer
also will always monitor during this work carried out to prevent unsafe action and unsafe
condition during this work.
