Safety
Intended use

2 Safety

2.1 Intended use

The argon plasma generator is intended solely for use in open surgery and minimally
invasive surgery on humans. Only soft tissue may be treated in short-term use.
The precise purpose is determined by the instrument connected. Only approved in-
struments and accessories may be connected (see catalogue or our website:
www.soering.com).
This results in applications for
non-contact argon plasma coagulation,
monopolar and bipolar cutting,
monopolar and bipolar coagulation.
Non-contact argon plasma coagulation is a monopolar application in which high-fre-
quency electrical current ionises an argon gas, thereby generating an arc between
the Arco handle probe and the tissue.
The argon plasma generator is not intended for use in heart or eye operations.
As no definite information is available for the treatment of high-risk patients prima-
rily pregnant women and haemophiliacs, the attending surgeon must weigh the risks
of the treatment particularly carefully against the benefits.
The same applies to neurosurgery. Monopolar cutting and the use of an operating
mode with higher peak voltage for monopolar coagulation as well as argon plasma
coagulation should be avoided. Only bipolar coagulation is recommended.
In each case, the surgeon providing treatment is responsible for weighing the risks of
treatment against the benefits.
The argon plasma generator may only be used in medical operating rooms.
Only qualified specialists such as OP staff, sterilisation department staff and operating
room assistants possessing the required education, training or knowledge as well as
sufficient experience may use the argon plasma generator.
Only qualified surgeons may operate on patients using instruments connected to the
argon plasma generator.
The connected instruments and the accessories used (e.g. instrument cable, probe
and neutral electrode) must be approved for a peak voltage that is higher than the
peak voltage for the selected operating mode (see section 10.1 Power tables and
power graphs on page 92).
For argon plasma coagulation the argon plasma generator may only be used with the
pressure reducer from Sring and only with argon gas.
In general the argon plasma generator may only be used by individuals properly in-
structed in its handling. The manufacturer or a person authorised by the operator
must provide instruction in the handling of the argon plasma generator with the help
of these instructions for use.

DocNr.: 03-4769_R02.01 | 09-2013 | en-GB | Arco 3000 9

ments may not be permanently activated. A pause must be made after each activa-
tion. The activation time during an operation must not exceed a total of one hour.
The required environmental conditions for operation as well as for transport and stor-
age must be met (see section 4 Transport and storage on page 35).
Under no circumstances shall liquid be allowed to penetrate into the argon plasma
generator.
Any other use is considered to be improper use. Use of the Automatic function for bi-
polar cutting, in particular, is not permitted.
The local occupational health and safety, hygiene and safety regulations and the
specifications in these instructions for use and the instructions for use for the instru-
ments used must be complied with.

2.2 General safety information

Insufficient qualification
Improper handling of the argon plasma generator may lead to severe personal injury,
irreversible physical injury or death.
Only deploy qualified specialists.
Do not grant access to unauthorised persons.
Contact the manufacturer or an authorised dealer if necessary for repairs and
maintenance work.

Magnetic fields
Magnetic fields may affect the function of the argon plasma generator and severely

Safety
General safety information

Use of a physiological monitoring device

If the argon plasma generator unit is used at the same time as a physiological moni-
toring device with monitoring electrodes (e.g. an ECG device), this may cause a
high-frequency current to flow in the monitoring electrodes. This can result in severe
burns in the area of the monitoring electrodes. In addition, the monitoring device can
fail.
Use a monitoring device with a high-frequency current restriction.
Attach the monitoring electrodes as far as possible from the surgical site and from
the neutral electrode.
Do not use needle electrodes as monitoring electrodes.

Using other electrical devices

The use of the argon plasma generator can impair the function of other electronic de-
vices.
Only use the argon plasma generator if the impairment of other devices by the
unit has no negative impact on the patient.

Damaged cable
Damaged insulation on the mains cable or the instrument cable can cause electric
shock and result in severe injury or death.
Check cables for damage.
If a cable is damaged, immediately switch off the power supply (or do not switch
it on).
Replace any damaged cables.
Only use undamaged cables.

