LOAD - OUT PLAN/PROCEDURE
1.0 INTRODUCTION
CLIENT), acting on its behalf as Technical Partner and on behalf of its partners
CONTRACTOR A, and CONTRACTOR B, intend to develop the Akepo Field at
OML-90.

The existing Akepo-1 well is located at Lat N 05 deg 15 18.837 Long E 05 deg
16 34.894. This exploratory well has recently been re-entered, tested and
completed. At the present level of completion, the well has been installed with a
free standing 30-inch conductor, with a Christmas tree at the top end. There is
no other structure at the well location presently.

CONTRACTOR with its partner CONTRACTOR C has been awarded a contract


to undertake Engineering, Procurement and Construction (EPC) for the design,
materials procurement, fabrication / construction, and installation of the Akepo
wellhead deck structure, process piping and its pipelines to tie-in at the Crude
Processing Facility (CPF) installed within NAOC Beneboye Flowstation perimeter.

The Plan in conjunction with CONTRACTORs HSE Plan shall ensure that Loss
Prevention is maximized during the Load-Out phase of the project.

The Load-Out activities shall be performed from Onne CONTRACTOR Cs pipe


storage to CONTRACTORs Worji Base Yard, where all materials and equipment
shall be assembled and subsequently loaded out from to project sites via the
jetty to the Work/Lay Barge at the open Calabar River using the Utility barges
and Trucks.

Particular emphasis has been made to ensure harmonious Security network


within, around, the load-out jetty, Mid River, and en-route Worji, Warri and
Offshore.

2.0 SCOPE
2.1 This procedure defines the minimum requirement, methods to be
applied and equipment to be used during the Load-Out and Sea-
Fastening of the Equipment, Facilities, and Pre-fabrication on
transportation Truck, Vessel/Craft using Cranes.
2.2 CONTRACTOR shall use Cranes, Trucks, and Flat top barges to Load-
Out all fabricated structures.

3.0 DEFINITIONS
3.1 Load-out
Load-out is the process of moving and materials and/or structures from
Onne CONTRACTOR Cs pipe storage to CONTRACTORs Worji Base
Yard, Warri and Offshore
3.2 Sea-Fastening
Sea-Fastening is the process of installation of tie-down braces in such a
way as to minimize damage to the Linepipes, Equipment or Structure on
the vehicle or water craft to provide a safe tie-down system during Load-
out.
4.0 REFERENCES
The following CONTRACTOR C approved Specifications, Industry Codes,
Standards, & Recommended Practices shall be followed during the Load-out
operations of the offshore structures.

4.1 CONTRACTOR C Job Specifications, Project AFC Drawings, and


Contract.
4.2 American Petroleum Institute API RP 2A; Recommended Practice
for
Planning, Designing, and Constructing Offshore Petroleum
Platforms.
4.3 American Welding Society AWS D1.1 Steel; Structural Welding
Code.
4.4 VESLs QA/QC Plan and other Procedures.

5.0 RECORDS & DOCUMENTATION


During the load-out and sea-fastening, all daily reports, logs, reports, etc. shall
be prepared, compiled, and retained by VESL.

These documents shall record, also, any deviations from the original plan, all un-
usual environmental conditions that may occur.

At the completion of the Load-out Operation, CONTRACTOR,s QA/QC Department


shall deliver to the CONTRACTOR C these documents in a form suitable for
permanent record.

6.0 RESOURCES
As a minimum requirement, the following list of manpower, tools and equipment
shall be provided by VESL for execution of this works.

7.0 PERSONNEL LIST & RESPONSIBILITIES


7.1 Personnel List
The personnel listed below are essential to the safe and successful
performance of the operation.
They shall attend the briefing meeting prior to the beginning of the
operation.

