ADDENDUM REPORT ON
THOROUGH EXAMINATION AND LOAD TESTING
OF A 35Tons
FOR
OCTOBER, 2015
2. The damaged safety latch on the Auxiliary hoist was replaced as shown in the picture.
3. At six (6) parts of line (three sheaves reeved), the SWL from the crane manual correspond to 37560kg
(37.560tons). To be on a safer operation the SWL will be 35tons.
4. A load test for 20tons was also carried out on the crane and it was found to be structurally stable
5. The load test on the main hoist was just to confirm the functionality of the load moment indicator (LMI).
From the report, the difference in values between the LMI and the load chart is within acceptable limits.
Observation/ Comment
1. The number of parts of line does not correspond to the safe work load on the load chart.
2. The auxiliary hoist safety latch is damaged
3. The safety critical gadgets are found functional including the ALARM systems and the
lightings.
4. LMI is fixed and functional.
5. The wire rope is not properly spooled on the drum
6. No significant leakages were observed from the seals, valves, outriggers and the hydraulic
hoses.
Recommendations
1. The crane safe work load should be reduce to 35tons as recorded on the crane manual due
to the number of reeved sheaves for maximum safe operations.
2. The wire rope must be spooled uniformly on the drum to prevent damage.
3. Load chart should always be kept in the crane cabin and should be consulted before
commencement of any Lifting Operations.
4. The Crane manual should also be made available in the cabin.
5. Pre use inspection must be carried out before operations.
6. Operate crane only in accordance with the recommendations as given in the manufacturers
manuals and only competent operators should operate the crane.
Survey Statement
The equipment after thorough examination is found in good structural and functional
condition as at the time of inspection and is hereby certified fit for purpose only within
designated operational parameters.
CONCLUSION:
We certify that the Equipment described herein was thoroughly examined in accordance with the
relevant sections of the factory Acts 1990 and those other standards upon which our procedure
is drawn.
