Our safety and prevention plan is to clarify the guidelines and procedures which will be
followed during working in (MARAFIQ) Project. Our objective is to maintain the highest
health and safety for the benefits of the workers, subcontractors, suppliers, client, public and
ourselves. Our commitment is that everyone takes responsibility for working safely and for
the safety of fellow workers. We believe that all safety related incidents are preventable.

3 COMPANY SAFETY POLICY:

KAFOU is committed to carry out the responsibilities and duties in a manner designed
to achieve and maintain a high standard of protection for our employees, and those of
our Contractors and Clients, the environment, property, equipment and the public in the
conducts of our business.
KAFOU will continually strive to enhance these standards in all phases of our
operations, from the initial planning and design stages through overall Construction and
including final commissioning turn over, and start-up operations.
The KAFOU management, supervision, employees along with our Subcontractors and
their employees are charged with the responsibility of providing and maintaining the
highest degree of Safety and Health performances and protection of our environment and
the Safety of others affected by our work.
It is this commitment to excellence, which motivates the KAFOU to be diligent in the
placement of field supervision and selection of Subcontractors. Through our established
programs as well as relevant training and open communication. We nurture the
employee's desire to excel in the areas of Safety and quality of work.
Consistent with its commitment, the KAFOU through its Project Management Team will
ensure full compliance with standards and safety requirements as covered in the Project
specification throughout the duration of project.

All provisions contained in this "Industrial Safety and loss Prevention Plan" shall be
totally adhered to by Subcontractor work forces in carrying out the necessary execution of
all works required for the Project.

4 SAFETY ASSIGNMENT OF RESPONSIBILITIES


4.1 PROJECT MANAGER

The Project Manager shall familiar with the terms accident, hazard, hazard control and
loss control. The front-line supervisor and upper management should know the procedures
for accident prevention and the step to follow in case of any accidents and know the best
approach to safety performance. He is responsible for the sub contractor and will help
him (sub-contractor) in establishing safe work practice.

The Project Manager shall:

Understand MARAFIQs Standard and ensure that they are followed.


Participate in the scheduled site inspection and contractors performance evaluation of
Marafiq CCS (Committee for Contractors Safety Team) or Marafiq EMT.
Communicate all the OHS issue and receive feedback from all level within the organization on OHS
relevant issues.
Know the requirements of relevant Kingdom of Saudi Arabia Government Workmen
regulations, and make sure it will be implemented.
Set personnel example on site by wearing appropriate PPE.
Ensure all the accidents are reported in accordance with the SEC Project Management
Reporting Guideline.
Ensure implementation of the SEC approved Loss Prevention Program.
Know basic precaution to handle construction hazards. Hazard control involves
developing a program to recognize the destructive effort hazard arising from human
error.
Provide written instruction to the subordinates to understand work method, explain the
sequence of operation, outline potential hazard at each phase of work.
Check work methods and precaution with field supervisor before commencing the work
Develop safety awareness by promoting the safety meeting, presentation and by
implementing safety training.
Follow any hazards identified under the Hazard Identification Procedure..
Will provide the required PPE for all employees on project.
Will be fluent in written and spoken English.

4.2 SAFETY SUPERVISOR

A full time Safety Supervisor will be appointed on site at mobilization until the
completion of the project, to coordinate and implement the Loss Prevention Program for
contractor and sub-contractor. All Safety personnel shall completed the Basic Life Support
from internal or external training provider.

The Safety Supervisor shall:

Contractor and subcontractors Safety Officer/Engineer shall undergo Marafiq Safety Section
interview in advance for selection process and reserves the right to reject nominated persons to insist
on an appropriate competent person for the role.
Understand MARAFIQs Standard and ensure all project personnel comply with this document.
Ensure that all foreman and supervisors are aware of Safety Plan before starting of any
work.
Inspect the work site daily and correct unsafe work method and conditions. Keep a
record daily report to Project Manager when necessary.
Keep records of all injuries, accident, fire, motor vehicle accident (MVA), property
damage and crane or heavy equipment accident to MARAFIQ that happened on site.
Keep records of all daily/weekly safety meeting on site, complete with subject discussed
and signed list of attendance.
Advice the management as follows:
Ways to prevent injury to personnel, damage to the site facility or equipment and fires.
Ways to improve the existing work methods.
Provision and use of protective clothing and equipment.
Potential Hazard of site before starting of any work.
Change in safety requirement.
Non-conformance from site management.
Carry out survey and inspections to see the only safe work methods are in operation,
health and safety requirements are being observed.
Determine the cause of any accident occurrence and recommended measure to prevent
any accident.
Supervise the recording and analysis of information on injuries, damage and production
loss. Access accident tends and reviews all safety performance.
Assist by training all level employees.
Take part in discussion on injury, damage and loss control.

Keep up-date with recommended safe practice and safely literature; circulate
information applicable to each level of employee.
Attend job progress meeting where safety is an item on the agenda. Report on job safety
performance.

4.3 FIELD ENGINEER/SUPERVISORS

The Field Engineer/Supervisor shall:

Ensure that they are aware of MARAFIQs Std. before commencing of work.
Organize work so that work is carried out as required in compliance with standards with
minimal risk to men, equipment and materials.
Be familiar with MARAFIQ & ARAMCO work permit system.
Give specific instruction to the worker for the correct work methods.
Plan and maintain good housekeeping.
Coordinate with the sub-contractor and contractor to avoid any confusion about area
responsibilities.

4.4WORKERS RESPONSIBILITES

Use the correct tools and equipments for the work.