7.2 RESPONSIBILITIES
The Responsibilities of the activity personnel are described here below:
7.2.1 Project Manager.
The Project Manager shall ensure that all activities are carried out in
accordance with CONTRACTORs Quality, CASHES and
CONTRACTOR C approved Standard construction specification and
other applicable codes and standards, reporting to the Project
Director.
However, he shall be directly responsible to ensure:
a) The adoption of safe working procedures including safe place of
work.
b) The establishment of CASHES Management system
implementation program.
c) Auditing of programme and plans to measure progress and
identification of deficiencies.
d) Adequate training of personnel to improve their safety at work.
e) That adequate Liaison with Corporate CASHES Management for
specialist advice is carried out.
f) Selected Sub-contractors are able to meet company standards.
g) Those deficiencies in equipment, standards, operating
procedures or training are corrected.
h) Promotion of a high degree of safety and environmental
awareness by personal example.
i) All accidents are reported and investigated according to company
procedures and CONTRACTOR C requirements.

7.2.2 Construction Superintendent


Construction Superintendent shall perform the following duties:
Draw up a sound Load-Out schedule consistent with the over all
philosophy for the project construction execution plan.
Liaise with the Project Manager and logistic team to reduce
interface problems.
Ensure the mobilization team is well orientated on the relevant
Procedures and Safety Precautions that must be followed.
Co-ordination and control of the mobilization operation to ensure
continuous workflow and high productivity
Monitor and review progress made and revise activity schedule to
recover lost times if any.
Draw up exhaustive equipment, materials, and tools checklist to
be mobilized for site works.

7.2.3 Construction Supervisor


The Construction Supervisor shall perform the following duties:
- Follow up the loading activity to ensure that all the materials,
equipment and tools loaded are according to schedule.
Monitor progress and in consultation with Construction
Superintendent draw up a revised program to keep the
mobilization operation on course.
Keep the mobilization team abreast of the materials handling and
personnel safety as per work instructions.

7.2.4 Logistics Manager


The Logistics manager shall be responsible for the overall Logistics
activities:
He shall execute the following duties:
Interface with the CONTRACTOR C's Rep on Logistics issues
Ensure that this procedure as approved by CONTRACTOR C is
made available on site for implementation
Ensure that necessary PPEs are provided and utilized on site
Ensure equipment/tools and load-out materials are on site on
time and at right quantities
Supervise the deployment of various categories of workers to
different positions of the work area.
Monitor the activities at different points and stages.
Promptly report progress/difficulties to the Project Manager.
Brief the Logistics Superintendent on all items of work to be
carried out.
Shall ensure that all safety measures are implemented.

7.2.5 Logistics Superintendent


The Logistics Superintendent shall report directly to the Logistics
Manager.
He shall be responsible for the following duties:-
Co-ordinate all load-out and Sea-Fasting activities.
Brief all Foremen on all items of work to be carried out.
Ensure that all safety measures are implemented.

7.2.6 Lifting Supervisor.


The Lifting Supervisor shall report directly to the Logistics
Superintendent.
He shall be responsible for the following duties:-
Ensure all necessary equipment/tools are on site in the right
quantity and capacity.
Assemble the required equipment on site.
Test and certify all equipment prior to commencement of the
operation.
Initiate, monitor and record the operation.
Ensure that all items of work -load-out and Sea-Fasting- are
carried out.
Prepare report of the operation.
Shall ensure that all safety measures are implemented.

7.2.7 CASHES Officer.


The CASHES Officer shall reports to the CASHES Manager.
He shall be responsible for the following duties:-
Obtaining the appropriate permit to work from CONTRACTOR C,
Ensure that all necessary safety requirements for a successful job
are met.
Manage the health and safety functions of the activity.
Manage the safety engineering function of the activity.
Support and advise the project Team on health and safety of
personnel.
Introduce Safety notices, bulletins and posters at site.
Contribute to the planning and implementation of risk
assessment and control measures and contribute to the
implementation of selected risk control and mitigation measures.
Ensure that the Emergency Response Procedure is effectively
drilled into the workforce.
Review all accidents and incidents report and remedial actions.
Liaise with CONTRACTOR C Safety Representatives.
Liaise with the CASHES Manager.
7.2.8 QA/QC Engineer
The QA/QC Engineer shall:
Ensure that the contents of this QA/QC Plan/Procedure are well
understood and adhered to by all project personnel.
Verify the implementation of this QA/QC Plan/Procedure and
evaluate its adequacy, by means of monitoring and auditing the
work performance in respect to the established quality
objectives.
Manage the Quality engineering function of the activity.
Support and advise the project Team on quality of work.

