acknowledge receipt of the mobile phone and Sim card from the undersigned.
I understand & agree that this mobile phone and Sim card etc is issued to me by the
undersigned, at their discretion, for use during my employment with them to ensure proper
communication and flow of information- free of cost to me. I undertake to use the wired
earphone-speaker system for receiving calls while travelling too. I understand that the
undersigned will pay the rental & call charges incurred by me for use in the normal course of
business. However personal calls will be chargeable to me and I undertake to pay the same.
I understand & agree that this mobile phone & sim card etc is to be used only for my official
communication. While proceeding on leave, I agree to hand over the same to the undersigned
unless specifically permitted to retain possession of the same.
I understand & agree that the Mobile Phone & Sim card etc will be used, maintained and
taken care of as per the instructions manual & as listed hereunder.
I understand & agree that this Mobile Phone & Sim Card etc is the absolute property of the
undersigned. I agree that I shall have to hand over possession of the Mobile phone handset &
sim card etc in a good working condition to the undersigned as and when I leave the services
of the undersigned.
I agree to allow my superiors to inspect the mobile Phone & sim card in my custody.
I agree & undertake not to allow any third party to handle and / or use the mobile phone and
sim card without prior permission of the undersigned.
I agree & undertake to keep the mobile phone fully charged & active all the time. I undertake
to receive all calls from the management & other colleagues / clients for and during the
furtherance of the consultancy of the undersigned.
I understand & agree that it is my duty to maintain the mobile phone and sim card in good
condition always. If the handset and sim card etc is lost, I shall report the same to the
undersigned and replace the handset and sim card etc at my own cost.
I hereby agree & undertake not to use my position & the contacts developed by being in this
position for participation in games and competitions, for personal gain or any other business
activity / profession or to assist third parties & send or relay any kind of messages which
are not in furtherance of the business interests of Bharadwaj Consultants & Jupiter & Co group
and could hurt the Social, Moral, Political, Religious, Legal Rights / Sentiments of sections of
the public, undersigned & other co workers etc. I agree to abide with the rules of usage as
fixed by the management from time to time.