DocNr.: 03-4769_R02.01 | 09-2013 | en-GB | Arco 3000 11

Safety
General safety information

Improperly connected instrument cable

An improperly connected instrument cable may cause the connector socket to melt.
This will lead to malfunctioning of the argon plasma generator and may result in se-
vere personal injury to patients. Treatment must be continued with replacement
units or by conventional means, thereby lengthening the treatment duration.
Correctly connect the instrument cable. The instrument cable must engage in the
connector socket.
Prior to each use of an instrument, ensure that the instrument cable is properly
connected.

Incorrect instruments and spare parts

The use of unauthorised instruments and unauthorised accessories can lead to mal-
function and severely injure operators and patients.
Only use approved instruments (see catalogue or our website:
www.soering.com).
Only use approved accessories (see catalogue or our website: www.soering.com).

Insufficient peak voltage for instrument or accessories

Use of an instrument or accessories only approved for a peak voltage that lies below
the peak voltage for the operating mode can lead to malfunctions and severely injure
operators as well as patients.
Prior to each use, check that the peak voltage for the instrument and the accesso-
ries is above the peak voltage for the operating mode (see section 10.1 Power ta-
bles and power graphs on page 92).

Safety
General safety information

Operation outside the specified range of operating conditions

Use of the argon plasma generator outside the specified range of environmental con-
ditions for operation as well as for transport and storage may have incalculable con-
sequences.
Only operate the argon plasma generator under the specified operating condi-
tions.

Treatment of high-risk patients

Treatment of high-risk patients with high-frequency current can lead to death or se-
vere bodily injury. High-risk patients include pregnant women, haemophiliacs and
patients with pacemakers.
Prior to beginning treatment, ensure that all precautionary measures have been
taken to avoid the hazard.

Treatment of pregnant women

Treatment of pregnant women with high-frequency current can lead to death or se-
vere bodily injury. The foetus, in particular, can be damaged.
In such cases, the bipolar application mode should preferably be used.
If monopolar application cannot be avoided, position the neutral electrode so
that the foetus cannot be damaged.

Treatment of haemophiliacs
In the treatment of haemophiliacs, there is a risk that bleeding cannot be stopped or
cannot be stopped sufficiently. This may lead to death or severe bodily injury.
Prior to beginning treatment, take all precautionary measures necessary to avoid
the hazard (e.g. adjust medication).

DocNr.: 03-4769_R02.01 | 09-2013 | en-GB | Arco 3000 13

Safety
General safety information

Treatment of patients with pacemakers or other active implants

Treatment of patients with pacemakers or other active implants with high-frequency
current can lead to death or severe bodily injury. The pacemaker or active implant can
be damaged or malfunction.
If in doubt, consult a cardiologist before beginning treatment.
Weigh the medical benefits of the treatment against the risks.
In such cases, the bipolar application mode should preferably be used.
If monopolar application cannot be avoided, position the neutral electrode so
that the path of the high-frequency current does not pass through the pacemaker
or active implant.
Prepare for the event that the pacemaker is damaged or malfunctions.

Treatment of patients with implants

The current density can be higher in the implant region when high-frequency current
is used. This can result in tissue damage.
In such cases, the bipolar application mode should preferably be used.
If monopolar application cannot be avoided, position the neutral electrode so
that the path of the high-frequency current does not pass through the implant.
Weigh the medical benefits of the treatment against the risks.

High-frequency current
The high-frequency current directed at the tissue by the instrument can cause cuts or
burns.
The instrument should be used with care and for its intended use only.
The electrode or probe may only be directed at the area to be treated.

14 Arco 3000 | DocNr.: 03-4769_R02.01 | 09-2013 | en-GB

Safety
General safety information

Plasma beam
The high-frequency current directed at the tissue by the plasma beam may cause cuts
or burns.
The Arco handle should be used with care and for its intended purpose only.
The plasma beam may only be directed at the area to be treated.

Sparking during high-frequency applications

When high-frequency current is used, sparks which can ignite explosive or flammable
materials are formed. The sparks and arcs formed during operation may cause fire or
explosions. Explosions and fire can result in severe injury or death as well as consider-
able material damage.
Do not use anaesthetic gas or oxygen.
Do not use any explosive or flammable materials. Use of the following is strictly
prohibited: oxygen-saturated cotton wool or cotton gauze, alcohol-soaked gauze,
flammable skin cleansers, flammable degreasers or flammable disinfectants.
Never point the plasma beam at explosive or flammable materials.
Avoid endogenous gases. Prior to the operation, adjust the patient's diet accord-
ingly.

