Disclosure Desk Policies and Procedures

Introduction

The Disclosure Desk will issue initial disclosure packages, re-disclosed Loan Estimates, and Initial
Closing Disclosures. The following list of procedures must be followed on all loans to insure
accurate disclosures are issued in a timely manner. Our goal is to issue disclosures requested
the same day or within 24 hours. The Disclosure Desk will post turn-times daily in the
Disclosure Desk Group in Spear during peak days of the month if turn-times exceed 24 hours.

Rush Requests

If you need to have your disclosures issued within 2 hours, a $50 fee will be charged to your
branch. Requests that are placed after 3:00 P.M. Arizona time for same day documents will
also be assessed a $50 fee. Please get your same day requests in prior to 3:00 P.M. just in case
we need to contact the title company for any clarification.

Information/Documentation Needed

The following minimum documentation and/or information is required to issue disclosures for
each category below:

1. Initial Disclosures:
Title fees
Purchase Contract ( If applicable )
Completed 1003 (Includes all information. NO BLANK SPACES. If borrowers are not
employed input Retired, Homemaker, Disabled, etc.)
Complete Aggregate Screen OR notate Escrows are WAIVED
Itemization needs to be completed w/ origination fees, discount points and any DPA
fees
A valid email address for BOTH borrowers

2. Re-Disclosed Loan Estimate and Change of Circumstance:


Completed and detailed Change of Circumstance Screen in Encompass must have
reason and fee comparison completed
Confirmation of Lock for Lock, Re-lock, or Extensions
Confirm you have a valid reason for re-disclosure on the PCR Matrix
Fee Variance Tracking screen completed for any applicable cures
Mortgage Insurance quote or certificate if applicable and make sure input correctly
in MI screen

3. Closing Disclosure:
We must have the final settlement numbers including tax pro-rations, HOA Fees,
Seller or Realtor Credits from the title company and encompass updated to match
Confirm credits do not exceed the actual closings cost
Loan must be locked and the lock expiration date updated for the closing date
Appraisal must be in and any Final Inspection fee already disclosed on an LE
Confirm closing and disbursement dates are correct in Encompass
Confirm Loan Amount, Rate, and Loan Terms with LOA/LO
Conditional Underwriting Approval is mandatory

Ordering Disclosures

A screen has been set up in Encompass called Disclosure Desk for you to order disclosures.
Please see attached Exhibit A. There is a dropdown to select the type of disclosures you are
requesting, the manner in which the disclosures will be delivered to the borrower, a Rush
Request checkbox, and a field for notes. Once the order has been completed, the loan will then
appear on the Disclosure Desk Dashboard. The Disclosure Desk Team will work on the orders
by date and time requested unless a Rush has been requested.

If you would like the wet signed documents emailed to you as a PDF, please request this in
the Notes section of the Disclosure Desk order screen.

Notifications
After the order has been placed, an email will be sent to the parties working on the file. When the
Disclosure Desk team has completed your order, you will receive an email notification from the team
member.

1. Go to the Disclosure Desk Screen in Encompass:

2. Select the Disclosure Type you want to issue and the Delivery Method. You can also
request a rush and add notes that you would like to communicate to the Disclosure
Desk Team. Next, hit the Order button:

3. Your order confirmation will appear, click okay, and you are done. You will receive an
email receipt for your order. Your request will be sent to a working Dashboard and
the Team will contact you if they have any questions on your file. If not, you will
receive an email when the disclosures are completed.