Use protective clothing and equipment provided. Do not Endanger himself or co-worker.
Keep tools in good conditions.
Report any accidents, near misses or hazardous conditions to immediate supervisor
Abstain from horseplay and abuse of safety devices, equipment and welfare facilities.
Read companies and proponent safety rules and take note of safety precaution in
restricted areas. Repeated Safety violation will lead to warning and eventual termination
from site.
Obey all the posted warning sign board.
Attend all the Safety Meeting.
Stop the work and ask other co workers to stop the work if the working condition is not
safe.

4.5 SUB - CONTRACTORS RESPONSIBILITES

Ensure that they are aware of (MARAFIQ)s Std. before commencing of work.
Understand (MARAFIQ) approved Loss Prevention and comply with all the requirements.
Ensure that all the equipment and tool mobilized on site in accordance with the (SEC).
Comply with all the Safety, Health and Securities requirement in all the area.
Attend the designated daily/weekly safety meeting given by the supervisor.
The employee nominated as Contractor Coordinator shall ensure that all the
requirements of this Element are complied by all the Sub-Contractors selected to work at
his company.
Hazard assessment and risk evaluation are performed in cooperation with the Contractor
based on the Degree of risk associated with the assigned work and the workplace.

5 EMPLOYEE SAFETY ORIENTATION

All KAFOU site personnel including the visitors shall be familiarize with clients
emergency procedures in the job sites. This will be done through pre-assignment
orientation of all job site employees prior to start of work on job site. Visitors and the
worker will be provided with the hard copy /Instructions of this general orientation
information upon arrival at job site.

All personnel who intend to perform task inside the Marafiq premises, plants and facilities
shall undergo Marafiq safety orientation training.

5.1 GENERAL SAFETY RULES

Commonsense and good judgment must be shown in the application of our daily
activities. KAFOU shall at all times comply with and ensure that its employees,
subcontractors and visitors comply with applicable Saudi Govt. Safety Regulations and
all (MARAFIQ)s Standard and Loss Prevention rules and Regulations.
All personnel working on site must be made aware on the potential hazards of the pipeline
and the actions they should follow in case of an emergency.
Personal Protective Equipment (PPE) such as hard hats, safety goggles and safety shoes
are mandatory for all personnel at all work locations. Ear protection (ear plugs),
respirators, face shields and similar personal protection shall be worn whenever required.
Full body harness shall be worn when working in elevated areas 6 feet and above,
without complete guardrails.
No activity shall take place in a restricted area unless a signed work permit is obtained
by the KAFOU work permit receiver from the appropriate issuing authority.
Employees shall report all occupational injuries, accidents, including near misses
immediately to their supervisors for appropriate preventive measures.
Fighting and or horseplay are prohibited. Employees involved in these activities are
subject to disciplinary action.
Running on the job site is prohibited.
Site employees have to report any observed unsafe condition at the work area.

Sleeping anywhere inside the construction site is absolutely forbidden. Violation of this
rule will result in immediate termination.
Safety devices and guards of any equipment shall never be removed or tampered.
Only tools, equipment and materials that are free of defect are allowed on project site.
Speed limits shall, at all time be observed on or off site.
All site personnel shall observe post job (End-of-Shift) inspection requirements.
Parking of vehicles is permitted ONLY in the designated parking area.
All site employees especially drivers shall, at all time are familiar with the existing
emergency procedures and specific alarms of the (MARAFIQ) facilities.
Use of cellphone is not allowed inside the enclosed perimeter of gas station.
No electrically operated power tools will be used unless protected with a Ground Fault
Circuit Interrupter (GFCI).
Housekeeping shall be observed frequently more than once a day in all work areas.

The required standard of housekeeping shall be established on site and all personnel
informed of this standard. Trash debris and refuse will be collected daily. All employees
shall clean their respective work areas daily before quitting.
Temporary toilet shall be provided outside the Berri station enclosed fence.
Covered containers, drums, etc. will be provided at various, clearly marked locations
throughout the work site. The containers will be emptied daily at approved rubbish
dumping area.
KAFOU will follow their approved Waste Management Program and comply with the
(MARAFIQ) Sanitary Code and safe housekeeping practices.
Accumulation of rubbish especially combustible materials is strictly prohibited.
Warning barricades and appropriate notices shall be installed and maintained where ever
required and, adhered to; e.g. no smoking sign, no entry, etc.
(MARAFIQ) identification cards shall be carried worn visibly every employee.
Riding on the rear of service pick-ups is prohibited.
While in a vehicle, wearing of seat belts must be observed on or off the site.
Under no circumstance, can unauthorized personnel operate any equipment on the
project.
Inspection of tools and equipment, including personal protective equipment, shall be
observed before and after use.
Isolation and Lock Out/Hold Tag procedures shall be observed when working on or near
energized equipment.
Supervisors must be completely aware of the actual number of personnel in his
respective area at the start of work shift.
All site personnel must exemplify safe personal behavior at all times and where required,
initiate appropriate measures in carrying out his job.
Water coolers shall be used for cooling and storage of drinking water only.
Disposable paper cups shall be provided.
Soap and tissue papers for hand washing shall be provided.

6 SAFETY TRAINING

Safety Training is to keep employees informed of the hazards of his job and how to control
them.

All K A F O U employees shall be given the necessary safety instruction concerning


his job. Pertaining shall teach the common type of hazard on site and the precautions he
must take. Employees, who will be working as welders, technicians, Scaffolding
technicians shall be trained according to their Job Skills.

A permanent record shall be kept for all safety training in accordance with established
procedures. An induction safety course shall be provided on site to point out the hazards
which could be encountered in their daily work. KAFOU is committed to upgrade and
train our employees with respect to (MARAFIQ) Work Permit System. Selected
employees who will be working in the extremely hazardous gas areas will be oriented
and shall follow all safety guidelines set by (MARAFIQ) Safety Representative.

