General Application and Admission Guidelines*

General Policy
UERMMMC, Inc. subscribes to a selective but non-discriminatory admission policy in accepting students. All applicants to
the first year level shall comply with the general requirements set by the Admissions Office and specific to the program.
However, the University reserves the right to refuse admission to any applicant who fails to meet the set standards and
requirements.

Certification from Dean/Principals Office that the applicant belongs to the graduating class

Accomplished Secondary Scholastic Form by the Principal/Registrar
Transcript of Records and National Medical Admission Test (NMAT) Report of Percentile Score (valid within the last 2

Transcript of Records and Diploma authenticated by the Philippine Embassy or Consulate in the applicants country (1
original and 1 photocopy)
Two (2) copies of Original Personal History Statement Form accomplished and signed by the applicant in English and in
national alphabet with left and right thumbprints (include personal seal if any)
Notarized Affidavit of Support and proof of adequate financial support
Original and one (1) photocopy of the following:
- Birth Certificate
- Passport
- Police Clearance from the applicants state or origin, authenticated by the Philippine Consulate
Units in an accredited English Proficiency Program

Note: All foreign students, including those with dual citizenship shall be subject to all the policies and rules, and shall
comply with the rules and regulations set by the CHED and the Bureau of Immigration.
Transferees
All applications to transfer to any of the Colleges of UERMMMC, Inc. from other institutions shall be subject to review
and approval by the respective College in which the program is primarily administered and subject to availability of slots.

General Procedure for Admission
Fill up Application Form online, print out, sign and attach listed requirements.
Submit accomplished application form and requirements on/or before the set deadline to the Admissions Office.
Secure payment slip and pay non-refundable and non-transferable Application Fee at the Cashier.
Take the UERM Admission Test (if applicable).
Secure schedule for interview from the Admissions Office. Schedule will also be posted at www.uerm.edu.ph.
Check status of admission at www.uerm.edu.ph. You might also receive SMS or email notice.
Pay the non-refundable and non-transferable reservation fee on the scheduled period. The amount will depend on the
accepting college.
Enroll on the scheduled period of enrolment.

I. GENERAL REQUIREMENTS FOR APPLICATION:

d. Original Certificate of Good Moral Character and 2 (two) letter of recommendation from 2 former
professors.
e. National Medical Admission Test (NMAT) Result
VALID NMAT: October 2016, March 2017, October 2017, and if there are slots
available, March 2018.
f. Birth Certificate authenticated by National Statistics Office (NSO)
g. Marriage Certificate, if married
h. 2x2 Pictures ( 2 Pieces Colored White Background)
i. Application and Processing Fees: Php 3,000.00(NON-REFUNDABLE/NON-TRANSFERABLE)
II. DEADLINE FOR SUBMISSION OF APPLICATION IS JANUARY 15, 2018.
List of Accepted Applicants will be posted on the Bulletin Board of the Office of the Registrar.
III. MINIMUM REQUIREMENTS:
a. Applicant must be a holder of baccalaureate degree;
b. Interview upon submission of required documents (for scheduling); and
c. Applicant must have taken the National Medical Admission Test (NMAT) within the
validity period and must meet a preferred NMAT percentile score of 75. Applicants
who wish to improve their NMAT percentile score may retake the test.

Preliminaries

Fill out the Online Application Form and follow the instructions given.
Pay the Php2,000.00 Admissions Processing fee via inter-branch deposit using any of the following accounts:

3.Submit the following requirements and information together with the proof of payment (validated deposit slip) to the
Lasallian Access & Success Office (LASO) thru email to:

4. Wait for the email / call from the College of Medicine for your skype interview schedule.
5. Undergo interview with the College of Medicine Collegiate Admissions Committee.
6. Wait for the status of application. A Notice of Acceptance from the College of Medicine shall be issued to qualified
applicants.

Confirmation
Pay the Php35,000.00 reservation fee should the applicant pass all the criteria for screening and evaluation (the
applicant shall be given an enrollment permit by the Lasallian Access & Success Office upon submission of the
requirements). The reservation fee is non- refundable. However, it shall be deducted from the tuition fees should the
applicant decide to enroll in the Institute.
Physical Examination
Secure Medical Exam schedule from the Lasallian Access & Success Office.
Undergo physical examination as scheduled by the Lasallian Access & Success Office.
Enrollment
Submit all the following requirements to receive Enrollment Permit from the Lasallian Access & Success Office:
Notarized Sworn Statement (you can get Notarized Sworn Statement Form at Lasallian Access & Success Office)
Transfer Credential / Honorable Dismissal from previous school
Original NMAT Result
Present the enrollment permit to the Registrars Office.
Enroll.

