Dear Sir,

Please be informed that one of our casual employee assigned to Crane Maintenance was injured while
fixing the wire rope of the Tadano crane.

The Incident happened around 9:20 oclock in the morning of November 17, 2015 at Motor pool Area of
DICT Yard, while Mr. Ortizo was working to cut-off the defective wire rope. The opposite end of the wire
rope suddenly slipped from the crane lock this accidentally hit Mr. Ortizos Nose Bridge resulting to
severe nose bleeding and lacerated wound at left eyebrow.

Mr. Ortizo was immediately brought to company clinic for First Aid before Admitted to Hospital.