8.0 LOAD-OUT PLAN


CONTRACTORshall be responsible for the following:-
The Load-Out of the Line pipes, fabricated Structures, Piping Spools
and shipped loose items onto Transportation Trucks, Barge or Vessel.
Providing and installing Lifting Slings and Rigging Platforms for Load-
Out.
Ballast calculation and ballasting the transportation Barge or Vessel
as required for Load-Out.
Provision and pre-installation of Lifting Slings and Rigging Platforms
for all structural items be by.
Sea-fastening and Tie-down design shall be the responsibility of
CONTRACTOR. Transportation analysis shall also be the responsibility
of CONTRACTOR.
Ballasting the Transportation Barge or Vessel as required for
transportation. CONTRACTOR shall provide the Barge or Vessel
Ballasting Plan.
A comprehensive List shall be compiled for all ship loose items on the
Trucks, and Barges. This List shall also act as a checklist to ensure
that all required items have been placed on the Trucks, and/or Barge.
The ship-loose List shall contain the following information:
- Structure Description.
- Quantity.
- Location on Truck, or barge (boxed or otherwise).

8.1 Load-Out Preparations


VESL shall be responsible for the Load-out & Sea-Fastening Operations
and shall prepare for the steps as follows:-
8.1.1 Prepare Loaded out items list (by Construction Superintendent).
8.1.2 Make the necessary documentation, Procedures, Manuals
available for CONTRACTOR C, Other Contractors, Third Party,
and Work Force.
8.1.3 Prepare the load-out arrangement in the Fabrication Yard,
including clean way for Trailers.
8.1.4 Preparation of Truck, and/or Barge Deck for Cargo.
8.1.5 The Transportation/Installation Contractor(s) shall make
available to Fabrication Supervisor all necessary Logistics
spread that meets all requirements for Trip & Tow Certification.
Also, to provide the Logistics Surveyor.
8.1.6 All mooring hardware shall be ready at the quay.
8.1.7 The barge ballasting system (pumps, headers, valves, hoses,
etc.) must be capable of compensating the changes in tide, and
barge load (weight) application.

8.2 Load Out Sequence


8.2.1 Plant and Equipment
After pre-mobilsation inspection and certification of all Plant and
Equipment they shall be moved from their various locations to the
Load-Out yard at WORJI CONTRACTORs Base Yard, prior to transfer to
Work Sites and Work/Lay Barge.

8.2.2 Prefabricated Items


All prefabricated Structural Members, Linepipes, and Piping Spools shall
be properly tagged and arranged according to their specific work
areas/scope at WORJI Base Yard prior to transfer to Work Sites and the
Work/Lay Barge.

8.2.3 Free Issue Materials & Equipment


All CONTRACTOR C free issue Items shall be moved from Kidney Island
warehouse to VESLs WORJI Yard/Jetty using Carnes, Trailer/Trucks, for
subsequent transfer to Work Sites and the Work/Lay Barge

8.2.4 Personnel
All personnel shall be transported to site by CONTRACTOR C air
operations from Port Harcourt.

Movement between the Jetty and Work/Lay Barge shall be by personnel


carrier boat escorted by the Security boat.

8.2.5 Rigging & Lifting


8.2.5.1 Lift Plans
Clear and concise Plans for any lift in the Fabrication Yard in
course of fabrication and erection shall be submitted to the
Project Manager for review and approval prior to any lifting
operation

5.2 Weight of Structures


During the course of Fabrication activities, the assigned
Project Engineer shall monitor the weights of the individual
structures. These weights shall be compiled from the
theoretical weights of each individual section. This
information shall be analyzed prior to the commencement of
any lift.

8.2.5.3 Certification of Lifting Equipment & Slings


Lifting equipment shall be load-tested to confirm the lifting
capabilities of such equipment. All slings, shackles to be
utilized on lifts shall also be Certified and accompanied by
Certification Documents.

