BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS

1.) Mobilization of Manpower & Equipment
 Finger injury. Body being caught in between or
struck by materials.
 Ensure all materials shall be stored, handled and used in
safe manner, by type, providing sufficient access/egress
 Employee shall be trained for using safe methods in
handling materials.
 Leather hand gloves to be worn.
 Ensure all the material fully secured by competent and
certified rigger.
 Physical barriers to be erected and warning signs to be
displayed.
 Trained banks man with high visibility jacket & whistle.
 Make sure that rigger is always present during off-loading
materials or handling by crane
 Slings are checked for properness and certificates are
available.
 Make sure that banks men are present with flags in hand.
 Competent Supervision.
 Workforce training in the selection & use of tools, material
& equipment.
 Inspection of all equipment prior to use (including crane &
lifting gear).
 All personnel to undergo TBT briefing, and PTW (where
required).

3.) Erection of Scaffolding to Access the Front and Rear
Cover of Cooler
 Ensure that all materials are properly secured by means of
correct sling belt to prevent materials from falling during
transportation.
 No overloading of materials on the trucks.
 Follow general guidelines in method statement.
 Competent Rigger to handle the activity.
 Manual handling training.
 Keep all body parts out of the line of fire.
 Barricading & signage
 Ensure valid vehicle driving license
 Observe proper traffic control and speed limit
 Equipment or material should be properly secured
 Warning signs shall be erected as means of warning where
applicable.
 Properly secure the equipment and material
 Ensure right position of the equipment and material is
properly set on the vehicle
 No person allowed to sit/present on the back of truck lorry
 Vehicle maneuvering unaided shall never be allowed
 All scaffolding material should be hoisted by manila rope
and pulley wheels.
 Loose scaffolding clamps shall be hoisted with the help of
tool bags.
 Area survey to be conducted before scaffolding erection
 Ground to be compacted properly
 Use proper base / sole plate.
 ICOM scaffolding inspector should inspect and approve
scaffolding and tagged near access point at eye level
 Scaffolding will be inspected again in case of modification
and every week.
 Maintain good housekeeping at all time to prevent slip, trip
and falls.
 Scaffolding material should be certified.
 Only experienced, third party certified and competent
scaffolders and supervisors to be employed.
 Install complete guard rails
 Area to be barricaded, vehicle will only be allowed with
permit and their movement should have controlled by
trained banks man
 Necessary signboard to be arranged during the erection and
dismantling of scaffolding.
 All areas, which require scaffolding should be mapped and
numbered. Scaffolding tags to be numbered in accordance
with mapping
 No scaffolding platforms are to be modified by
unauthorized personnel, except a competent Scaffolder

moving it to secure place
 Sufficient Barrication, Restrict entry for Unauthorized
persons in Jetting Area
 Mandatory PPE’s: Full Face Shield with Hard Hat, Chemical

 Strict No Entry Sign Board and Hard Barrication to Restrict
Entry on the opposite site of the tube being Hydro jetted.
 Job to be started only after prior instruction from site
Supervisor to Jetter regarding Max. Pressure, Safety
Requirements and Work Permit Information.
 Mandatory Tarpaulin Barrication to prevent any projectile
materials to come out of Job site.
 Compulsory Whip check on every Pressurized Hose
Connections
 Either Parking brake or Mechanical Strain to be used to Park
Equipment
 Fire Extinguisher to be placed near to the Equipment
 Gas Test to be carried out by Client prior to Starting of the
I.C Engine Operated Equipment