KAFOU shall provide and maintain in good working condition suitable firefighting
equipment as per the MARAFIQ Standard. All KAFOU personnel shall properly be
trained in the use of such equipments. Storage lay down and fabrication yards shall be
laid out in accordance with MARAFIQ Standards with respect to the spacing of rows, fire
lanes and compatibility of material.
Fire spreads from an ignition source to a fuel source or other fuel sources by
Conduction, Convection, and Radiation. Conduction: - Transfer of heat through contact
with solid material, Convection: - Transfer of heat through heated air, Radiation: - Heat
transfer through electromagnetic waves given off by flames.
Removed Anticipated Volume of Hot Work
No Activity involving Hot Work is allowed and used only non-spark tools.
All equipment shall be equipped with spark arrestor.
Fire prevention will be given the highest priority by KAFOU on the project. This
requires careful planning and execution especially when activities are to be carried out
within the critical area.

Prior to Start of Work


Employees shall follow safe housekeeping practices to prevent fires. Continuous
training in fire safe procedures, regular inspection of work site areas, and close
supervision of employees job performance are the primary requirements of a
successful fire prevention program.
Prior to Mobilization the Following Considerations Must be Established:
Severity and nature of fire risk and identify Fire hazards, causes of fires, and
safeguards required to prevent fire.
Area safety requirements and understand the basic principles of fire safety and
procedures for Reporting fire, fire fighting, and evacuating the work site.
Training of personnel, understand the use and operation of fire protection equipment
and systems.
No activity involving hot wok is allowed and use only non-spark tools.
Location(s) of activities. Conduct regular periodic inspections to ensure that work
areas remain in a fire safe condition.
Number of motorized equipment.
All equipment shall be equipped with spark arrestor.
Quantity and size of storage yards or buildings.
Quantity and size of flammable storage areas.
Minimum quantity and types of fire extinguisher equipment, i.e. portable 9 Kg
capacity ABC Dry chemical fire extinguishers, fire water barrels, fire buckets, etc.

of fire extinguishing equipment required shall be determined

6.5 Reporting a Fire:


All fires including those extinguished by KAFOU shall be reported to (MARAFIQ) Site
Representative with a preliminary report followed by a detailed report. All fire incidents
shall be reported and documented entitled Report of Fire Emergency or False Alarm.

6.6 Fire Extinguisher and Training:


KAFOU shall ensure that all site personnel on the project are trained in the proper usage
of fire extinguishers.

6.7 Fire extinguisher:


There are three type of fire extinguishers normally found at the construction site:
Water type, Carbon dioxide and dry chemical powder (DCP) types. All employees
should be aware of the firefighting equipment available on site and be familiar on how
to use it.

Water type Fire Extinguisher:


Class A fire: Pressurized water system to be used only on a fire involving ordinary
combustible materials such as wood, paper, clothing, and some rubber and plastic
materials.

Carbon Dioxide Type Extinguisher:


Class C fire: The Carbon Dioxide (CO2) type extinguisher is normally used for
controlling electrical fires. Fire involving energized electrical equipment, such as
motors, switch gears, circuits and control panels are to be extinguished using carbon
dioxide or DCP fire extinguisher.
CAUTION: A carbon dioxide Extinguisher should never use in closed area where people
are present, because the gas displaces oxygen from the immediate environment. When
oxygen level in the environment is reduced sufficiently to put out a fire, the oxygen level
is also incapable of supporting human life.

6.8 Electrical Installation & Equipment:


All material and equipment used in temporary electrical installations shall meet (SEC)
standards, including (NARAFIQ) Construction Safety Manual. KAFOU shall ensure that
Every piece of electrical equipment is a potential source of electric shock Even through an
electric shock is too small to cause injury it may trigger an involuntary reaction that results
in physical injury for example after touching a live wire, a person may lose his balance
and fall off a ladder or scaffolding.
Obviously, design, layout, installation, and preventive maintenance programs can
minimize such incidents on live or hot electrical equipment.
An effective electrical safety policy coupled with an employee training and hazard
awareness program can further prevent electrical shocks.
To minimize electrical related injuries, it is important to follow basic safety
precautions to avoid such an accident.
All temporary electrical installations and equipment on the project will conform to
(MARAFIQ) Std. and National Electrical Code (NEC).
Extension cords will be kept clear of walkways, water pools, and other locations
where they may be exposed to damage, create trip hazards, or set up other hazardous
conditions.
Color tagging, following inspection by a qualified electrician, of hand held power
tools, GFCI units, all electrical equipment/boards, outlets, extension cords, table
saws, appliances, and etc. is required monthly. It is understood a color scheme has
been provided and this requirement is subject to change.

Minimum safety measures that KAFOU will ensure on the project include but not limited
to the following;
Distribution of electrical power on construction site is different from permanent
installation. Distribution boards and supply cable must be properly installed and
protected from accidental damage.
All materials used shall be of an approved type and in good condition.
All portable electric tools must be well maintained and provided 3 core grounded
cable and proper plugs. Approved double insulated or all insulated tools may be used
without a ground wire. Double insulation does not protect against defects in the
cords, plug, and receptacle. Continuous inspection and maintenance are required.