8.2.5.4 Inspection of Rigging Installation


After rigging has been completed on any unit, the designated
QC Inspector shall ensure that all rigging has been
implemented in accordance with the provision of the pre-
approved Plan and Job Safety Analysis, and that all items are
properly installed.
8.2.5.5 Lifting Tackle Requirement
Lifting tackle shall meet the following requirements:-
8.2.5.1.1 Minimum Factor of Safety for slings and
shackles shall be 4.0
8.2.5.1.2 Attachment of each sling to its lift point shall
be with a shackle and a pin to a pad eye. Only
one sling shall be connected to any one
shackle.
8.2.5.1.3 Slings in a loaded condition shall not bear
against any part of the component being lifted.
8.2.5.1.4 Slings intended for use on lifts exceeding
100 Short Tons shall have a history of their past
usage. When required, an Independent Agency
shall certify that each sling is capable of
handling the forces anticipated during each lift.
8.2.5.1.5 Slings intended for use on lifts not
exceeding 500 Short Tons, need not be Re-
Certified for the Work, if these have been
certified within one (1) year of the date of their
planned use.
8.2.5.1.6 The Certification Documents shall include
the measured length of the sling from the
inside of one eye to the inside of the other eye.
Length shall be measure with the sling under
axial tension of 5% of its Safe Working Load.

9.0 LOAD TRANSFER TO CARGO BARGE


1 During load-out, the cargo barge behavior shall be monitored.
*All structures shall be loaded to the cargo barge by Crane.*
2 The data contained in the Cargo Barge Ballast Tables shall be adjusted one day
ahead of the load-out according to the tide observation during 72
hours.

3 These corrective measures shall revise the River Bar tide table.

10.0 CARGO BARGE


A cargo barge shall be furnished to carry the fabricated structures, materials
and Equipment, from the Fabrication Shop / Jetty Yard to the installation Site.

The cargo barge shall be brought Stern to Quay and held secured on the
mooring hardware already at the quay.
11.0 BARGE BALLASTING SYSTEM
.1 Introduction
The Barge Ballasting system consists of the followings:
High capacity, low head, diesel driven, multi stage centrifugal
pump(s).
Suction Hoses, foot valves, & strainers.
Discharge Header; containing:-
- Header Pipe.
- Tees.
- Butterfly Valves.
- Discharge Hoses.

The Ballast Pumps, and the header / manifold (c/w valves) shall be
installed onboard an
800hp Tug Boat.

Suitable hoses (c/w quick couplings) shall be used for water distribution.

This shall help in giving room on barge deck, and improving pump NPSH.

.2 Ballasting Sequence

i) Pre-ballasting
Pre-ballasting of the barge shall be done using external pumps.

before Load Out date.

During the load out the alignment of the barge shall be

Due to limited inclination forces, the heel of the barge shall be
monitored and corrected if the inclination reaches 0.5%. The
e.
monitoring shall be done by means of a water level with a
length of 30 mt from one side of the barge to the other.
In the same way the trim of the barge shall be monitored and

corrected if the inclination reaches 0.5%.
The level of the barge in relation to the quay edge shall be

monitored with two gauges from the quay edge.

iv) Ballasting of barge during Load Out

.3 Mooring Ancillaries
Mooring system shall consist of the following minimum components:
- Two sling wires tied to jetty mooring cleats.
- Trucks (or Caterpillar crawler tractors) shall pull the said sling wires via
turnbuckles.
- Ocean going Tug Boat (Pusher) at the bow side of the Cargo Barge.

