Sir,

I have been from acute knee pain since a long time and my case was referred to the
Citys downtown Hospital on 3.3.2017 from CGHS Wellness Centre no 1. I have been under
treatment of Dr. Nilavjyoti Choudhury of the Downtown Hospital. In spite of medication and
physiotherapy , the position have not been approved and due to pain the day to day movement
is very painful to me .

In my last visit to the Hospital, Doctor advised to take Visco supplementation in both
knees and I have reported the matter to the CGHS Wellness Centre no.1 on 29.08.2017. It was
advised to submit an application to me separately enclosing copies of all related papers to the
Additional Director , CGHS through The Wellness Centre no 1, for the above medicine .

In enclosing the copies of all prescriptions etc I am therefore to request you kindly to
supply the medicine(s) to me through CGHS Wellness Centre 1.

Sub:- Continuation of treatment at Downtown Hospital- regarding.

Sir,
I have been continuously in treatment with the Cardiology , Orthopaedics and ENT
department of Downtown Hospital since CGHS Wellness Centre referred my case there and
Accountant General ordered me take the advice of the relevant department of the Hospital vide
order no Admn II/order no 441 dated 16.12.2016 and no Admn.II/order no 579 dated
28.03.2017.

Since I am still in treatment with the Downtown Hospital and with the CGHS Well ness
Centre for follow up medicine and other diagnostic and pathological treatment etc which is in
record of this office too, I may be permitted to continue the treatment .
