TOPIC: PROCEDURES OF REPORTING UNSAFE ACT AND UNSAFE CONDITION

OBJECTIVE:

The objectives of this topic are to ensure the trainees understands and be able to
perform reporting procedures in the event of unsafe act and conditions.

1. REPORTING OF ACCIDENTS

All job related accidents resulting in injuries, or occupational illness, must be


reported to immediate supervisor within 24 hours of the accident.

Additionally, accidents resulting only in property damage and incidents of a "near


miss" nature must be reported to immediate supervisor within 48 hours of the
accident or incident.

It is the responsibility of every employee, supervisor and manager to report all


information concerning an accident, or "near miss' incident, within the required time
frame.

Every injured employee shall receive prompt medical attention, which may range
from first aid treatment to hospitalization, depending on the extent of the injury or
illness.

2. PROCEDURE OF REPORTING

Reporting procedure of unsafe condition, unsafe act and accidents start with the employee
recognizing unsafe situations immediately to superior or SHO. Further action will be taken by
superior to ensure the unsafe situation is remedied of attended to by relevant personnel.

2.1 SAFETY AND HAZARDS REPORTING

(a) Safety and Hazard report forms should be completed for all situations.
Completed Safety and Hazard report forms should be sent to superior
or Safety & Health Officer.
(b) Superior or Safety & Health Officer should assign someone to
coordinate, complete forms, and report accidents. Completed accident
forms should be reviewed and signed by the injured employee and the
department, program, or project supervisor. A process for the regular
review of accidents and report forms should be established by the head
of department

2.2 REPORTING REQUIREMENTS FOR HAZARDOUS CHEMICALS

a) Each department will be responsible for compiling and maintaining a


Workplace Chemical List. It should contain the following information:

(i) Chemical Name


(ii) Case Number
(iii) Quantity

b) This information will be reported to the Safety Director by department.


The information will be consolidated and reported via the Safety&
Health Officer who will make the appropriate reports of these chemicals
to the state. This must be updated annually and kept on file for thirty
(30) years

2.3 REPORTING REQUIREMENTS FOR HAZARDOUS CHEMICALS

a) Each department will be responsible for compiling and maintaining a


Workplace Chemical List. It should contain the following information:
(i) Chemical Name
(ii) Case Number
(iii) Quantity

b) This information will be reported to the Safety & Health Office by


department. The information will be consolidated and reported via the
Safety& Health Officer who will make the appropriate reports of these
chemicals to the state. This must be updated annually and kept on file
for thirty (30) years.

2.4 ACCIDENTS/ DANGEROUS OCCURRENCE REPORTING PROCEDURE

(a) An accident is an unplanned and undesired occurrence which results


in injury to a person/persons
(b) A dangerous occurrence is an unplanned and undesired occurrence
which has the potential to cause injury and which may or may not
cause damage to property, equipment or the environment.
(c) Accidents and dangerous occurrences may result from a sequence of
events and circumstances involving a combination of unsafe acts,
unsafe conditions, system failures, human factors and/or omissions.
(d) An employee is required, to promptly report lost time accidents and
prescribed dangerous occurrences to the Safety & Health Officer
(e) In addition to the above, the staffs elected Safety Representatives
have a statutory right to investigate accidents and dangerous
occurrences. Accordingly, the Safety Representatives need to be
advised of such incidents