When cords are not in use, they should be disconnected and neatly stored. Cords
should be stored in a dry room they should not be exposed to excessively cold or hot
temperatures.
A visual electrical inspection should be made of the extension cord each time it is
used. Cords with cracked or worn insulation or damaged ends should be removed
from service immediately. Cords with the grounding prong missing or cut off must
also be removed from service.
All connections shall be made with approved and proper connectors. When cords
must cross passageway, whether vehicular or personnel, cords should be protected
and identified with appropriate warning. Cords should be used in this manner only
for temporary or emergency use.
Once in every month, the Safety Supervisor and Equipment Foreman shall inspect
equipment and tools. It remove damaged tools, and from the site for repair or scrap.
Once every week the Electrical Supervisor shall check that all electrical equipment is
adequately grounded.
All maintenance work identified from the electrical inspections shall be rectified by
an adequately trained electrician and supervised by the Electrical supervisor.
Ground Fault Circuit Interrupter (GFCI)/ELCB will be used on all electrical circuits
including portable power electrical supplies.
Only qualified electricians will be allowed to perform repair or carry out
maintenance of electrical tools and equipment. When performing maintenance on
electrical system, lockout and tag system must be used.
Keep adequate clearance between electrical line and object being moved or handled.
Machinery guarding is to protect against electric shock and to prevent body contact
with a grounded surface.

6.9 Grounding for Protection of Workers


Grounding cables shall be connected to the ground first, and then the equipment.
Grounding cables shall be installed and removed using hot sticks or insulated gloves.
Grounding cable shall first be disconnected from the line or equipment and then be
disconnected from ground.
Grounding cable shall be at, or as close as practicable to, the work locations.
Grounding cable and clamps shall be capable of conducting the anticipated fault current
and shall have a minimum size of No.2 AWG standard copper.

6.10 Work Permit:

KAFOU shall obtain a WORK Permit (MARAFIQ) for any WORK to be carried out
during each shift in any (MARAFQ) specified "Restricted Area" or as may be required
by the Company Representative. It shall be KAFOU's responsibility to ascertain, in
advance, whether the WORK area designated is a Restricted Area. The Company
Representative can give guidance on Restricted Area locations. KAFOU shall only
provide qualified and certified craft personnel to execute and/or supervise WORK.
WORK Permit courses are provided by the Loss Prevention Department. The certificates
of all (MARAFIQ) contractors shall be approved by the proponent organization
superintendent.
Supervisory personnel or other qualified staff must always be present at the WORK Site
while any WORK is in progress. All WORK Permit procedures shall be carried out by
KAFOU in accordance with (MARAFIQ) Work Permit System. Pressure Testing Safely,
Isolation, Lockout and Use of Hold Tags, and
Restricted Area - Restricted area is a portion of facility where approved work permit is
required before any job can be done and comes under the category of restricted area as
designated by (MARAFIQ).
Authorized issuer - are those employees designated and certified by concerned
(MARAFIQ) Site Division heads to issue and approve work permits in restricted area
under their supervision.
Authorized Receiver - are those craftsmen, who have been certified by the (MARAFIQ)
- Site Division heads to sign and receive work permits

PTW nomination forms for MARAFIQ contractors APR nominees shall be forwarded to

7 BEHAVIORAL COMPLIANCE
KAFOU Safety Supervisor shall monitor all Site working activities to ensure that the
services are performed in conformity with (MARAFIQ) Loss Prevention Policies and the
entire employee including the high management are following the required Behavioral
Compliances. Such as traffic violation, horseplay, and proper usage of PPE. Observations
and inspections are frequently conducted.
Unsafe acts and conditions shall be immediately reported to the relevant supervisor for
correction. Life threatening hazards shall be corrected immediately. Corrective actions for
unsafe conditions shall be identified and tracked until completion, with follow-up to verify
proper implementation.

9 SAFETY INSPECTIONS
KAFOU Safety Supervisor shall monitor all SEC working activities to ensure that the
services are performed in conformity with MARAFIQ Loss Prevention Policies,
principals and practices, and do not imperil any person or property.
KAFOU Safety Supervisor will perform a weekly inspection which will be recorded in
the form of Weekly Site Inspection Checklist signed by all required placed in the site
safety file for review by MARAFIQ Safety Representative as may be required during
shutdown. All inspection and meeting activities will be carried out on a daily basis. This
inspection will be beneficial for all the parties, and KAFOU Project Management and
MARAFIQ Representative shall accompany for the walk through.
The MARAFIQ Safety Representative will visit the site frequently to ensure that the
work is being performed in compliance with MARAFIQ Loss Prevention Policies
findings. Findings of the visit must be received immediate provided to the Corrective
Action Report, or a written justification MARAFIQ Safety Representative by close of
business the following day.

10 INCIDENT REPORTING & INVESTIGATION


We are often able to identify future hazard areas and apply preventive measure through
analysis of previous accident. However effective remedial action can only be applied when
the basic problem is clearly defined by investigation and submission of detailed realistic
report. The evaluation of these facts and the real cause of any accident will assist in
directing our accident prevention activity in the most efficient way.

10.1 REPORTS

An immediate oral report will be made to KAFOU office and the (MARAFIQ)
Representative office in the following cases:
All fatal injuries.
All injuries required medical attention which results in lost time.
Near Misses
All damage to the contractors plant or equipment in excess of SR 10000/-.
All damage to (MARAFIQ) property in any
amount.
All Fires
Damage and near misses to cranes, heavy equipment and personnel.