Use of combustible fluids

Sparks formed can ignite combustible fluids and cause burns as a result. Especially
dangerous is fluid that has collected on the patient's skin, in bodily depressions or
cavities.
Only use non-combustible fluids, e.g. disinfectants.
Before the operation, wipe off fluid that has collected on the patient's skin, in
bodily depressions or cavities. Allow any residues to evaporate completely.

3 Overview

3.1 Overview of argon plasma generator

The argon plasma generator is suitable for non-contact argon plasma coagulation as
well as for monopolar and bipolar applications. Various instruments can be operated
with the argon plasma generator (see section 3.3 System overview on page 27).
The power settings can be adjusted on the operator control panels according to the
requirements of the treatment. Frequently used power settings can be saved in ten
program storage locations.
The treatment is actuated via a or a button on the instrument. With the aid of an au-
tomatic function, bipolar instruments can also be activated via measurement of the
tissue resistance.
An endoscopy monitor can be connected to the argon plasma generator. Through
the direct connection of the endoscopy monitor, the endoscopic image is less dis-
turbed because the high-frequency current output is synchronised with the line out-
put transformer in the endoscopy monitor.

Overview
Overview of argon plasma generator

Display panel
On the display panel, there are five displays which provide information about the sta-
tus of the argon plasma generator.
With monopolar applications, the argon plasma generator monitors the connection
between the neutral (patient return) electrode and the patient, provided that one of
the recommended split (dual) neutral electrodes is used. The Patient electrode traffic
light indicator indicates the quality of the connection. The traffic light indicator lights
green or greenish yellow when the neutral electrode is properly connected to the pa-
tient. The traffic light indicator lights yellow if the neutral electrode is not connected
optimally to the patient. If the neutral electrode is not properly connected to the pa-
tient, the traffic light indicator lights up red. An alarm tone sounds at the same time.
The argon plasma generator is equipped with safety functions. It independently de-
tects faults during the self-test and certain other faults during operation. The Func-
tional error indicator signals whether a safety shutdown or a fault in the argon plasma
generator has occurred. As soon as a fault is detected, the Functional error indicator
lamp lights red. An alarm tone sounds at the same time and the argon plasma gener-
ator no longer outputs any power.
The argon plasma generator also carries out checks during monopolar cutting to de-
termine that an electrode suitable for cutting has been selected and that the selected
cutting power is high enough. The Dosage error indicator lamp lights yellow if an elec-
trode that is not suitable for cutting has been selected or if the power setting is too
low.
The argon plasma generator monitors the gas flow. If there is not enough argon gas
in the gas cylinder, the Gas shortage display lights up yellow. The Gas blockage display
lights yellow when a gas blockage occurs.

The correct operating mode must be selected for each monopolar and bipolar appli-
cation. For some operating modes, an effect level must also be selected.
Monopolar handles and Arco handles come in different versions: with and without
buttons.
Activation is carried out via buttons on the handle body or a foot switch.
Monopolar cutting is always activated with the yellow button on the handle body or
the yellow pedal on the foot switch.
Monopolar coagulation is always activated with the blue button on the handle body
or the blue pedal on the foot switch.
Bipolar coagulation is activated with the blue pedal on the foot switch (recom-
mended). However, it can also be activated with the help of the Automatic function
by measuring tissue resistance. The Automatic function may not be used for bipolar
cutting.
Several instruments, i.e. a monopolar handle, a bipolar instrument and an Arco han-
dle, can be connected at the same time to the argon plasma generator. However, only
one instrument can ever be activated at a time.
During application, specific activation tones sound. Monopolar cutting and coagula-
tion have different activation tones. During monopolar and bipolar cutting in Pulse
mode, different activation tones can be heard that indicate the switching over be-
tween cutting and coagulation.

6.1.2 Attaching the neutral electrode and checking the connection

For both monopolar applications and argon plasma coagulation, a neutral electrode
(patient return electrode) must be correctly positioned and applied as specified to the
patient's skin. Split neutral electrodes are recommended.
The neutral electrode must be attached at least 15 cm away from the surgical site. The
high-frequency current flow path between the active electrode and the neutral elec-
trode must be as short as possible.
The argon plasma generator can only monitor the connection between a split neutral
electrode and the patient's skin. The Patient electrode traffic light indicator indicates
the quality of the connection. If the neutral electrode is not connected properly, the
traffic light indicator lights up red (see Fig. 3.9 on page 25, No. 1). In this case, the in-
strument cannot be activated.

