AFFIDAVIT OF LOSS

I, IREENA M. AQUINO, of legal age, married, Filipino citizen, and a

resident of Brgy. Ibung, Villaverde, Nueva Vizcaya, after having been duly
sworn to in accordance with law, hereby depose and state

1. That I am a bonafide holder of an Employment Identification Card issued

by Saint Marys University (SMU), Bayombong, Nueva Vizcaya;

2. That sometime in July 2017, the said card was inadvertently lost and
despite earnest exerted efforts, I failed to find it anymore;

3. That finding the futility of recovering the said card, it is hereby declared
that the same is considered lost and beyond recovery;

4. That I attest to the veracity of the foregoing statements and hereby execute
this affidavit for the purpose of securing another Employment
Identification Card in lieu of the lost one.

1. That I am a bonafide holder of an Employment Identification Card issued

by Saint Marys University (SMU), Bayombong, Nueva Vizcaya;

2. That sometime in August 15, 2017, the said card was inadvertently lost in
Baguio City and despite earnest exerted efforts, I failed to find it anymore;

3. That finding the futility of recovering the said card, it is hereby declared
that the same is considered lost and beyond recovery;

4. That I attest to the veracity of the foregoing statements and hereby execute
this affidavit for the purpose of securing another Employment
Identification Card in lieu of the lost one.

by Saint Marys University (SMU), Bayombong, Nueva Vizcaya;

2. That sometime in first week of September, 2017, while I was heading

home to Dupax del Sur, the said card was inadvertently lost inside the
jeepney I ride on and despite earnest exerted efforts, I failed to find it
anymore;

3. That finding the futility of recovering the said card, it is hereby declared
that the same is considered lost and beyond recovery;

4. That I attest to the veracity of the foregoing statements and hereby execute
this affidavit for the purpose of securing another Employment
Identification Card in lieu of the lost one.

by Saint Marys University (SMU), Bayombong, Nueva Vizcaya;

6. That sometime in September 18, 2017 I noticed that the said card was
detached from my ID holder while I was within the school premises and
despite earnest exerted efforts, I failed to find it anymore;

7. That finding the futility of recovering the said card, it is hereby declared
that the same is considered lost and beyond recovery;

8. That I attest to the veracity of the foregoing statements and hereby execute
this affidavit for the purpose of securing another Employment
Identification Card in lieu of the lost one.