All accidents shall be reported within 24 hours from the time of the incident occurrence using
the Marafiq Preliminary Incident Report Form and use the Marafiq final Incident
Investagation Report Form after the investigation.
For accidents resulting in KAFOU employee fatality, serious injury to KAFOU
employee(s), or damage to MARAFIQ equipment or property, a written report including
MARAFIQ Form where applicable shall be submitted promptly to the Company
Representative. In addition, MARAFIQ may convene an engineering review or
investigation committee in accordance with the requirements, Notification Requirements
for Incidents (Including Fires); and, Guide for Committees Investigating Major Incidents
and Engineering Reviews of Other Incidents.
Fully detailed written reports of these incidents will be submitted within 24 hours with
copy to the MARAFIQ Site Representative. All other accident will be recorded and
monthly injury and damage reports submitted to MARAFIQ

10.2 INVESTIGATION

The responsibility to investigate all accidents, incident and to submit reports and recommend
corrective action is incumbent on the site Safety Supervisor.
Visit the scene of the incident and examine any plant or equipment for damage or
misuse.
Question witness what happened, who it happened and why it happened.
Question any injured personnel when it is possible.
Ascertain the events that led to the incident and any irregularities in the operations.
Describe the operation that was taking place and the sequence of events which led to the
incident.
Assist the Safety Supervisor to examine any plant or equipment involved in the accident
and to question witness.

Accident investigation techniques are many and all may be successfully employed in
varying circumstance, the overriding factor, however must always be accident investigation
is to determine the cause and preventive requirements not to apportion blame.

11 JOB SAFETY ANALYSIS


Before starting of any work at the (MARAFIQ) job site a site survey will be conducted
by K A F O U Safety Supervisor to identify the potential hazard of the job and the work
needs to be performed to understand the entire hazard at job site. After the survey a
meeting will be conducted with all concern responsible people and a job specific Hazard
Identification Plan will be prepare by KAFOU Safety Supervisor which will be submitted to
(MARAFIQ) Representative for approval before starting of any work on the (MARAFIQ)
Job site

Dynamic Risk Assessment shall be submitted to TAWREED authorized personnel for all
activity having significant risk.

The Hazard will be classified in three different classes:

11.1 Class A Hazard

A condition or practice which likely to cause permanent disability, loss of life or body part
fully or partially or extensive loss of structure, equipments or materials

11.2 Class B Hazard

A condition or practice which likely to cause serious injury or illness or property damage
that is disruptive, but less than Class A.

11.3 Class C Hazard

A condition or practice which likely to cause minor injury or illness or disruptive


property damage.

12 WORK PERMIT SYSTEM


The HSE Procedure establishes the requirements and process to be considered within the
work permit system, to perform relevant project activities in a safe manner.

Permit must be issued:-


Hot Work:- Any work that creates enough heat to ignite flammable Gas-Air mixtures of
flammable materials

Oxygen deficient Atmosphere:- An Atmosphere with less than 19.5% by volume


oxygen at level .The oxygen we normally breathe at lower elevations is about 20.8%.

Oxygen Enriched Atmosphere:- An atmosphere containing more than 23.5% oxygen


by volume . Oxygen-rich atmospheres expand the range of flammability, lower the
ignition energy, and will cause combustible materials to burn violently when ignited.

Permit issuer:- An employee who has been designated by management shall be trained,
qualified and certified to issue a work permit.

Permit receiver:-An employee who has been designated by management shall be


trained, qualified and certified to receive a work permit.

13 SCAFFOLDING
Any temporary elevated structure /platform and its supporting structure used for supporting
workmen or materials/both.

A tag showing clearly whether the scaffolding has been inspected as per MARAFIQ
HSE Scaffolding requirements and is ready to use.
A Ladder consisting of a pair of side rails connected together by cleats, rungs or Steps.
Shall assure scaffolding above 37 meters plans are reviewed and approved by a qualified
engineer assigned.
To prepare JSA Prior to erection.
To ensure proper work permit is available to perform the activities related to erection,
modification or Dismantling
To perform pre-task toll boxes talk with scaffolders to relay information and instruction
To ensure work permit copies and JSA are available at work site.
To ensure qualified scaffolders are assigned for the task.
To ensure a daily inspection and periodic inspection within 7 days by qualified scaffolding
inspector.
To ensure qualified scaffolders are wearing full body harness belt with double shock
absorber lanyards and attach to an anchorage point at all times (100% tie off).
To conduct daily routine inspection and to notify any observation such as unsafe
scaffolding or scaffolding that is not complying with the requirements of this procedure, to
the scaffolding supervisor, and to change the tag to RED or take any actions as needed
To conduct inspection on modified scaffolding and tag accordingly.
To wear the appropriate personnel protective equipment at all the times.
To wear the full body safety harness and fasten to anchorage point at all times of
scaffolding erection Modification and dismantling process.
For working at heights above 1.8 meters, all scaffolds shall be installed with proper
guardrails.
All personnel who work at more than 1.8 meters (6 feet) height shall wear a full body
harness belt attached to Proper anchorage point or a fall protection device
Loose items shall not be kept on the scaffolding platform

14 HAND & POWER TOOLS


.Only pneumatic power and hand tools driven by compressed air shall be used inside the gas
station and used of electrically power driven tools is not allowed.
Shall ensure proper training of the employee on the proper use, care and inspection of
portable hand and power tools.
Shall implement proper periodically inspection programs for portable hand and power
tools.
Correct tool shall be selected for the job intended. Faulty tools shall not be used
Tools should be stored in designated storage areas.
Tools shall be regularly cleaned and, where necessary, lightly oiled as a protection
against corrosion.
Every portable hand tool shall be connected to ground fault interruption or earth leakage
circuit breaker (ELCB)
Electrical operated tools shall be grounded with a ground conductor or shall be double
insulated.
Tools shall not be altered or repaired without authorization
Cutting tools shall be kept sharp.
Files without a handle should not be used.
Files should not be used as a hammer or as a pry
Appropriate protective equipment shall be worn while using chisel.
Electrical drill shall be unplugged before changing the bit
Appropriate eye protection shall be worn during use of grinding wheels
The tongue guard should be maintained within 1/4inch of the grinding wheel
The soldering iron shall not be used as a screw driver or pry.

