Dear Sir or Madam,

I acknowledge that my son, Karl Ian Maliwat, refused the van rental of the UPCP as
the mode of transport to the staff house in Indang, Cavite. All risks that will be
encountered throughout the trip to the said staff house will be voluntarily assumed
by my son or me.
I hereby agree not to hold the school or any individual affiliated with the institute
liable to any loss, expense, personal injury or accident that will happen to my child
which is not the result of any negligent act of any employee of the institute.