Extended application for MCAT


The PLM College of Medicine freshman application, otherwise known as MCAT, is extended up to Thursday, March 16,
4pm.
The MCAT fee of P3,500 may be paid not later than Friday, March 17, 4pm.
The exam will be on March 19, Sunday, 7:30am. Pls. come early.
Please be guided accordingly.

Qualifications for admission

The applicant must be a graduate of a Bachelor of Arts or Science (AB, BS) college program. The conferment
of this degree must be before August 1, 2018. It is highly recommended that the applicant have a strong
foundation in the biological sciences.

The applicant must take the National Admission Test (NMAT). Scores are valid for only two years.

Important Reminders
All requirements listed above are for application purpose. Additional requirements may be called for at the
enrollment phase.
Submission of fraudulent documents shall cause the Admission Committee to withdraw the
application/acceptance.
All requirements with a single asterisk (*) must be authenticated by the Philippine Consulate Office or
Embassy and bear the red ribbon of the consulate at the enrollment phase.
A photocopy of Marriage Certificate is required for married female applicants.
TOEFL/ESL for non-English speaking students may be required upon interviewers discretion.
All Requirements must be secured in long brown envelope, together with your printed application form
with signature upon submission.

PROCEDURE

Fill out the Application Form in this website. Once information is accurately completed click on the Submit icon. An
Application Number will be given to you. You will use this number for all further transactions in the application process.
Pay the application fee at any Rizal Commercial Banking Corporation (RCBC) branch or at the Accounting Office located
at the Lower Ground Floor of the Institute of Medicine Building. This application fee is non-refundable.
Local Citizen Graduate - Php 1,500.00
Foreign Citizen Graduate - USD 400.00
Dual Citizen Graduate - USD 400.00

Place required documents in a long brown envelope, together with your printed application form with signature and
submit at the Admissions Office located at UG3, Institute of Medicine Building. Submission may be in person or by
courier.
Qualified applicants will be invited for interview.
Only when the required documents and proof of payment are submitted will the application be reviewed.

Thank you for expressing your interest in enrolling in the San Beda College of Medicine. Please read through the
following:

A. Application Requirements:
1. Applicants must be a graduate of aBaccalaureate program or a candidate for graduation of a Baccalaureate program
by the end of the second semester preceding the application.

2. Applicants must have a valid National Medical Admission Test (NMAT) percentile rank of at least 40.
3. Applicants must submit the required documents to the San Beda College Admissions and Placement Center (APC)
placed in 1 long brown envelope:

a. Accomplished application form (forms are available at the APC or may be downloaded fromthe official website:
www.sanbeda.edu.ph);

b. Photocopy of Transcript of Records (TOR) of the last 3 years of any accredited Baccalaureate program;

c. For graduates of a Baccalaureate degree, official TOR must be submitted;

d. Certificate of Class Ranking;

e. Certificate of General Weighted Average (GWAG);

f. Photocopy of National Statistics Office (NSO) issued Birth Certificate;

g. Two (2) Recommendation Letters (from former professors and/or Dean of the institution where the baccalaureate
program was taken dated within the last two years);

h. Two (2) Certificates of Good Moral Character(from former professors and/or Dean of the institution where the
baccalaureate program was taken dated within the last two years);

i. Four (4) copies of a recent 2x2 colored photo with white background.

4. An application fee of P 1,500.00 shall be charged for processing and entrance examination.

B. Application Guidelines:
1. Upon submission of the required documents, the APC will schedule the entrance examination.

2. Applicants will be informed of the schedule of interview through contact information provided in the application
form.

a. An INITIAL AND PARTIAL RELEASE of accepted applicants will be announced on April 28, 2017.Applicants NOT
INCLUDED in this initial list are still being considered for admission. Accepted applicants will be given until May 15, 2017
to pay the reservation fee and/or enroll for the incoming academic year otherwise they will be wait-listed.

b. The OFFICIAL LIST OF ACCEPTED APPLICANTS shall be released by the second half of May 2017. Accepted applicants
will be given two (2) weeks from the posting of the list to reserve and/or enroll for the incoming academic year
otherwise they will be wait-listed.

c. The list of successful applicants will be posted here.

3. The list of wait-listed applicants will be posted by June 2017.

C. Requirements for accepted applicants:


1. Pay a non-refundable reservation fee of P 30,000.00.

2. Submit the original copies of all required documents mentioned above.

3. Submit Honorable Dismissal from your previous school.


4. Submit a medical clearance certificate and copies of the following laboratory exams from any reputable hospital or
clinic:

a. CBC

b. Urinalysis

c. Chest x-ray PA view (please bring film)

5. Submit two (2) copies of a recent 2x2 colored photo with white background.

Application deadline for academic year 20172018 is on April 29, 2017.