Requirements
The argon plasma generator is on.
The neutral electrode cable is connected to the argon plasma generator.

Procedure
1. Apply a neutral electrode to the patient's skin in the correct position and accord-
ing to guidelines.
2. Connect the neutral electrode to the neutral electrode cable.
The neutral electrode is attached.
3. Check the Patient electrode traffic light indicator.
4. If the traffic light indicator system lights up red, improve the connection between
the neutral electrode and the patient.
If the traffic light indicator lights yellow, check the neutral electrode connection
and improve if necessary.
The traffic light indicator lights green, greenish yellow or yellow.
Check the connection between the neutral electrode and the patient.
Before treatment can be started, functional testing must be carried out (see section
6.1.4 Carrying out functional testing on page 51). The power settings must also be ad-
justed (see section 6.1.5 Adjusting power settings on page 56).
If treatment had to be interrupted because the neutral electrode was no longer prop-
erly connected to the patient, it may be continued as soon as the Patient electrode
traffic light indicator lights green, greenish yellow or yellow.
During treatment, the traffic light indicator must be continuously monitored.

6.1.3 Adjusting the volume

An activation tone signals that high-frequency current is being supplied to the instru-
ment. The volume of the activation tone can be adjusted by means of the volume
regulator (see Fig. 3.2 on page 18, no. 2). Adjustment of the volume only affects the
activation tone. The setting has no effect on the volume of the alarm tones. The acti-
vation tone cannot be shut off completely.

Procedure
1. To lower the volume, turn the volume regulator to the left.
To raise the volume, turn the volume regulator to the right.
The volume has now been adjusted.

6.1.4 Carrying out functional testing

Functional testing of monopolar cutting and coagulation

Prior to use for a monopolar application, functional testing must be carried out. First,
a neutral electrode must be applied to the patient's skin in the correct position and
according to guidelines, a neutral electrode cable must be connected and the Patient
electrode traffic light indicator must light green, greenish yellow or yellow. For func-
tional testing, the required monopolar handle must be connected to the argon
plasma generator. However, the monopolar handle should not yet be fitted with an
electrode.
Monopolar cutting is also possible using an Arco multipurpose handle. If an Arco mul-
tipurpose handle is used functional testing must be carried out for argon plasma co-
agulation.

Requirements
The argon plasma generator is on.
The neutral electrode has been applied to the patient's skin according to specifi-
cations and the Patient electrode traffic light indicator is lit green, green-yellow or
yellow.
A monopolar handle is connected to the argon plasma generator, but not yet fit-
ted with an electrode.
The operator control panels for monopolar cutting and coagulation are activated.
The power settings are set to low values: monopolar cutting to 10 W and monop-
olar coagulation to 5 W.

6.2 Starting treatment

6.2.1 Requirements for all applications

Contaminated instruments and accessories

Use of contaminated instruments or accessories (instrument cables, electrodes and
probes) may result in life-threatening infections in patients.
Only use sterilised instruments.
Only use sterilised accessories.

Cable touching the patient or other cables

If the instrument cable or the neutral electrode cable touch the patient, despite intact
insulation, excessive current densities can occur. Even contact between several cables
can lead to excessive current densities. This may result in minor burns.
Lay the instrument cable or the neutral electrode cable so that they do not touch
the patient.
Lay the instrument cable or the neutral electrode cable so that it does not come
into contact with other cables that are connected to the patient.

6.2.2 Requirements for all applications

Unintentional activation of an instrument

An unintentionally activated instrument can result in severe injury to the patient.
Position the foot switch such that it cannot be actuated unintentionally.

Incorrectly set down instrument

Instruments that are not in use can be accidentally activated. Instruments set down
on a patient can result in severe injury to the patient.
Always set down an unused instrument away from the patient and other people.
Never set down an unused instrument on a patient.