15 LIVE ELECTRICAL CABLE HANDLING


Purpose:- The HSE procedure establishes the minimum basic requirements for basic
requirements for safe performance of Live (HOT) Electrical work within the project.

Electrical Hazards:-A Recognizable, dangerous condition involving electrical equipment


or systems, which could be the cause of injury or fatality if proper precautions or actions
are not taken.
Electricity: - A form of energy that can be transformed in to the mechanical, chemical,
light or sound energy.
Electrically hazardous tasks are subdivided into four classes: hot work, voltage testing,
proximity work, and other hazardous work.
All live electrical parts in excess of 50 volts AC to ground or 100 volts DC Positive(+)
to negative (-), to which an employee may be exposed shall be de-energized and locked
and tagged out before the employee works on or near them ( as per EPM-SS- 03-04-
3),Hazardous Energy Control Lock Out /Tag Out procedure.
Only qualified persons shall work on electric circuit parts or equipment that have not
been de-energized and / or perform high voltage test. The contractor shall appoint in
writing one or more suitably qualified electricians. Such persons shall be capable of
working safely on energized circuits and shall be well informed of the proper use of
precautionary techniques, personal protective equipment, insulating and shielding
materials and insulating tools.
Safe working distance from energized Apparatus
The four types of Hazards associated with work on or near energized un-insulated
electrical equipment are the following.
Shock and burn due to contact.
Electrical flash burn from arcs.
Blast injury resulting from electrical faults
Up set or shut down of an operating process.

Employee shall be provided with, and shall use, electrical protective equipment rated for
the voltage involved and that is appropriate for the specific parts of the body to be
protected for the work to be performed.
Protective equipment shall be maintained in a safe, reliable condition and shall be
periodically inspected or tested. (As per OSHA 1910.37).
Employee shall wear nonconductive (not the ordinary plastic safety helmets) head
protection wherever there is a danger of head injury from electric shock or burns due to
contact with exposed energized parts.
Employee shall wear protective equipment for the eyes, face, and body wherever there is
danger injury caused from electric arcs, flashes or from flying objects resulting from
electrical explosion.
Ladders, Harness, hooks, ropes, hand lines, etc used near exposed energized parts shall
be conductive.

Conductive articles of jewelry and clothing such as watch bands, bracelets, ring, key
chains, necklace, metalized aprons and cloth with conductive thread or metal headgear
shall not be worn when working close to electrical circuits.
All protective devices, which are placed on conductors or apparatus to protect employee,
shall be left in place until all work is completed.
Adequate hazard warning signage (risk of electric shock, no smoking, hearing protection

16 HAZARDOUS ENERGY CONTROL (LOTO)


Lock Out Tag Out or Isolation shall be performed by TAWREED authorized personnel.

Hazardous Energy control Board: A key controlled board that contains the master
control HASP documentation and Keys to all Hazardous energy control lock supporting
work.
Hazardous energy control (LOTO) coordinator:-The designated project senior
Representative, assigned with the responsibility for the implementation of this
Hazardous energy control (LOTO) procedure. Hazardous energy control coordinator
shall maintain an inventory of locks, Hasp, Tags and supporting documents.
Hazardous Energy control survey: A summary of all hazardous energy sources and
associated hazardous energy isolation devices that must be identified and isolated in
order to achieve a zero energy state prior to the performance of any specified
maintenance, Inspection, or construction activity.
Hazardous Energy isolation device: A Mechanical device that physically prevents the
inadvertent transmission or release of hazardous energy i.e. electrical disconnect switch,
circuit breaker, slip blind flange mechanical block, etc.

All isolation to be applied in system or requirement shall conform MARAFIQ OSHM no.
11 isolation procedure.

16.1 Responsibilities
Shall ensure an appropriate Hazardous energy control (LOTO) system is established.
Shall ensure that hazardous Energy Control (LOTO) system training programs are
established and maintained.
Auditing /Monitoring /reviewing of the Hazardous Energy Control (LOTO)System
Shall establish an appropriate Hazardous Energy Control (LOTO) System for the
project.
Shall assure this procedure is implemented throughout project execution.
Shall establish auditing programs to verify contractor compliance with Hazardous
energy control (LOTO) procedure.

All Electrical Energy Lockouts shall include assurance of a complete physical break in
the power conductors, either by visual inspection or by testing for absence of voltage
with an acceptable voltage detecting device or both.

16.1.1 LOTO Removal:-

Before lock out and or Tag out devices are removed and the energy is restored
authorized personnel perform the following.
Inspect the work area to assure that tools and equipment have been removed.
Ensure that the machine or equipment components are operationally intact.
Ensure that all personnel are safely positioned or removed from the area.
Notify the affected personnel that the isolation devices have been removed.
Lockout &or Tag out devices shall be removed from each isolating device only by
authorized personnel who installed them.

16.2 Group Lock Boxes

A group loto box may be used when maintenance or servicing is performed by a crew,
department, or other group that requires an excessive amount of isolation devices, under
the control of the crew coordinators.

1 PERSONAL PROTECTIVE EQUIPMENT


KAFOU Safety Supervisor shall make sure the usage of proper use of Personal
Protective Equipment (PPE). Use PPE which can protect a person from Head to toe. To
develop an effective PPE program, all the employees shall be familiar with required
standards and requirements.
Each Supervisor shall verify the following basic PPE are provided and worn by all the
workers at job site before proceeding to any job site.
All PPE shall be inspected on project site prior to use. Defective or Damaged PPE shall
be immediately removed from site and discarded.
Employee shall perform a thorough inspection of his PPE prior to use.
For specific Products (Chemicals, Liquid Gases etc).Corresponding material safety data
sheet shall be followed for PPE Selection.
PPE requirement shall be addressed by means of the corresponding JSA.

