Application Instructions:
Option 1:
Print this document, fill out by hand, sign, than mail back to the address below.
Please send all pages back.
Option 2:
Open in Adobe Reader, fill out all fields. (Minimum version 8 required)
To sign, click “Fill & Sign” in the top right corner of Adobe Reader, than click “Place Signature”.
You will see a pop-up box.
Enter your name than click “Accept”.
Your cursor will then turn to your signature.

Wherever you click is where your signature will be placed.
(Although you can move it if it’s in the wrong spot)
Scroll to the locations where your signature is required (Page 5 and 6).
Hover over the signature field and “click” to place your digital signature.
Do this for both signature fields.
Also for Initials field. (Page 6) You create an initial the same way.

AND IN MY PETS BEST INTEREST BY OBTAINING VETERINARY CARE AT MY EXPENSE IF DEEMED NECESSARY FOR ILLNESS OR
INJURY. I FURTHER AGREE TO PAY ALL VETERINARY AND OTHER NECESSARY SERVICES INCURRED BY AND FOR MY PET DURING
ITS STAY IN THIS FACILITY. THIS BOARDING FACILITY AGREES TO EXERCISE ALL DUE AND REASONABLE CARE TO PREVENT
INJURY OR ILLNESS TO MY PET. HOWEVER, IN THE EVENT OF ILLNESS, INJURY OR DEATH THE OWNERS AND EMPLOYEES OF THIS

PRICES ARE PER NIGHT. NO DROP-OFFS OR PICK-UPS BEFORE
8AM OR AFTER 6:00PM. NO DROP-OFFS OR PICK-UPS ON
SUNDAYS AND HOLIDAYS.
Special pick up arrangements can be made IF our staff can
accommodate the request, for an ADDITIONAL fee of $50.00.
Option 2 - “Training REFRESHER w/Boarding” - $550.00 Per Week (1 week min)

To include refresher training with boarding, your dog MUST be
boarding with us for a minimum of one week and MUST have
participated in one of our Full Service Programs.

IS HEREBY GRANTED A LIEN ON THE PET(S). I UNDERSTAND THAT ANY ANIMAL LEFT FOR TEN DAYS
BEYOND THE AGREED DATE OF PICK UP MAY BE SOLD OR RE-HOMED AT THE DISCRETION OF THE KENNEL
OWNER.
THIS AGREEMENT SHALL APPLY TO ALL FUTURE SERVICES FOR MY PETS.