12.0 TIE-DOWN & SEA FASTENING PROCEDURE


.1 TIE-DOWN
The Truck and/or Barges characteristics, Sea and/or Road States, the
structures position on
the Truck and/or Barge and CONTRACTOR C requirement shall be
considered during the planning of the
Tie-Down and Sea-Fastening process.
The tie-down braces shall be located on the barge in such a way as to
minimize installation and removal time, material requirement and damage
to the equipment or structure and yet to provide a safe tie-down system.
.2 SEA FASTENING
VESL shall fabricate and install the Sea Fastening for the fabricated
Structures and Shipped loose items onto the Transportation Truck and
Barge supplied by CONTRACTOR.
All Sea Fastening welds shall be classified as temporary welds in
accordance with the Structural Welding and Inspection Specification.
13.0 SAFETY REQUIREMENT
Before the commencement of the Load-out operation, a Safety Meeting shall be
held with the team involved.
This meeting shall brief to all concerned the load-out procedure, the associated
emergencies, possible causes of Accidents, Near Misses, Unsafe Acts, and
conditions noted from similar operations.
The dedicated Safety Officer shall cover the Load-out Operation activities.

Also, a special transport / ambulance shall be standing-by during the Works.

A detailed Job Safety Hazard Analysis shall be discussed.

All personnel to be engaged for this work shall be mobilized and properly
inducted, using Workbooks and Videotapes as Teaching Aids.

This shall enable the workers appreciate the hazards involves in the offshore
operations.

13.1 Safety Measures To Be Put In Place.


13.1.1 Position adequate fire extinguishers
13.1.2 Position fire water pumps.
13.1.3 Continue gas monitoring.
13.1.4 Position fire blankets.
13.1.5 Install safety signs.
13.1.6 Install barricade where handrails are removed.
13.1.7 Good house keeping.
13.2 Personal Protective Equipment. (PPE)
Safety wears shall be provided as appropriate to the working conditions at
site.
Thus, Coveralls for all workmen,; Safety helmets, Safety boots and
protective gloves to all workmen handling heavy, sharp, awkward or hot
materials or equipment; Safety goggles whenever grinding and blasting or
other activity which could cause damage to the eyes is involved.
Note: Personnel Protective Equipment shall be worn at all
times within the lifting area
and vicinity.

The PPEs shall include:


Safety Boot,
Coverall,
Safety Glasses,
Hand Gloves,
Life Jackets,
Helmet,
Safety Harness,
Ear Defender (Muff)

13.3 Safety Officer


The dedicated Site Safety Officer shall cover all work on the sites.

The Safety Officer shall ensure that:


All equipments are pre-mobilized by CONTRACTOR C i.e. Cranes,
Barges/Vessels, welding machines,
compressors, boats, etc
All lifting equipment are tested, certified and inspected.
Records of all inspection are kept and given to authorized personnel when
demanded
All lifting equipment are colour coded and their safe working load
displayed
Danger signs and barriers are positioned around lifting areas
Cranes are use within their rated capacities at the required radius of lift
Safety boat stands by throughout the operation

Appropriate to PPE is used at all times i.e. safety harnesses & the
personnel on the platform use work vests.

13.4 Fire Protection Equipment

13.4.1 The Fire Protective Equipment shall include but not


limited to the following:

Standby firewater pumps/personnel are in place and shall


be used as required for critical tie-ins and other hazardous
operations in the facilities.
Workers shall be taken through induction courses on fire
hazards, fighting, prevention and protection.
Periodic fire drills shall be organized on site.
Fire muster points shall be designated at every work place.

13.5 Medical Coverage.


All medical evacuation shall be as per the medical evacuation procedure.

The operations shall be carried out in a manner that shall prevent health
problems to the staff and others, and give health of staff adequate
attention backed up with required resources.

All workers shall be medically certified fit to work at the site. The
result and data obtained by medical examination shall be suitably
recorded and kept for reference.
The health, safety and environment of the workers shall be properly
safeguarded to avoid injuries and illness
Site Clinics shall be located on each Barge. These shall be fully
equipped with essential drugs, with qualified Nurses in charge.
Any illness that could not be handled by the site Clinic shall be
referred to the company retainer Clinic in Port Harcourt.
Medivac shall be available to convey any victim to the nearest
affiliated clinic should there arise an emergency.
All injuries/illnesses shall be recorded in the First Aid register and
also treated as an accident reported through normal lines of
reporting.
All Catering staff to be engaged in the catering services on the
project shall undergo thorough medical examination and certification
before commencing the job.

