1. Safety
1.1 Ensure lube oil circulation is stopped and isolated Yes / No
1.2 Ensure power supply to motor is isolated & tagged Yes / No
1.3 Ensure Casing is fully depressurized & purged Yes / No
1.4 Ensure Valid PTW is taken and available on job Yes / No
1.5 Ensure that all requirements of JSA have been fulfilled Yes / No

2. Coupling
2.1 Check free axial/radial movement of coupling Free / Tight
2.2 Inspect coupling and fill Coupling inspection Checklist
2.3 Check tightness of coupling bolts prior to removal Loose / Tight
2.4 Check match marking of coupling parts before removal
2.5 Measure and record coupling hub draw

2.8 Check tightening torque of cap screws (3/8 - 24) 50.2 N.m
2.9 Check tightening of lock nuts (1/2 - 20) {Factory set value} 130 N.m

4. Lube oil and seal systems


4.1 Ensure marking / securing of all lube oil connections Yes / No
