CURRENT MEDICATIONS

The patient has a maintenance medicine for his hypertension, which is

Amlodipine 5mg and Aspirin 80 mg. He verbalized that he does not take any
medication for his cough but his attending physician gave him a prescription of
medications to take which was dated November 26,2016. The patient says he
uses the prescription, which has the following medications:
1. Piptaz 4.5g
2. Salbutamol + ipratropium
3. Ferrous + Folic Acid
4. Calcium Carbonate 500mg
5. Amlodipine 10mg
PERSONAL, SOCIAL HISTORY AND USUAL DIET
The patient is a Catholic, fourth year high school graduate. He used to
work as a maintenance man at Carmenville Subdivision and Timog Park and used
to wear protective gears like helmet and mask but due to his illness he does not
work anymore, also his wife does not work but instead stays at home with him to
do household chores. He verbalized that he has three sons and one grandson. He
claims that they get their income by means of his sons work. The first-born is a
tricycle driver, he is married and has one son, second is a construction worker and
last is a school service driver.
According to the patient he was a smoker in his teenage years and claims
to smoke Marlboro red 10 packs a day and that he drank 2 bottles of beer and
also drank gin (tanduay, white castle gin) every Sunday with his friends when he
was a teenager. He said that he does not recall when he stopped drinking and
smoking. He denies trying or taking illicit drugs.
The patient verbalized that when he was young he used to play basketball
but now, he jogs every morning together with his wife. He also said that since the
time he underwent dialysis he did not do chores at home.
The patient claims that he avoids a lot of food due to his illness and the
doctors advice him to avoid fatty foods, meat, pork, chicken, fruits, juices, soft
drinks, milk etc. He verbalized that he is restricted to only 1L of fluid a day, which
was advised to him by his attending physician. For breakfast, lunch and dinner he
only eats fish which they just cook in different ways such as grilled, fried or made
as paksiw. He claims he was not given any supplements his doctor
He verbalize that he has a hard time sleeping at night and he claims that
he only sleeps an estimated 2 hours because he said that sometimes at night he
feels like he is drowning.
ENVIRONMENTAL AND LIFESTYLE HISTORY
They are a nuclear type of family, his eldest son does not live with them
anymore because he has a family of his own; their house is made of concrete and
cement with 1 bedroom where he and his wife sleeps in while their sons sleep at
their living room. They have one toilet which they empty using a pale of water for
flushing the excreta, their garbage is collected regularly 1 2 times per week by
the barangay. The patient verbalize that there are no factories nearby and their
area is clean and dust free. Their roads are cemented and its not an air-polluted
place.
IMMUNIZATION HISTORY
Patient XY claims that he was immunized when he was young. These
vaccines are as follows: #1 BCG, #3 DPT, #3 OPV, #3 Hep B, #1 Measles but he
is not immunized for Pneumonococcal and Haemophilus Influenza Type B. vaccine.
