Instructions to DOH Deployment Program Applicants

A. Instructions to Applicants taking the Qualifying Exam

Please read these instructions carefully. An applicant who breaches any of the
Examination Regulations will be liable to disciplinary action including (but not limited
to) disqualication from the roll of applicants.

Requirements: PRC ID ( other government issued IDs are allowed for other health
allied professions which doesnt require licensure) & Exam Permit; Black ballpoint pen.

Examination Schedules

Examinations will be conducted during the allocated times shown in the


examination timetable as posted on the DOH website/ DOH Regional Oce VI/
Provincial DOH Oces.
The examination hall will be open for admission 10 minutes before the time
scheduled for the start of the examination. You are to nd your allocated seat but
NOT turn over the question paper/folder/envelope until instructed at the time of
start of the examination.

Personal Belongings

All your personal belongings (such as bags, pouches, ear/headphones, laptops


etc.) must be placed at the designated area outside the examination hall. Please
do not bring any valuable belongings except the essential materials required for
the examinations.
Any unauthorized materials, such as books, paper, documents, pictures and
electronic devices with communication and/or storage capabilities such as tablet
PC, laptop, smart watch, portable audio/video/gaming devices etc. are not to be
brought into the examination hall.
Handphones are not allowed in the examination hall. If your handphone is found
in the examination hall, the handphone will be conscated and retained for
investigations of posibble violation of exam regulations.
Photography is NOT allowed in the examination hall at ALL times.
All materials and/or devices which are found in violation of any examination
regulation will be conscated.
DOH-RO VI will not be responsible for the loss or damage of any belongings in or
outside the examination hall.

At the Start of the Examination

Do NOT turn over the question paper placed on your desk until instructed at the
time of start of the examination.
Please place your identication documents (such as exam permit & identity card)
at the top right corner of your examination desk for the marking of attendance
and verication of identity during the examination.
Please check that you have the correct question paper and read the instructions
printed on your examination question paper carefully.
All examinations are anonymous. Therefore, do not write your name on the
questionnaire. You should write only your examinee & conrmation number,
correctly and legibly, in the space provided on the cover of each questionnaire &
answer sheet. Providing incorrect/illegible examinee number and test question
code could risk your answer sheet being considered void.
During Examination

You are not allowed to communicate by word of mouth or otherwise with other
applicants (this includes the time when answer sheets are being collected).
Please raise your hand if you wish to communicate with a proctor.
Unless granted permission by a proctor, you are not allowed to leave your seat.
Once you have entered the examination hall, you will not be allowed to leave the
hall until your answer sheets were submitted.
If, for any reason, you are given permission to leave the hall temporarily, you
must be accompanied by a proctor throughout your absence from the
examination hall. You are required to leave your mobile/cellular phone on your
desk when you leave the hall temporarily.
All answers, with the exception of graphs, sketches, diagrams, etc. should be
written using black pen. Answers written in pencil will not be marked. The back
pages in the answer sheet are to be used only for examinees rough work.
Solutions or any other materials written on these blank pages will not be marked.
Do not write on, mark, highlight or deface any reference material and
questionnaire provided for the examination. If found doing so, the reference
materials will be removed from your use for the rest of the examination and you
will be made to pay for the cost of the materials that have to be replaced.

At the End of the Examination

You are NOT allowed to leave the examination hall without submitting the exam
sheets & questionnaire. All examinees must remain seated throughout the period
of properly accounting all answer sheets to be collected.
Do NOT continue to write after the examination has ended. You are to remain
seated quietly while your answer sheets are being collected and counted.
No papers, used or unused, may be removed from the examination hall.
You are to stay in the examination hall until the proctor has given the permission
to leave. Do NOT talk until you are outside of the examination hall.
You are responsible to ensure that your answer sheets are submitted at the end
of the examination; otherwise, you will be deemed to have sat for and failed the
examination concerned and this oce would deem the answer sheets as null and
void.
Once dismissed, you should leave the examination hall quickly and quietly, and
remember to take your personal belongings with you.
Please sign the attendance sheets outside the examination hall.

Dress Code

Examinees who are not properly attired will not be admitted to the examination
hall. Please ensure that you comply with the dress code of the Qualifying
Examinations.
Dress code: Modest attire
Spaghetti straps, tube blouses and the like are not allowed.
Shorts, miniskirts and the like are not allowed.

B. Cheating

The DOH Regional Oce VI takes serious view of cheating. All examinees are to take
note of the written examination instructions issued to them as well as the
announcements made during examinations.

An examinee who is suspected of cheating in examinations is liable to disciplinary


action including (but not limited to) disqualication in the program applied for. All
materials and/or devices which are found in violation of any examination rules and
regulations will be conscated.

C. Absence

If you have qualied to take the examination but failed to take the examination, you
shall be deemed to have sat for and failed the examination unless the technical
working group for deployment program nds good and sucient reason for your
absence from the examination.

D. Compassionate Leave

In the event of death in your immediate family (dened to include parents, siblings
and grandparents), you may decide whether to proceed with your scheduled
examination(s). If you decide not to take the examination(s), please inform HRDU of
DOH RO VI. Leave of absence on compassionate grounds will be granted for absence
from the examination(s). The forth letter request for Application for Compassionate
Leave for Examination together with a copy of the death certicate must be submitted
to DOH-RO VI latest by the 2nd working day after the examination period.

E. Supplementary Notes

You are advised to plan your schedule and allow for extra travelling time to attend the
examinations. No extra time will be given for examinees who are late due to trac
jams, bad weather, etc. Applicants who come in after the time the examination started
SHALL NOT BE ENTERTAINED.
