Other Observations:
1. Following works were being performed at height during night;
a) Cable pulling works in cable trays at steel racks in West side of 340.
b) Welding/Cutting work at top platform of Steam reformer in 305.
2. Exhaust pipe of a diesel power generator in South of 305 was giving sparks being spread in the vicinity with the wind. The wire mesh used as spark
arrestor was red hot and almost burnt out. This could have been dangerous if combustible material was there in the vicinity. Safety supervisor was
immediately informed and shown as well as HUALU Safety Manager was informed too.
3. A labor was performing bolt tightening at some height standing on a temporary unstable platform.
4. Poor lighting arrangement was observed specially in the North of 312X where a labor was working with the help of a pocket torch.
