REMARKS: On Wednesday, February 6 2017, a significant landslide occurred al Greentree Landfill at
approximately 2:00 p.m. Mr. Don Hentichs, General Manager notified the DEP by leaving voicemail
‘messages for Chris Myers, Solid Waste Specialist and Jack Crow, Sold Waste Supervisor. Department
Staff contacted Mr. Henvichs by telephone shortly before 3:00 p.m.
According to Mr. Henrichs, approximately 3 acres of waste in Cells 0 and 10 suddenly sl to the south and
‘west. One lanl employee who was operating a compactor was missing, Search and rescue operations
had begun. PEMA, Ek County EMA, local re and rescue, and the Red Cross responded to the ste. DEP
Emergency Response team members Rich Sheriff and Kevin Jordan were dispatched to the landfl
(On Thursday, February 9, 2017, a site inspection was conducted by the folowing DEP personnel: Brian
‘Mummert, Environmental Group Manager, Jack Crow, Solid Waste Supervisor, Chris Myers, Sold Waste

“The site was inspected by all DEP and Advanced Disposal Services personnel sted above. Ine folowang
parative is compiled from interviews withthe various Advanced Disposal Services personnel present
(On Monday, February 6, 2017, a vertical crack was observed on the south-facing slope of Cell 10B above
the intermediate cover. ‘Setting cracks were observed inthe active disposal area. The cracks atthe
working face were staked, recorded with GPS and filed.
(On Tuesday, February 7, 2017, the site appeared to be stable.
‘On Wednesday, February 8, 2017, a crack was noticed atthe working face, The crack was reportedly 4 ~
' feet wide by 100 feet long, The active disposal area was moved further in towards Cell 9, away from the
‘rack. The dling rg was preparing to install @ gas collection well near the active disposal area. Four gas
‘collection wells had been driled in the vicinty during the prior week. A bulge in the intermediate cover was
‘observed on the south-facing side of Cell 108. The bulge was nex! to the area were an access road was
previously located. At about 2:00 p.m. the intermeciate cover onthe southside of Cell 108 spilt open,
‘causing a se that was approximately 8 feet in width. Almost immediately, the waste mass in Cals 9 and
0B stared to shit. Personnel were evacuated just before the massive slide occurred. As stated above,
‘one lanfill employee was unable to escape.
‘Approximately 3 acres ofthe waste mass sid tothe south and west. Of that, 1 to 2 acres of waste had
likely Spread beyond the lined area. Al the time of this inspection, work was focused on finding the
missing compactor and shoring up the high walls to faciitate search efforts. A ravine tothe west of Cell
"TOB wae being converted into two emergency leachate collection ponds. Clay and other earthen materials,
‘were being placed to trap leachate from the waste that was outside ofthe lined area. A channel was being
formed along the shifted waste mass ta direct leachate drainage to the emergency collection ponds. As a
last resort the outflow of water in Sedimentation Pond C could be blocked if leachate flowed past the
‘emergency ponds. As ciscussed, ADS wil also routinely sample Sedimentation Pond D to monitor water

‘A smal fre broke out inthe shifted waste on Thursday morning. The water truck normally used for
‘washing the roads was being used to try and extinguish the fre.
Gas collection was cutoff from the affected area. All employees were using Individual hydrogen sulfide
‘gas meters, Two fixed gas meters were planned to be installed near the property line, downwind from the
affected area