Confusing cutting and coagulation

Confusing cutting and coagulation can result in severe injury to the patient. The but-
tons on the monopolar handle, the Arco multipurpose handle and the pedal on the
foot switch can be confused. Confusing them can lead to an undesired tissue effect.
Familiarise yourself with the instrument in order to press the correct button at all
times.
Familiarise yourself with the foot switch in order to press the correct pedal at all
times.

Button or foot switch doesn't switch off

An instrument that remains active because a button or the foot switch doesn't switch
off may lead to severe injury to the patient.
Hold the instrument up in the air.
Switch off the argon plasma generator.
Close the valve on the gas cylinder.

Operation
Starting treatment

Software error
Unintentional restart of the argon plasma generator due to a software error may lead
to severe personal injury to the patient.
Hold the instrument up in the air.
Readjust the power settings after the restart.

Power setting too low

Selection of power settings that are too low will lead to undesired cutting effects or
coagulation effects. This may result in increased blood loss and lengthening of the
time required for the operation.
Prior to beginning treatment, ensure that all power settings have been adjusted
correctly.

Power setting too high

Selection of power settings that are too high will lead to undesired cutting effects or
coagulation effects. Unintentional tissue burns, uncontrolled tissue destruction and
carbonisation may result.
Start the treatment at a low power setting.
Slowly increase the power setting during the treatment.

Power setting too high due to an unnoticed loose contact

A loose connection in the instrument cable or in the neutral electrode cable can re-
duce the tissue effect. If the output power is increased in such a case in order to
achieve a stronger tissue effect, this can result in serious burns due to suddenly in-
creased power on the electrode of the instrument.
If a loose contact is suspected, immediately replace the relevant cable.
Only use undamaged cables.

74 Arco 3000 | DocNr.: 03-4769_R02.01 | 09-2013 | en-GB

Operation
Starting treatment

Sudden stimulation of nerves or muscles

High-frequency current can suddenly stimulate nerves and muscles. If the patient is
not adequately immobilised, he/she can be seriously injured by the muscle contrac-
tion suddenly triggered. The danger arises particularly during monopolar cutting,
when using an operating mode with high peak voltage during monopolar coagula-
tion as well as with non-contact argon plasma coagulation.
Immobilise the patient, especially his extremities.

Varying output power on the active electrode

The output power on the active electrode can vary during the treatment. The cause
can be a stuck electrode, for example. This can result in a less than optimum tissue ef-
fect.
Clean clogged electrodes.
Adjust the power settings during the treatment if necessary.

Operation
Starting treatment

6.2.3 Requirements for monopolar applications

Monopolar applications include monopolar cutting, monopolar coagulation as well

as non-contact argon plasma coagulation.
A neutral electrode must be used for all monopolar applications. Use of a split neutral
electrode is recommended.
There are more risks to the patient in monopolar applications than in bipolar applica-
tions. The risks of monopolar application must be weighed against the benefits. Con-
sider whether bipolar application is also possible. In order to avoid unintentional
tissue damage, a bipolar application is to be preferred, especially if the high-fre-
quency current flows through parts of the body with a small cross-section or through
particularly sensitive tissue structures as in neurosurgery, for instance.

Skin-to-skin contact of body parts of the patient

A current flow can result from skin-to-skin contact of body parts of the patient and
lead to serious burns. Skin-to-skin contact can arise especially between the arm and
upper body as well as between the legs.
Ensure that the patient's body parts make no skin-to-skin contact.
Lay dry gauze (cotton wool) between closely adjacent body parts.

Improper neutral electrode

A neutral electrode with a quality and size not suitable for the patient can lead to seri-
ous burns.
Use a neutral electrode with a quality and size suitable for the patient.

Incorrectly applied neutral electrode

A neutral electrode which has been improperly applied to the patient's skin can result
in severe injury and burns or death.
Correctly position the neutral electrode.
Properly apply the neutral electrode to the prepared surface of the patient's skin
with the entire surface firmly pressed against the skin.

76 Arco 3000 | DocNr.: 03-4769_R02.01 | 09-2013 | en-GB

Operation
Starting treatment

Use of a non-split neutral electrode

The argon plasma generator cannot monitor the connection between a non-split
neutral electrode (patient return electrode) and the patient's skin. Nevertheless, the
traffic light indicator lights greenish yellow. For this reason, a non-split neutral elec-
trode can detach from the patient's skin during treatment without being noticed,
which is why serious burns can result beneath the neutral electrode.
Use split neutral electrodes so that the argon plasma generator can monitor the
connection between the neutral electrode and the patient's skin.