17.2 HARD HAT:


Safety hats or helmets are rigid head gear made of materials that are designed to protect
the head from impact, flying particles, electric shock, etc.
Head Protection signs shall be posted at visible places.
User should visually inspect hard hats for components (shell, suspension, headbands,
sweatbands etc.) prior to use.
Every employee shall be provided hard hats and must wear them all the time during
working hours while on the job site. Metal hard hats and bump or laceration caps are
prohibited. This requirement includes office personnel and visitors to the work site.
The suspension cradle is necessary as it provides the impact distribution quality of the
helmet, so it is important that it be properly adjusted to wearers head.
In case of special requirement like welding, grindings or cutting a face shield shall be
used by the worker and a full body harness will be used by the work if the work on
ladder or scaffoldings as required.
All requirements on proper use and selection of PPE will be in compliance with
(MARAFIQ) Std.

17.3 EYE PROTECTION:

In all project areas appropriate eye & face protection shall be worn by all personnel and
or visitors.
The colored safety glasses shall not be permitted to be worn at night, dark shady areas or
indoors.
Contact lenses shall not be permitted to be worn inside the project.
Safety glasses protect against flying objects. The use of approved eye protection safety
glass with hard side shields is required. All employees shall be issued with safety glasses
at the start of the project. Goggles, face shields etc., shall be issued from the store room.
Goggles provide additional protection against dust, chemical, or harmful light.
Face shields are an additional safeguard that are worn over protective eye wear.

17.4 FOOT PROTECTION

Foot protection signs shall be posted visible places.


In project areas, appropriate foot protection made of hard leather with slip- resistant and
puncture resistant sole must be worn by all personnel and visitors.
All employees must wear approved work boots or shoes in good condition. Approved
foot-ware is steel-toed shoes. Soft shoes, such as tennis shoes and sandals are not
permitted. Boots/shoes should also protect the ankle

17.5 EAR PLUG & EAR MUFF:


Ear plugs must be worn when personnel are involved in or working close to high noise
operations such as air hose cleaning, concrete breaking or cutting or chipping, etc. or
when performing activities in areas identified to be "High Noise Areas" or close to high
noise producing equipment such as compressors, etc.
Hearing protection safety signs shall be posted at visible places where the noise level
equal or higher than 85 dba.
Provision for the use of double hearing protection (Earmuffs over earplugs) to reduce
high noise levels to less than 85 dba are acceptable.
In all project areas with high noise levels (equal or higher than 85 dba), appropriate
hearing protection shall be worn by all concerned personnel and visitors.
It is imperative to select the appropriate device on the basis of comfort, but more
importantly, on the noise attenuation and protection they provide.

17.6 HAND GLOVES


For all activities requiring hand protection from sharp or rough objects which include:
cable pulling, hand digging, rigging, lifting operations or piping work. Gloves will not
be used in situations where the individual is exposed to "pinch points or working close to
moving machinery parts or gears or objects as they can be caught and trap the hand
before it can be removed from the glove. Welding Leathers (gloves and leggings) will be
provided for welders on site.
Gloves are like a second layer of skin, as they provide protection against chemical,
rough surfaces, sharp objects and temperature extremes. However, remember that gloves
should not be worn when working on or with moving machinery as they can get caught
and trap the hand.

17.7 FACE SHIELDS, WELDING HELMETS

Face shield shall be worn when possibility of flying particles, sprays of hazardous
liquids, splashes of molten metals and hot solutions can affect the workers.
Welding helmets or hand shields shall be used during all arc welding or oxygen cutting
operations.
Welding goggles shall be used to protect welding helpers from visible and infrared
radiations.
Chemical goggles shall be worn when there are possibilities that the chemicals or
harmful liquids affect the workers.
User shall physically inspect eye face protection equipment for such things at light leaks,
lens chips and structural integrity.

17.8 Respiratory Protection:

Where respiratory are necessary to protect the health of the employee. The contractor
shall establish and implement a written respiratory protection program.

18 EMERGENCY RESPONSE PROCEDURE


KAFOU must familiarize their personnel with the Disaster Management Plan for the
plant and the area in which the job is located. KAFOU must know exactly what to do
when the emergency evacuation or stop works alarms are sounded and do it without
delay. The KAFOU Safety Supervisor shall follow the following guideline in case of any
emergency:
When the emergency alarm sounded for any reason all the KAFOU worker should stop
the work immediately, make it safe and proceed to nearest assembly area in a timely
manner.
In case of any emergency situation like serious personnel injury, fire or critical damage
to operating equipment help may be obtained by contacting nearest (MARAFIQ) Security
Control Center.
MARAFIQ Emergency No. 341-99-11 shall be posted on site
In case there is no telephone available on job site a radio user can send a message to the
other radio user to contact (MARAFIQ) Security Control Center.
While transmitting the message to the (MARAFIQ) Security Control Center you should
ensure that you are identifying yourself by giving the following details:

18.1 Gas Release EMERGENCY RESPONSE PROCEDURE


If there is a reason to suspect of Gas release through which you are working all the
KAFOU employee do the following things:
Dont try to investigate, leave the area.
Check the wind direction by looking at the wind socks.
Walk in cross wind direction towards the nearest assembly area away from the affected
area.
Inform the plant operation about the situation after coming to safe assembly area.