13.6 Warning and Caution Signs


Adequate number of specific warning and caution signs shall always be
displayed.

This shall include but not limited to:-


Men at Work
Muster point
Work in Progress
No Access
No Smoking
Toxic/Flammable liquids
Radiography in progress
Hard Hat and Helmet area
Ear defender MUST be worn
Others as required.

13.7 Gas Testing


Gas Detection Tests shall be carried out before any hot work, and shall be
carried out by
VESL, with a nominated CONTRACTOR C representative witnessing and
approving.

Gas tests shall be continued throughout the course of the work where
explosive gas build-up is suspected and the frequency and range of such
tests shall be to the approval of the CONTRACTOR C Representative.

No hot work shall be undertaken if the results of the gas tests show a level
of gas concentration that could constitute hazards.

The Supervisor shall be held responsible and accountable for ensuring


that no work commences at such site without the required tests being
performed.

13.8 Swimming Test/Certification


Every Personnel engaged in these operations should undergo Personal
Survival Swimming
Test and Certification.

All employees that shall be engaged in this project shall undergo medical
fitness test prior to
swim test and mobilization offshore.

Arrangement shall be worked out with the Responsible Department to use


CONTRACTOR C facility in
Port Harcourt for the certification.

13.9 Hazard Management System


Hazards on the project shall be identified and documented by the site
Safety Officers.

Hazard Management Forms shall be used in recording all types of Unsafe


Acts/Unsafe Conditions, health, Pollution, Environmental disorders,
Security lapses as well as potential HSE hazards on a daily basis.
The Safety Officer and Site Supervisor shall rectify the observed unsafe
acts/conditions immediately. Daily copies shall be submitted to the
Project Manager and CONTRACTOR C Representative.

13.10 CASHES Meeting.

13.10.1 Project CASHES meeting.


The Project CASHES meeting shall be held bi-weekly and shall be
chaired by the Project Manager or his designate.

The meeting is intended to be a forum for the discussion of CASHES


issues.

All Managers, Supervisors and Superintendents present at site shall


attend this meeting.

This meeting shall be documented with minutes and all parties or


individuals requiring completing these action points named as
action parties.

The minutes of this meeting form the basis of communication


downward through the general CASHES meeting as well as upward
to the company wide CASHES meeting.

13.10.2 Unit level meeting.


The unit level CASHES meetings shall be held weekly and shall be
chaired by the supervisor in charge of the unit.

The meeting is intended to be a forum for the discussion of CASHES


issues.

All employees must attend these meetings. These meetings shall be


documented with general topics noted.

The meeting forms the basis of communication from the supervision


level to the worker as well as upward to the project CASHES
meeting.

13.10.3 Morning unit meeting.


The morning unit meetings shall be held daily. The meeting shall be
chaired by the
unit leader lasting not longer than 15 minutes.

The purpose of the meeting is both safety of the worker at the tool
handling level as
well as organization of the work on daily basis.

The topic of the discussion shall be the days operations and to


highlight the major
hazards which shall be encountered.

13.10.4 On-the-job task meeting.


On-the-job task meeting is the most important link in the CASHES
communication
system.
It is the responsibility of all supervisory personnel to address any
hazardous
situations they envisage in the task to be perform and to inform
workers under their
supervision of the appropriate actions to pursue to alleviate these
hazards.

Also, it is the responsibility of any employee who perceives a


situation to be
hazardous to notify his supervisor immediately.

13.10.5 Accident Reporting.


Every worker, working under this project shall immediately report to
his
Supervisor/Foreman or Safety officer, any accident which occur to
him, his fellow
employee/or any machinery/ equipment in his position of work.

The Supervisor/Foreman and the Site CASHES Officer shall visit the
accident scene
immediately, with those involved to obtain information for Accident
Investigation
Report Form.

All accidents involving lost time injury or loss of equipment shall be


immediately
followed by stoppage of work on the area. Work shall only resume
when the area is
adjudged safe and all situations put under check.