Poor connection between the neutral electrode and the patient's skin
A neutral electrode which is poorly connected to the patient's skin can result in se-
vere injury and burns or death.
Use split neutral electrodes so that the argon plasma generator can monitor the
connection between the neutral electrode and the patient's skin.
Continuously monitor the traffic light indicator for the neutral electrode.
Continuously check the connection between the neutral electrode and the pa-
tient's skin.
If the neutral electrode has detached, remove the neutral electrode completely
and inspect the skin. Then attach a new neutral electrode (disposable accessory).

Unusually low tissue effect despite high power setting

Even when the neutral electrode is apparently correctly attached, the connection of
the neutral electrode with the patient's skin can be poor. An indication of a poor con-
nection is that the tissue effect is reduced. If the output power is increased in such a
case in order to achieve a stronger tissue effect, this can lead to serious burns be-
neath the neutral electrode.
If a poor connection between the neutral electrode and the patient's skin is sus-
pected, examine the skin beneath the neutral electrode. Then attach a new neu-
tral electrode (disposable accessory).
Only increase the output power if the skin under the neutral electrode is undam-
aged.

Operation
Starting treatment

Neutral electrode cable not fully inserted

If there is a loose connection due to a neutral electrode cable that is not fully inserted,
the tissue effect can suddenly increase. This can result in severe burns.
Always connect the neutral electrode cable completely to the connector socket of
the argon plasma generator.

Body parts or tissue with a small cross-section

High-frequency current that flows through body parts or through tissue with a
cross-section that is too small can cause an unintentional tissue effect. This can result
in severe injury to the patient.
Correctly position the neutral electrode.
In such cases, the bipolar application mode should preferably be used.

Metal parts inside or on patient

Metal parts inside or on the patient can create uncontrollable current paths. This can
result in severe injury to the patient.
Correctly position the neutral electrode.
Do not touch metal parts inside or on the patient with the instrument.

Contact with earthed objects

Contact with earthed objects will result in earthing of the patient. When this occurs,
the high-frequency current cannot flow through the intended path. This can result in
severe burns.
Do not touch the patient with an earthed object.
Do not use passive instruments during the treatment.

Operation
Starting treatment

Contact with fluids

Contact with fluids can result in earthing of the patient. When this occurs, the
high-frequency current cannot flow through the intended path. This can result in se-
vere burns.
Place the patient on a dry surface.
Keep the surface dry.
Keep the patient's entire body dry using suitable materials.
Immediately vacuum up any fluids.

6.2.4 Requirements for argon plasma coagulation

Argon plasma coagulation is a monopolar application. All requirements for monopo-

lar applications must be met.

Selected argon plasma coagulation power too low

If the selected argon plasma coagulation power is too low the thickness of the scab
formed on the tissue will be insufficient. This may result in gas embolism.
Prior to beginning treatment ensure that the set argon plasma coagulation power
is sufficiently high.

Gas flow rate too high

An excessively high gas flow rate may cause gas embolism.
Select the gas flow rate according to the argon plasma coagulation power.
Do not select an excessively high gas flow rate.

DocNr.: 03-4769_R02.01 | 09-2013 | en-GB | Arco 3000 79

Operation
Starting treatment

Gas flow rate too low

If the gas flow rate is too low a poorly controlled arc may result and visibility may be
impaired due to smoke generation. Unintentional tissue burns and uncontrolled tis-
sue destruction may result.
If the arc is not precise increase the gas flow rate.
If smoke is generated increase the gas flow rate.

6.2.5 Requirements for bipolar applications

Unintentional activation of an instrument when the Automatic function is ac-

tive
When the Automatic function is active, the instrument can be activated unintention-
ally when it comes into contact with tissue, electrically conductive objects or fluids.
This can result in severe injury to the patient.
Prior to activating the Automatic function, ensure that the instrument is not in
contact with any tissues, electrically conductive objects or fluids.
When the Automatic function is active, ensure that the instrument does not con-
tact the patient's body in the wrong places.
When the Automatic function is active, ensure that the instrument cannot come
into contact with any electrically conductive objects or fluids.