18.2 Emergency Incident


An unexpected and undesirable event which must be dealt with at once. This may
include any kind of event that may result in injury of personnel or equipment damage,
involving fire and explosion, combustible & chemical liquid spills, flammable or toxic
gas releases, traffic incidents. A civil protest demonstrations will also be considered
part of An Emergency Incident.

18.3 Major Incident


A Major Incident involves serious property damage, fatal or personnel injury, and may
require assistance of external emergency response teams to control the impact of an
explosion, demolition, severe or life threatening injury, or illness to one or more persons.

18.4 Minor Incident


A Minor Incident involves only internal emergency response to control the impact (such
as a small fire, small flange leak, first aid injury etc.).

19 FIRST AID FACILITIES


KAFOU shall provide and maintain adequate first-aid facilities at the project site
in accordance with the most rigorous requirements of:
The Saudi Arab Labor Law as revised from time to time.
First Aid/CPR Training and First Aid Kits Remote Areas.
Minimum Standard Requirements and Recommendations for Contract Industrial Clinics.
If KAFOU employs 50 or more workmen at a WORK Site (including all personnel at
the WORK Site employed by KAFOU Subcontractors and involved in performance of the
WORK):
KAFOU shall provide and maintain a full-time, dedicated emergency vehicle
(ambulance), properly marked and supplied (including stretcher), to transport injured
personnel to the nearest healthcare facility.

The minimum requirements that the first aid facility will contain include

A desk, hot & cold water, wash basin, examining table, air conditioner, adequate
lighting, splints for bone fractures, bandages and sterile wash water in clinic and dust
tight medical supply cabinet.
First aid supplies will be kept available in a cabinet designed only for those supplies.
The cabinet will be placed under the charge of the qualified nurse, who will ensure the
cabinet is well stocked at all times.
A suitable type of stretcher shall be available on project site at all times.
The ambulance shall be equipped, as minimum with the following supplies:
Stretcher and blankets
Portable oxygen
Splints for bone fractures
Bandages
Sterile wash water

20 SUBCONTRACTOR MANAGEMENT
All the sub contractors will be selected and controlled from (MARAFIQ) approved
contractor list and KAFOU approved contractor list. Preference will be given for the
contractors those are having good track records in the compliance of safety requirements
as per (MARAFIQ) Std. and Saudi Government Law.
Before signing any contract KAFOU will request the sub contactor to submit their
Safety Plan to verify their records. And KAFOU will check if the subcontractors have
(MARAFIQ) certified personnel.

20.1 INJURY REPORTING AND RECORDKEEPING

We are often able to identify future hazard areas and apply preventive measure through
analysis of previous injury. However effective remedial action can only be applied when
the basic problem is clearly defined by submission of detailed realistic report. The
evaluation of these facts and the real cause of any injury will assist in directing our
accident prevention activity in the most efficient way

21.1 Control Measures:

Traffic management plan to be in place to cover movements of pedestrian and site


traffic. TMP to be strictly adhered to
Adequate advance warning signage and local signage will be erected and maintained in
order to alert public traffic to the construction works.
A full time security / gateman operative will be stationed at site entrance / exit during

22.1 Control Measures:


Access to site (both pedestrian and vehicular) can only occur at designated entrance
point. Entrance (and exit) are manned during working hours by Security / Gateman

24.1 Control Measures:


Hazardous materials, practices or substances must be eliminated or
substituted for less hazardous alternatives.
Where usage of hazardous substances is justified, all recommendations or
MSD sheets must be complied with and management must monitor such

25.1 Control Measures:


Risk assessment required for all work at height. Method statements to be produced where
risks are deemed to be greater than LOW
Collective fall preventative measures be put into place where possible and individual
measures only as a measure of last resort
Inspections and Sign-offs must be completed for all work at height equipment and working
platforms, eg. Scaffolds, ladders, guardrails, walkways, decks, fall arrest equip etc.

26.1 Control Measures:


Hearing protection required for any noise producing activity over 80dB (minimum).
As far as is reasonably practical, new machinery will be fitted with silencers and plant cabs
insulated to quieter the source of noise.
Workers must be informed of the hazard and the risks involved and noise exclusion zones
established and maintained following risk assessment.
Work must be planned so that minimum numbers of workers are exposed to noise.
Ear protection must be changed on a regular basis to maintain safety and health standards.
Operatives must be shown how to use ear protection correctly and toolbox talks will be
provided as necessary.
Where ear muffs cannot be worn e.g., in a JCB rock breaker, where vibration would render
muffs impractical, ear plugs must be worn by operators. All hearing protection is subject to
risk assessment.
Work must be organized so as to cause minimum disruption to the operations of the
neighboring premises & to general public. Protection to existing buildings to be used where
required.

Live services must be cordoned off form safe areas.


A competent person will operate equipment to locate existing underground services.
All underground services likely to cause a hazard will be identified prior to
commencement of excavation works.
Slit trenches/exploratory works to be carried out on site to identify existing services &

The location of buried services must be clearly signed caution buried services and

Sufficient water points with salt / glucose packs to be positioned throughout site
Operatives to be given regular tool box talks on how avoid risks associated with hot
weather work and the requirement to maintain fluid intake and wearing of appropriate
PPE
Appropriate PPE Operatives to wear clothing which covers arms, legs and head
(Coveralls ,Face Shield and Helmet)

Appendix 2: Emergency Plans

REPORTS
An immediate oral report will be made to K A F O U office and the SEC
Representative office in the following cases:
All fatal injuries.
All injuries required medical attention which results in lost time.
Fully detailed written reports of these injuries will be submitted within 24 hours with
copy to the MARAFIQ Site Representative. All other injury will be recorded and
monthly injury reports submitted to MARAFIQ.
